Disitamab vedotin for HER2-positive breast cancer with liver metastases: phase 3 results from the RC48-C006 phase 2/3 trial.

Patients with HER2-positive advanced breast cancer with liver metastases (BCLM) have poor prognosis. Study RC48-C006 is a prospective, open-label, multi-center, randomized phase 2/3 trial (ClinicalTrials.gov identifier, NCT03500380). Based on the phase 2 results, the phase 3 stage was initiated to evaluate disitamab vedotin (DV) versus lapatinib plus capecitabine (L + C) in patients with HER2-positive BCLM who were previously treated with trastuzumab and taxanes. The results from the phase 3 stage are reported in this article. A total of 104 patients were randomized at 1:1 to receive DV (53 pa

Isolated Proximal Femoral Valgus Osteotomy for Hinge Abduction Improves Femoroacetabular Relationship in Patients With Perthes Disease.

BACKGROUND: Proximal femoral valgus osteotomy is a recognized salvage procedure for managing hinge abduction in Perthes disease. Although proximal femoral valgus osteotomy aims to improve joint congruency, previous studies have been limited by heterogeneous methodologies and concomitant procedures, making it difficult to determine its isolated effect. Furthermore, no study of which we are aware has specifically examined how proximal femoral valgus osteotomy influences acetabular development or lower limb alignment over time. QUESTIONS/PURPOSES: (1) Is isolated proximal femoral valgus osteotom

Atopy is associated with osteoarthritis severity: results from the KHOALA cohort.

OBJECTIVE: Atopy has been identified as a risk factor for osteoarthritis, but without specifying its impact on disease severity. We sought to investigate the association between atopy and the symptomatic and structural severity of knee and hip osteoarthritis. METHODS: We used data from KHOALA, a French multicentre cohort of patients with knee and/or hip osteoarthritis (OA) investigating "atopic" subjects, defined as those with history of asthma and/or atopic dermatitis (AD) compared to "non-atopic" subjects. We compared: WOMAC pain, stiffness and function scores; and the proportion of having

A Comparative Study of Generative Artificial Intelligence Tools for Human Bone Learning.

The aim of this study was to evaluate the effectiveness of three different generative AI tools, ChatGPT-4o, Claude 3.7 Sonnet, and Gemini 2.0 Flash, with a specific focus on their accuracy and response consistency in supporting self-directed learning in human skeletal anatomy. A total of 143 human skeletal specimens were selected for evaluation. Bone specimens from different donors were photographed to represent each structure, resulting in a total of 715 images. Four types of questions were generated to assess each AI model's ability to identify anatomical features. Responses were categorized

ERK-Mediated Phosphorylation of YAP Defines a Noncanonical FGF Signaling Mechanism in Stem Cells.

While Fgf and Hippo-Yap signaling are fundamental for proper development, homeostasis, and disease, their crosstalk remains largely unknown. Here, we identified that Yap and Taz, canonical Hippo effectors, function as noncanonical effectors of Fgf signaling to maintain the proper function of neural crest (NC) lineages. NC cells are a multipotent stem cell population during vertebrate embryogenesis that contribute to numerous structures and diverse cell lineages, including craniofacial and cardiac tissues, neurons, and suture mesenchymal cells (SMCs), a specified cell population required for cr

Parathyroid hormone and alkaline phosphatase are associated with fracture risk in non-dialysis-dependent chronic kidney disease.

UNLABELLED: We studied over 45,000 Danish adults with non-dialysis-dependent chronic kidney disease to assess whether parathyroid hormone (PTH) and alkaline phosphatase (ALP) were associated with fracture risk. Elevated PTH (over twice normal) and ALP (above 70 U/L) were linked to higher fracture rates, suggesting these markers may help identify high-risk patients. PURPOSE: Patients with chronic kidney disease (CKD) have an increased risk of bone fracture. We aimed to examine the association between parathyroid hormone (PTH), alkaline phosphatase (ALP), and fracture risk in non-dialysis-depen

Chronic Plaque Psoriasis as a Predictor of Spinal Osteoproliferation on Magnetic Resonance Imaging (MRI).

Chronic plaque psoriasis is a systemic inflammatory condition that extends beyond the skin and has been linked to musculoskeletal changes, including excessive bone formation along the spine. Emerging evidence indicates that individuals with psoriasis are more likely to demonstrate spinal bony overgrowth than those without the disease. Although biologically plausible mechanisms have been proposed to explain this relationship, there is currently no clear framework for translating the association into clinical decision-making. Existing practice recommendations do not specify which patients might

Evidence on the Relationships Between Total Fat Intake and Health Outcomes Other Than Unhealthy Weight Gain in Adults and Children: A Scoping Review.

INTRODUCTION: Global guidance provides evidence-informed recommendations on total fat intake to prevent unhealthy weight gain in adults and children, however, evidence regarding total fat intake and other health outcomes, particularly non-communicable diseases (NCDs), also exists. This scoping review aimed to map and identify gaps in this body of evidence. METHODS: Four databases were searched from 2014 to 2024 for published systematic reviews (SRs), in any population, assessing the effects or associations of reduced total fat intake (e.g., ≤ 30% total energy [TE]), compared with usual fat in

A controllable human spinal cord model with full dorsoventral patterning.

Dorsoventral (DV) patterning of the spinal cord (SC) is orchestrated by morphogen gradients that specify distinct neural progenitor domains, the dorsal roof plate (RP), ventral floor plate (FP), and delaminating neural crest cells (NCCs). While foundational insights into SC patterning have been gained from animal models, key aspects - such as the role of retinoic acid (RA), dynamics of NCC lineage development, and human-specific features - remain poorly understood due to limitations in existing in vivo and in vitro models. Here, we present a human pluripotent stem cell (hPSC)-derived SC model,

Post-operative survival and use of systemic therapy in metastatic long-bone disease: 12 years of institutional experience.

BACKGROUND: Survival prediction in metastatic long-bone disease is essential for surgical decision-making, yet the performance of legacy prognostic models has declined in the era of immunotherapy and molecularly targeted agents. As these systemic therapies become routine, treatment patterns themselves may influence survival in ways not captured by earlier models. We, therefore, quantified preoperative systemic therapy use over the last decade and evaluated its association with postoperative overall survival in patients undergoing surgery for long-bone metastases. METHODS: We conducted a retro

Hungry, hungry bones: electrolyte abnormalities in the presence of severe malnutrition.

Severe malnutrition in the presence of child maltreatment and other specified feeding or eating disorder (OSFED) can lead to significant effects on bones. We present a case of severe malnutrition from child maltreatment that developed into osteoporosis and persistent electrolyte abnormalities due to hungry bone syndrome (HBS). We describe a 15-year-old female with a history of medical neglect and severe malnutrition presenting to the hospital after removal from mother's home. The patient was admitted, refed, and monitored for refeeding syndrome. She had an elevated parathyroid hormone (PTH) le

Allogeneic stem cell transplantation for major T-cell lymphoma entities: an analysis of the EBMT-lymphoma working party.

BACKGROUND: Allogeneic hematopoietic stem cell transplantation (allo-SCT) is an established treatment for peripheral T-cell lymphoma (PTCL), particularly for patients with relapsed/refractory (r/r) disease. We aimed to retrieve novel information on the role of histology, disease status prior to transplantation, and donor choice for patients with PTCL not otherwise specified (NOS), angioimmunoblastic T-cell lymphoma (AITL), and anaplastic lymphoma kinase (ALK)-negative ALCL. We compared imaging by computed tomography (CT) or positron emission tomography (PET) for defining disease status prior t

Measurable residual disease is not a universally reliable surrogate for progression-free survival in clinical trials of new chronic lymphocytic leukemia therapies.

Several recent trials of new chronic lymphocytic leukemia (CLL) therapies used results of measurable residual disease (MRD)-testing as a primary endpoint assuming it is an accurate surrogate for progression-free survival (PFS) which is, itself, a surrogate endpoint for survival. But whether MRD-testing results accurately correlates with PFS following Bruton tyrosine kinase-inhibitor (BTK-i) therapy with or without venetoclax or for fixed duration therapy regimens is controversial. We searched PubMed, Web of Science, Embase, and Cochrane Library up to February 8, 2025, identifying 43 trials inv

Human HSPCs clones balance stochastic diversification with cytokine-induced differentiation.

Hematopoietic stem and progenitor cells (HSPCs) balance self-renewal with on-demand differentiation of blood lineages. Modern bone marrow atlases specify static transcriptional states, but not HSPC dynamics, which are only partly understood, particularly in humans. Here, we introduce cHSPCTrack (clonal HSPC tracker), an in vitro framework for serially tracking clonal differentiation and fate acquisition from human HSPCs extracted from circulation. By following thousands of single-cell-derived clones stimulated by different cytokine combinations, cHSPCTrack reconstructs the balance between stoc

HGBL-NOS presenting as widespread extranodal disease without lymphadenopathy: a case report.

High-grade B-cell lymphoma, not otherwise specified (HGBL-NOS), is a rare and highly aggressive B-cell lymphoma with overlapping morphologic and immunophenotypic features that complicate distinction from other aggressive B-cell lymphomas. Disseminated extranodal involvement, absence of lymphadenopathy, and lack of standardized treatment guidelines pose significant diagnostic and therapeutic challenges in HGBL-NOS. We report a 27-year-old female who presented with a six-month history of progressive left-sided chest pain, dyspnea, hemoptysis, and dry cough without constitutional B symptoms. Labo

Extensive Bone Destruction, Lymphovascular and Perineural Invasion, and Local Recurrence: Unusual Features in Salivary Gland Microsecretory Adenocarcinoma.

PURPOSE: Microsecretory adenocarcinoma (MSA) is a recently described salivary gland adenocarcinoma with distinctive histological features and novel genetic signature viz. MEF2C::SS18 fusion. Archival diagnoses are most frequently adenocarcinoma, not otherwise specified (NOS) and polymorphous adenocarcinoma (PAC). To date, most documented MSA cases have shown indolent behaviour, supporting their categorization as low-grade adenocarcinomas. METHODS: Molecularly confirmed cases of MSA with unusual clinical and histological features indicative of aggressiveness are described. Archival cases of ad

Metastatic Adnexal Adenocarcinoma: A Case Report of Successful Treatment With Carboplatin and Paclitaxel.

Metastatic adnexal adenocarcinoma not otherwise specified (AANOS) is a rare and aggressive tumor with little evidence about treatment options. We describe a case of metastatic AANOS that showed a positive response to carboplatin and paclitaxel. This response is novel in the current medical literature. We report the case of a 72-year-old male with no significant past medical history, presenting with a 4 cm occipito-temporal skin mass. A cutaneous biopsy and staging confirmed the diagnosis of primary cutaneous AANOS, with no evidence of metastasis. The patient underwent radical surgical excision
