Applications of ZIF-8-Based Nanoplatforms in Orthopedic Diseases: Evidence Base, Mechanistic Attribution, and Translational Barriers.

Orthopedic diseases severely impair the quality of life of hundreds of millions of people worldwide and impose a substantial socioeconomic burden. Conventional treatments remain limited in terms of disease-site targeting, sustained drug release, and mitigation of adverse effects. Zeolitic imidazolate framework-8 (ZIF-8), formed through the coordination of Zn2⁺ with 2-methylimidazole, has a porous structure, a high cargo-encapsulation capacity, and environment-dependent disassembly characteristics. Through the release of Zn2⁺, it may also contribute to osteogenesis, antibacterial activity, and

Circulating Natural Killer Cells in Patients With Type 2 Diabetes and Peripheral Neuropathy Are Altered in Their Frequency and Exhibit Impaired Functions and Angiogenic Activities.

Patients with type 2 diabetes mellitus (T2DM) and critical limb-threatening ischemia (CLTI), a severe vascular complication associated with amputation and high mortality, exhibit chronic inflammation and endothelial dysfunction. We previously reported an increased frequency of natural killer (NK) cells in advanced CLTI; however, whether NK cell alterations are already present in patients with peripheral neuropathy, which often precedes diabetic foot lesions with or without distal macroangiopathy, remains unclear. We conducted an observational pilot study to assess circulating NK cell frequency

Disseminated MDR tuberculosis manifesting as Pott's disease with tuberculous septic ankle arthritis: an uncommon sequential dual-site involvement.

Disseminated osteoarticular TB is uncommon, and the occurrence of multidrug-resistant (MDR) strains impacting many skeletal locations in an immuno-competent individual is very rare. A 25-year-old male from a TB-endemic rural region is presented, having a three-month progression of worsening low back pain, subsequently accompanied by lower limb weakness. GeneXpert from both sites detected Mycobacterium tuberculosis that was resistant to rifampicin. The line probe assay detected resistance to rifampicin, high-level resistance to isoniazid, and resistance to ethambutol. After surgical procedures

The microbiome bone axis in osteomyelitis: integrating evidence from bone oral and gut microbiota.

Osteomyelitis is an inflammatory bone disorder that includes both infectious and noninfectious forms and remains a major cause of bone destruction, impaired skeletal repair, and long-term disability. Although bacterial infection is the predominant cause, disease progression is influenced by interactions among microorganisms, host immunity, inflammation, and bone remodeling. This review critically evaluates current evidence from genome-wide association and Mendelian randomization studies, clinical investigations, and experimental animal models. Clinical studies demonstrate that osteomyelitic bo

Pharmacokinetic/pharmacodynamic evaluation of multidose oritavancin for prolonged durations of therapy.

Managing multidrug-resistant gram-positive infections is challenging, especially when prolonged therapy is needed for conditions like bacteremia, endocarditis, osteomyelitis, or other deep-seated infections. Oritavancin, a long-acting lipoglycopeptide, is increasingly used in these scenarios and offers an alternative to central catheter placement. However, despite its growing use for extended treatment, optimal dosing strategies and pharmacokinetic targets remain unclear. An in silico pharmacokinetic/pharmacodynamic simulation was performed to assess the predicted oritavancin concentration res

Short-course versus long-course antibiotics in bone infections: a meta-analysis of randomised controlled trials.

PURPOSE: The optimal duration of antibiotic therapy for bone infections remains controversial. We therefore conducted a meta-analysis of randomised controlled trials (RCTs) comparing short- versus long-course antibiotic therapy for bone infections. METHODS: PubMed, Embase, Cochrane Library, Web of Science, and ClinicalTrials.gov were searched through February 2026 for RCTs comparing short- and long-course antibiotic therapy in bone infections, including periprosthetic joint infection, diabetic foot osteomyelitis, vertebral osteomyelitis, and paediatric acute haematogenous osteomyelitis. Pediatric Rosai-Dorfman-Destombes Disease: From Isolated Cervical Lymphadenopathy to Multisystem Disease.

Rosai-Dorfman-Destombes disease (RDD) is a rare non-Langerhans cell histiocytosis with a broad clinical spectrum, ranging from localized lymphadenopathy to severe multisystem involvement. Due to its rarity, clinical data on pediatric RDD remain limited, particularly in North Africa. We report two contrasting pediatric cases of RDD managed at a tertiary center in Morocco to illustrate this clinical heterogeneity. The first was that of a five-year-old girl who presented with chronic, isolated, painless cervical lymphadenopathy. Laboratory findings were normal. Biopsy confirmed RDD with classic e

From Heart Murmur to Bone Involvement: Tricuspid Valve Endocarditis With First-Rib Osteomyelitis in a Patient With Methicillin-Resistant Staphylococcus aureus (MRSA) Bacteremia.

Tricuspid valve infective endocarditis (TVIE) is most commonly associated with intravenous drug use and Staphylococcus aureus bacteremia. While right-sided endocarditis may present with septic pulmonary emboli, hematogenous spread to the bone is a rare but serious complication. Among osseous sites, first-rib osteomyelitis is particularly uncommon and may be underrecognized in patients presenting with localized chest pain or swelling. We present the case of a 37-year-old woman with intravenous substance use who presented to the emergency department with left-sided upper chest pain, swelling, an

Diagnostic uncertainty in a patient with severe hidradenitis suppurativa revealing Buschke-Löwenstein tumor.

Buschke-Löwenstein tumor (BLT), also known as giant condyloma acuminatum, is a rare human papillomavirus-associated lesion characterized by locally aggressive growth with the potential for malignant transformation. Diagnosis may be particularly challenging when BLT arises in the setting of severe chronic inflammatory disease. We present a 44-year-old man with severe hidradenitis suppurativa, well-controlled HIV infection, recurrent gluteal abscesses, and chronic sacral osteomyelitis who presented with worsening perineal pain and chronic drainage. Although imaging demonstrated extensive infiltr

Hemipelvectomy in paraplegic patients with chronic osteomyelitis.

INTRODUCTION: Hemipelvectomy - surgical removal of half of the pelvis including the ipsilateral limb, is a rare operation with a narrow indication spectrum, most often performed for malignant tumors in the proximal femur area with cranial spread. Other, less frequent indications include devastating trauma to the pelvic area and deep soft tissue infections involving the pelvic bones. In our two case reports, we present deep decubitus ulcers complicated by osteomyelitis of the pelvic bones in paraplegic patients. Case report 1: We describe the case of a 33-year-old paraplegic who underwent right

Locomotor Health in Modern Broiler Chickens: Risk Factors, Major Disorders, Assessment Methods, and Implications for Welfare and Production Efficiency.

Genetic selection for rapid growth, improved feed efficiency, and increased breast-muscle yield has substantially enhanced broiler production; however, it has also increased the functional demands placed on the locomotor system. This review critically examines locomotor health in modern broiler chickens in relation to welfare and production efficiency. Particular attention is given to the multifactorial interactions among genotype, growth rate, body weight and conformation, skeletal development, nutrition, physical activity, housing conditions, age, and infectious factors. Major locomotor diso

Overcoming Therapeutic Bottlenecks of Infected Bone Defects via 3D-Printed Scaffolds Integrated with Antibacterial, Osteogenic and Angiogenic Synergistic Functions.

Infected bone defects (IBDs) triggered by open fractures, implant-related infections, chronic osteomyelitis and bone tumor resection remain intractable orthopedic complications, contributing to elevated disability rates, recurrent infection, prolonged treatment courses and heavy social-economic costs. Conventional treatments such as bone grafting and antibiotic-loaded bone cement are plagued by donor shortage, immune rejection, uncontrolled antibiotic leakage and poor anatomical matching, incapable of synchronously eradicating bacteria, inducing osteogenesis and reconstructing blood vessels.
