X-linked hypophosphatemia and spinal cord compression: a systematic review and illustrative case.

STUDY DESIGN: Systematic review. OBJECTIVE: X-linked hypophosphatemia (XLH) is a rare genetic disorder characterized by impaired phosphate homeostasis due to renal phosphate wasting. It leads to osteomalacia and skeletal abnormalities and represents the most common inherited cause of vitamin D-resistant rickets. In some patients, heterotopic ossification of the ligamentum flavum and paravertebral ligaments may result in spinal cord compression and myelopathy. Due to its rarity, large case series evaluating patient characteristics and surgical outcomes are lacking. METHODS: We conducted a PRI

Phosphaturic Mesenchymal Tumour of the Soft Tissue in the Finger Presenting without Tumour-Induced Osteomalacia: A Case Report.

Phosphaturic mesenchymal tumour (PMT) is a rare neoplasm that can occur with or without tumour-induced osteomalacia (TIO). While PMTs commonly arise in the lower extremities or head and neck, digital involvement is rare. A 70-year-old man presented with a 12-year history of an enlarging subcutaneous mass on the dorsal aspect of his left ring finger. Imaging studies suggested an organised thrombus within a venous malformation. Marginal excision was performed, and histopathological examination revealed a lesion composed of capillaries and small vessels surrounded by spindle cells with small ovoi

Hypophosphatemic osteomalacia caused by diffuse tenosynovial giant cell tumor misdiagnosed as spondyloarthritis: a case report.

Tumor-induced osteomalacia (TIO) is an acquired hypophosphatemic osteodystrophy caused by increased renal phosphate excretion due to tumors. Tenosynovial giant cell tumor (TGCT) is a benign mesenchymal tumor but rarely causes hypophosphatemia. TIO is a clinically rare disease, and its clinical manifestations often overlap with rheumatic diseases such as spondyloarthritis (SpA), leading to misdiagnosis. A 32-year-old female had a prolonged misdiagnosis of undifferentiated spondyloarthritis due to chronic low back pain, limb weakness, and spinal deformity, with poor response to non-steroidal ant

From Routine Supplementation to Targeted Repletion: Vitamin D Deficiency in UVB-Constrained, Pollution-Exposed, and Ageing Populations-Musculoskeletal and Extra-Skeletal Perspectives.

Background/Objectives: Vitamin D deficiency remains common in UVB-constrained, pollution-exposed, sedentary, and ageing populations, yet recent guidelines discourage routine screening and empiric high-dose supplementation in generally healthy adults. This review clarifies the clinical distinction between untargeted population supplementation and monitored repletion of documented deficiency in high-risk individuals. Methods: We conducted a focused narrative review of PubMed/MEDLINE, Scopus, Web of Science, and the Cochrane Library. The focused evidence search supporting the narrative synthesis

Low Prevalence of Exploratory Biochemical Patterns Compatible with Osteomalacia in a Large Cohort of Participants with Low Serum 25-Hydroxyvitamin D Levels.

The threshold for vitamin D deficiency is mainly based on osteomalacia prevention. Osteomalacia is diagnosed by bone biopsy, but may potentially also be identified biochemically. Our objective was to examine the prevalence of exploratory biochemical patterns compatible with osteomalacia in a population with low vitamin D status, as well as the effect of vitamin D supplementation on the biochemical parameters. The study was cross-sectional followed by a randomized controlled trial (trial registration number: NCT02750293). The participants were recruited from the population-based Tromsø study. P

Biochemical phenotype of hypophosphatasia in asymptomatic individuals carrying ALPL variants.

Hypophosphatasia (HPP) is the rare metabolic disorder caused by variants in the ALPL gene, resulting in deficient activity of tissue-nonspecific alkaline phosphatase (ALP). This leads to accumulation of substrates contributing to impaired bone mineralization. Hypophosphatasia manifests with a broad clinical spectrum; however, an increasing number of individuals with ALPL variants have been identified presenting the hallmark biochemical feature of HPP of low serum ALP activity, with or without elevated serum pyridoxal-5-phosphate (PLP) or urine phosphoethanolamine (PEA), while remaining asympto

Vitamin D and Fracture Healing.

Maintaining bone health involves complex interactions among nutrients, hormones, and physiological mechanisms, with vitamin D playing a pivotal role. Vitamin D is essential for the absorption of calcium and phosphate, which are critical minerals for bone development, maintenance, and repair. Disruptions in mineral ion homeostasis can lead to conditions such as hypocalcemia, osteoporosis, osteomalacia, and rickets, which are prevalent worldwide. Inadequate vitamin D impairs bone health, increasing the risk of fractures and catabolic bone diseases. This chapter elaborates on the synthesis, activ

Expanding the Autosomal Recessive Hypophosphatemic Rickets Type I Carrier Phenotype and Adult Treatment with Burosumab.

Autosomal recessive hypophosphatemic rickets type 1 (ARHR1) is a rare, inherited cause of fibroblast growth factor 23 (FGF23)-mediated renal phosphate-wasting that results from homozygous inactivating pathogenic variants in the dentin matrix acidic phosphoprotein-1 (DMP1) gene. A variable clinical spectrum has been described in this condition, including features of rickets (in children), hyperostotic osteomalacia (in children and adults), bone pain, short stature, lower limb deformities, recurrent dental abscesses, and hearing defects. The purpose of this report is to describe the clinical evo

Burosumab treatment in an adult with FGF23-mediated hypophosphatemia due to cutaneous skeletal hypophosphatemia syndrome.

CONTEXT: Cutaneous skeletal hypophosphatemia syndrome (CSHS) is an ultrarare disorder defined by epidermal and/or melanocytic nevi, mosaic skeletal dysplasia, and FGF23-mediated hypophosphatemia. As in other FGF23-mediated hypophosphatemia conditions, individuals with CSHS have renal phosphate wasting and inappropriately normal or frankly low 1,25-dihydroxyvitamin D levels with resultant hypophosphatemia leading to rickets and osteomalacia. Conventional therapy for FGF23-mediated hypophosphatemia consists of multiple daily doses of oral phosphate and active vitamin D. OBJECTIVE: Burosumab is

Tumor induced-osteomalacia caused by a phosphaturic mesenchymal tumor in the sartorius muscle: a case report.

Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome resulting from excessive secretion of fibroblast growth factor-23 (FGF23) by phosphaturic mesenchymal tumors (PMTs). We report the case of a 35-year-old man with a three-year history of progressive musculoskeletal symptoms and severe mobility impairment, who had not received a correct diagnosis during this period. Laboratory evaluation revealed severe hypophosphatemia and elevated FGF23 levels. Magnetic resonance imaging (MRI) and 68Ga-DOTA-TATE PET/CT identified a metabolically active lesion in the sartorius muscle. Surgical r

Insight into Natural History and Phenotype in Untreated Adults with X-Linked Hypophosphatemia.

X-linked hypophosphatemia (XLH) is a rare genetic rachitic/osteomalacic and dental disorder caused by pathogenic variants in PHEX gene resulting in fibroblast growth factor 23 (FGF23) excess leading to hypophosphatemia by renal phosphate wasting and decreased 1,25-dihydroxyvitamin D production. The aim of the study was to provide insight into natural history and phenotype in untreated adults with XLH (no phosphate supplements and active vitamin D metabolites or burosumab). Clinical, biochemical, skeletal features, co-morbidities, and patient-reported outcomes (PROs) were examined in 52 patient

Vitamin D in the UK: an urgent call to redefine the threshold for deficiency.

ACT NOW Vitamin D is a multidisciplinary collective tackling persistent public health concerns in the UK, including widespread vitamin D deficiency, nutritional rickets in children, and osteomalacia across the lifespan. National Diet and Nutrition Survey data show significant declines in vitamin D intake - especially among lower intake percentiles - and rising rates of severe vitamin D deficiency (serum 25-hydroxyvitamin D <25 nmol/L) among people aged 4-64 years. Contributing factors include reduced meat, fish and seafood consumption, increasing obesity, and growing high-risk ethnic minority

An educational intervention for unpaid/family carers of patients with breathlessness in advanced disease: developing a web-based resource.

BACKGROUND: Chronic refractory breathlessness is distressing, disabling, and common in advanced disease. Unpaid/family carers provide support for patients, but often lack knowledge and confidence, experiencing anxiety which can lead to crises. The Learning about Breathlessness programme (LaB) sought to address this. Working with carers and clinical experts, the LaB1 Study identified six topics carers of patients with breathlessness due to cancer or chronic obstructive pulmonary disease (COPD) want to learn about breathlessness, and how they want to learn. To accommodate carers' varied learning

Unveiling the biochemical signature of tumor-induced osteomalacia: implications for distinguishing it from primary osteoporosis.

PURPOSE: Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome caused by FGF23-secreting tumors, resulting in renal phosphate wasting and hypophosphatemic osteomalacia. Low bone mineral density (BMD) in TIO frequently leads to misdiagnosis as the more prevalent primary osteoporosis (POP) and subsequent diagnostic delay. This study characterizes the distinct biochemical profile of TIO and proposes a stepwise diagnostic approach to differentiate it from POP for timely clinical identification and appropriate management. METHODS: We conducted a retrospective comparative analysis of 1

Coexisting paraneoplastic SIAD and tumour-induced osteomalacia in stage IV small cell lung cancer: a diagnostic challenge.

A 70-year-old man with stage IV (extensive-stage) small cell lung carcinoma presented with progressive respiratory symptoms, refractory hypophosphataemia and hyponatraemia. The diagnostic evaluation revealed paraneoplastic arginine vasopressin-mediated syndrome of inappropriate diuresis and fibroblast growth factor 23-mediated renal phosphate wasting, consistent with tumour-induced osteomalacia. This report highlights the diagnostic complexity and reasoning required to identify dual paraneoplastic syndromes in an advanced malignancy, with emphasis on biochemical interpretation and integration
