Chronic kidney disease (CKD) disrupts mineral metabolism, skeletal remodeling, vascular homeostasis, and immune regulation, producing the systemic phenotype of CKD-mineral and bone disorder (CKD-MBD) together with chronic inflammation and impaired host defense. Osteoimmunology provides a framework for understanding how bone cells, immune cells, and vascular cells interact through shared mediators such as RANKL, osteoprotegerin, NF-kappaB, NFATc1, inflammatory cytokines, fibroblast growth factor 23 (FGF23), Wnt inhibitors, and phosphate-dependent osteogenic signaling pathways. We performed a 15-year retrospective audit of all cases of malignant otitis externa (MOE) referred to Alice Springs Hospital in Central Australia from 2009 to 2024, the largest Australian series of MOE. Data on demographics, risk factors, microbiological culture results, management and outcomes were collected, identifying 64 cases of MOE, with the incidence increasing over time. The incidence of MOE was associated with Indigenous status and the presence of diabetes mellitus and chronic kidney disease. Pseudomonas, Staphylococcus aureus and fungal pathogens predominate as causal agents. Complic

BACKGROUND: The cardio-renal-bone axis in chronic kidney disease-mineral and bone disorder (CKD-MBD) couples vascular calcification with osteoporosis. Non-specific systemic drug distribution creates a "calcium paradox", where treating one compartment worsens the other.
OBJECTIVE: This narrative review evaluates combined targeted drug-delivery systems for CKD-MBD, focusing on mineral metabolism outcomes in vascular and skeletal tissues.
METHODS: We searched PubMed, Embase, Web of Science, Cochrane Library, and ClinicalTrials.gov from inception to May 2026. Search terms combined MeSH headings

BACKGROUND/OBJECTIVES: Skull base osteomyelitis and necrotising otitis externa require prolonged antibiotic therapy, yet determining optimal treatment cessation timing remains challenging. Conventional imaging modalities demonstrate persistent abnormalities beyond infection resolution, confounding treatment decisions. This systematic review evaluated the diagnostic test accuracy of 18F-fluorodeoxyglucose positron emission tomography (18F-FDG PET) for treatment response monitoring in skull base osteomyelitis and necrotising otitis externa.
METHODS: We conducted a systematic review following PR

Complications of acute otitis media (AOM) are rare but can occur intratemporally or intracranially. Clinically relevant mastoiditis usually presents with subperiosteal abscesses, whereas subclinical opacification of the mastoid cells is common but does not require specific treatment. Other potential complications include labyrinthitis, facial nerve palsy, intratemporal abscess, cerebral venous sinus thrombosis, or meningitis. Diagnosis relies on clinical examination, laboratory tests, and targeted imaging. Intratemporal complications are primarily treated with antibiotics. Surgical interventio

BACKGROUND: Cleidocranial dysplasia (CCD) is a rare inherited skeletal disorder characterized by distinctive skeletal and dental abnormalities, often leading to delayed or missed diagnosis. Dental findings are frequently the most prominent clinical features and may play a key role in identifying the condition.
CASE PRESENTATION: A 16-year-old girl presented with missing permanent teeth, impaired mastication and esthetic concerns. The patient had previously been diagnosed with growth delay despite normal endocrine evaluation, including growth hormone stimulation testing. The diagnosis of CCD w

Acute kidney injury (AKI) triggers a rapid rise in circulating fibroblast growth factor 23 (FGF23), potentially mediated by elevations in glycerol-3-phosphate (G-3-P). Although elevated FGF23 contributes to mineral and bone disorder in chronic kidney disease (CKD), its pathophysiological role and temporal changes in mineral metabolism during AKI and recovery remain incompletely understood. We investigated longitudinal changes in mineral metabolism, including FGF23 and G-3-P, in a rat ischemia-reperfusion injury (IRI) model induced by unilateral nephrectomy followed by 35 min of contralateral I

OBJECTIVES: Persistent hyperparathyroidism and calcitriol deficiency coexist in most long -term kidney transplant recipients, yet the combined skeletal effects of these presentations have not been formally quantified.
MATERIALS AND METHODS: In this cross -sectional analytical study at People's Hospital 115 (Ho Chi Minh City, Vietnam ), we studied 135 kidney transplant recipients (mean age 51.0 ± 11.3 years; 66.7 % male; mean time posttransplant 71.4 ± 28.1 months ) who underwent simultaneous measurement of serum intact parathyroid hormone, 1,25 -dihydroxyvitamin D, and dual -energy x -ray abs

BACKGROUND: Paracoccidioidomycosis (PCM) is an endemic systemic mycosis in Latin America, classically characterized by pulmonary, mucosal and lymphatic involvement. Extrapulmonary manifestations affecting uncommon organs remain poorly characterized and may contribute to diagnostic delay and adverse outcomes.
METHODS: We conducted a retrospective case series of proven or probable atypical extrapulmonary PCM diagnosed at a Brazilian tertiary referral center between 2006 and 2025. A systematic review registered in PROSPERO and conducted according to PRISMA recommendations identified osteoarticul

The calcium-sensing receptor (CaSR) is a Family C G protein-coupled receptor (GPCR) that functions as a principal sensor of extracellular calcium ions ([Ca2+]o) and a broad-spectrum integrator of polyanionic, amino acid, and ionic stimuli. Its structural architecture - encompassing a bilobed Venus flytrap domain (VFT), cysteine-rich domain (CRD), heptahelical transmembrane domain (TMD), and an intrinsically disordered intracellular C-terminal tail - enables a remarkable allosteric landscape that goes far beyond simple on/off switching. CaSR operates as an obligate homodimer stabilized by inter

OBJECTIVE: To assess the peri-implant soft-tissue and hard-tissue response to monolithic zirconia (ZrO2) compared to porcelain-fused-to-metal implant crowns (PFMs).
METHODS: This report constitutes a pre-specified secondary analysis of this RCT. Eighty-three patients rehabilitated with single-tooth implants in the molar region were originally randomly assigned to receive either a ZrO2 or a PFM restoration. At 6 months, clinical parameters (plaque control record, probing depth, bleeding on probing, width of keratinised mucosa and marginal bone level changes) were recorded at the implant site a

Blastomycosis is a systemic pyogranulomatous infection that primarily involves the lungs and often results in asymptomatic lung infections. In some cases, however, the fungus can lead to acute or chronic pneumonia. Rarely, hematogenous spread can occur, with the most common extrapulmonary manifestations including skin, bone, and genitourinary involvement. We present a case of a 20-year-old male patient with no significant past medical history who presented with lower extremity pain and difficulty walking. After an extensive work-up, the patient was found to have disseminated blastomycosis with

BACKGROUND: Dysregulated polarization of spinal microglia is a key contributor to the pathophysiology of bone cancer pain (BCP). Bone marrow-derived mesenchymal stromal cells (BMSCs) have demonstrated significant analgesic and microglial-modulatory effects in various pain models. The nuclear factor erythroid 2-related factor 2 (Nrf2)/heme oxygenase-1 (HO-1) signaling pathway is implicated in the regulation of microglial polarization. However, the specific role and mechanistic basis of intrathecally administered BMSCs in influencing spinal microglial polarization during BCP remain to be fully c

Sinonasal and petrous bone fungal infections are rare and life-threatening conditions that essentialy arise in immunocompromised patients. They are mainly due to Aspergillus in patients with hematological diseases and to Mucorales in diabetic. Invasive fungal sinusitis can be acute (invasive aspergillosis, rhino-orbito-cerebral mucormycosis) but chronic invasive sinusitis can also exist (invasive chronic fungal sinusitis and granulomatous invasive fungal sinusitis). Petrous bone fungal infections are similar to bacterial necrotizing otitis externa and are most often due to Aspergillus. The dia

UNLABELLED: Coccidioidomycosis is a pulmonary disease caused by the dimorphic fungus Coccidioides, which is endemic to the southwestern United States and South America. Coccidioides grows as septate hyphae in soil and forms spores that can become airborne during mechanical disruption of the soil. In the lungs, the inhaled spores develop into large multinucleated spherules, a unique morphology among medically important Coccidioides species, and reproduce by endosporulation. A hallmark of the host immune response toward endosporulating spherules is the robust recruitment and infiltration of Ly6G

Sporotrichosis is an infection caused by fungi of the Sporothrix complex that can affect both humans and animals. In most cases, it is a benign infection limited to the skin and subcutaneous tissue, rarely disseminating to other organs with systemic involvement. Diagnosis is based on clinical data, epidemiological data, and laboratory tests, with culture being considered the gold standard. The disease generally has a good prognosis and is usually resolved with appropriate antifungal treatment. This case report describes an atypical presentation of sporotrichosis with suppurative nodules and or

BACKGROUND: Peripheral precocious puberty (PPP) results from estrogen excess independent of gonadotropin activation. In cases of ovarian activation, McCune-Albright syndrome (MAS) and autonomous functional ovarian cysts should be considered. In the latter condition, clear evidence-based management guidelines are currently lacking.
CASE: A girl with PPP presented with premature thelarche, recurrent vaginal bleeding and elevated estradiol with ultrasound detection of ovarian cysts. MAS was excluded by clinical, radiological and genetic evaluation. Progressive pubertal development, recurrent ble
