Cytomegalovirus (CMV) infection, though usually systemic, has been known to cause localised involvement of organs like lung, liver, testis and gastrointestinal tract. We report a case of cytomegaloviral infection involving the epididymis without systemic manifestations in an young male one month after renal transplantation. The diagnosis was made on histopathologic examination of the epididymo-orchidectomy specimen. Clinical improvement occurred after the emergency epididymo-orchidectomy. Nested polymerase chain reaction was used for rapid detection of human cytomegalovirus in blood samples of patients subjected to bone marrow transplantation. Use of the four-primer system appreciably improved the sensitivity and specificity of the method and helped eliminate the difficulties arising in the detection of various viral strains with the minor nucleotide substitutes in the DNA template. The proposed method is a reliable tool for the early diagnosis of cytomegaloviral viremia and for monitoring the efficacy of antiviral therapy.

A 32-year-old man was admitted after bone marrow transplantation because of hematochezia. He had history of chronic graft-versus-host disease (GVHD) of the skin and the liver, and cytomegaloviral pneumonia. Barium enema and colonoscopy showed multiple colon ulcers in the ascending and transverse colon. This feature is very rare in chronic GVHD and resembles the feature in autoimmune disease such as periarteritis nodosa. Thus, this ulceration is thought to be caused by vasculitis due to an autoimmune reaction in chronic GVHD.

We retrospectively reviewed parainfluenza and influenza virus infections that occurred in pediatric organ transplant recipients at our hospital from January 1985 through September of 1992. Cytomegalovirus infection, commonly observed in immunodepressed patients, raises a number of problems both in clinical practice (early rapid diagnosis, therapeutic options) and biological research (resistant strains, mutations). The classical methods of diagnosis do not provide specific information for decision making in each particular clinical situation. We report a case of cytomegalovirus infection in a renal transplant recipient in which quantitative cytomegalovirus antigen level in leukocytes provided rapid early diagnosis of the primary infection and two episodes of recurrent infection.

Spontaneous pneumopericardium occurred in a patient almost 4 weeks after bilateral lung transplantation for cystic fibrosis. The patient had no specific complaints and was in stable haemodynamic condition. We suggest that this pneumopericardium was related to a concomitant active cytomegalovirus (CMV) infection. After treatment of the CMV infection, the pneumopericardium resolved spontaneously.

We report our experience with 25 living-donor and 427 cadaveric-donor kidneys followed for over 6 years. Patient survival exceeds 90% at 5 years for all recipients. Overall one-year and 5-year graft survival rates were 82.7% and 65.5%, respectively. Approximately one-third of CAD were of extreme age (eg, < or = 10 and > or = 60 years old).
