Pigeonpea leaves in orthopedic diseases: bioactive metabolites, pharmacological actions and therapeutic potential.

Pigeonpea leaves are derived from Cajanus cajan (L.) Huth [Fabaceae] and have long been used in traditional medicine to relieve blood stasis, swelling, pain, and musculoskeletal disorders. Flavonoids, stilbenes, phenolic acids, and triterpenoids are among the main metabolite classes identified in pigeonpea leaves. Experimental studies have linked pigeonpea leaf-derived materials to changes in inflammatory and oxidative-stress-related outcomes, microcirculation, and bone metabolism. Accordingly, their potential relevance to orthopedic disorders, including osteonecrosis of the femoral head, oste

Bacteria inside kidney tumors associate with myeloid inflammation and disease progression: A TRACERx Renal study.

BACKGROUND: Intratumoral bacteria are increasingly recognized as active contributors to neutrophil-driven inflammation and disease progression in solid tumors. Yet, in clear cell renal cell carcinoma (ccRCC), where myeloid inflammation is a key feature of aggressive disease, live bacterial presence, their intratumoral distribution, and interaction with the tumor microenvironment remain unexplored. METHODS: We genetically characterized the microbiome of primary ccRCC tumors across two independent cohorts (TRACERx: 616 samples, 143 patients; The Cancer Genome Atlas [TCGA]: 511 samples and patie

Control of M. tuberculosis infection requires CD28 to generate lung-homing T cells and CTLA-4 to limit immunopathology.

Mycobacterium tuberculosis (Mtb)-specific CD4 T cells differentiate into protective lung parenchyma-homing and non-protective intravascular subsets, but the signals that instruct these fates are poorly understood. Here we show that CD28 co-stimulation regulates host protection during Mtb infection. Cd28-/- mice are susceptible to Mtb infection, displaying higher bacterial loads and reduced survival compared to wild type mice. Cd28-/- Mtb-specific T cells expand normally but are skewed towards the lung intravascular phenotype. CD4 T cells primed in the absence of CD28 co-stimulation in B7 defic

18F-Fluorodeoxyglucose PET/CT in Tuberculosis and Other Granulomatous Diseases (except Sarcoidosis and Fungal Disease).

Granulomatous inflammation consists mainly of activated macrophages, epithelioid cells, and lymphocytes, which have high glucose metabolism and therefore show increased 18F-fluorodeoxyglucose (18F-FDG) uptake on PET/computed tomography (CT). This makes FDG PET/CT useful in detecting and mapping granulomatous disease, including clinically silent or extrapulmonary involvement. It is particularly crucial in tuberculosis, where it can help identify active disease, guide biopsy, assess treatment response, and detect dissemination. FDG PET/CT also has applications in granulomatous vasculitis such as

18F-fluorodeoxyglucose Positron Emission Tomography with Computed Tomography in Pediatric Non-oncological Conditions: A Comprehensive Review.

This article examines the expanding role of 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography with computed tomography (PET/CT) in pediatric non-oncological conditions, moving beyond its established oncologic use. It outlines radiation dosimetry principles unique to children-emphasizing the As Low as Reasonably Achievable principle, reduced tracer activity, and low-dose CT protocols-given their heightened radiosensitivity. In pediatric neurology, FDG PET/CT aids evaluation of drug-refractory epilepsy (temporal and extratemporal foci) and neuronal migration/proliferation anomalies l

AX-35 Targets the Cytochrome bc1 Complex in Mycobacterium abscessus and Reveals a Novel Resistance Mechanism.

In patients with cystic fibrosis, nontuberculous mycobacteria are opportunistic, intracellular and extracellular pulmonary pathogens that contribute to deterioration of lung function and, ultimately, morbidity from respiratory failure. Among nontuberculous mycobacterial respiratory pathogens, members of the Mycobacterium abscessus complex pose additional treatment complications as they are notoriously drug-resistant. As part of our routine screening against mycobacterial species, one of the lead antituberculosis compounds, the cytochrome bc1 inhibitor AX-35, was found to be active against isol

[Medication-related osteonecrosis of the jaw in patients receiving antiresorptive therapy: a contemporary review].

Medication-related osteonecrosis of the jaw (MRONJ) is a severe complication that develops in patients receiving antiresorptive or antiangiogenic therapy, most often with bisphosphonates or monoclonal antibodies (denosumab), which are used to treat osteoporosis and hypercalcemia in case of metastatic bone lesions. These drugs affect bone tissue, suppress the osteoclasts activity and, indirectly, osteoblasts, slowing down mineral metabolism in general. Denosumab is a monoclonal antibody against RANKL that blocks the differentiation and activation of osteoclasts. Despite their effectiveness in p

Two heads are better than one: Single stranded DNA translocation of UvrD-family dimers vs. monomers.

UvrD-family Superfamily 1A helicases are processive ATP-dependent motor proteins that function during DNA replication, recombination, repair, and transcription. UvrD-family monomers translocate along single stranded (ss) DNA with 3'-to-5' directionality but must be activated by dimerization to become helicases in the absence of force or accessory factors. Mycobacterium tuberculosis (Mtb) UvrD1 helicase forms dimers via a disulfide bond between native cysteines in the 2B sub-domains of each monomer. E. coli UvrD forms non-covalent dimers using the same 2B domain interface as in Mtb UvrD1. Using

Detection of unamplified mycobacterium tuberculosis genomic DNA in sputum samples using a metal-enhanced fluorescence-based biosensor.

Mycobacterium tuberculosis the development of a metal-enhanced fluorescence (MEF)-based biosensor in this study enabled the detection of Mycobacterium tuberculosis DNA extracted from sputum samples with high sensitivity and without the need for DNA amplification. The detection process involved the utilization of silver dendritic nanostructures, which were grown on the surface of silicon wafers and subsequently modified with a probe that was designed to target the specific DNA Insertion Element 6110 (IS6110) gene of M. tuberculosis. A comparison was made between the performance of the developed

Low-fitness rpoB mutations in rifampicin-resistant Mycobacterium tuberculosis: Host immunity, compensatory evolution, and diagnostic challenges.

BACKGROUND: The rpoB S450L mutation is the most prevalent high-fitness rifampicin resistance-conferring mutation in Mycobacterium tuberculosis. However, numerous alternative rpoB mutations associated with varying fitness costs have been reported, particularly in specific clinical settings. This systematic review critically evaluates the relationship between low-fitness rpoB mutations, host immune status, HIV co-infection, compensatory evolution, and diagnostic discordance. METHODS: A systematic literature search was conducted across multiple scientific databases. Studies reporting genetic, ph

Targeted pathway mining reveals novel modified hexapeptides from the demiguisin family.

Following the discovery of the piperazic acid (Piz)-containing hexapeptide demiguisin A from a toxic bird-associated Amycolatopsis strain, we aimed to investigate the structural diversity and bioactive potential of this natural product family. Here, we used the dmg biosynthetic gene cluster from Amycolatopsis sp. PS_44_ISF1 as a pathway mining probe to search for alternative demiguisin A producers. A global search identified only one other soil-derived Amycolatopsis sp. NBC_01488, encoding a highly syntenic pathway. Under feather-mimicking conditions, PS_44_ISF1 demonstrated enhanced growth an

In Nietzsche's Sickroom: Kafka, the Will to Power, and a Tragic Ontology of Nursing.

Nursing philosophy has drawn on Friedrich Nietzsche before, but almost always to theorise the nurse's professional identity or the profession's political position: his Übermensch as a figure of self-overcoming against managerial instrumentalisation, his genealogy of morals as a lens that unmasks caring itself as a resentful inversion of medicine's authority, his transvaluation of values as a resource against neoliberal capture. Each reading stops at the threshold of the ward's politics. This paper turns instead to the ontological relation between the nurse and a body that cannot be cured, brin

The need for fully-effective federated analytics of data sources for clinical trials.

INTRODUCTION: Clinical trials are usually analysed in a single environment allowing for flexible analysis including adjustment or stratification by subgroup: 'one-stage' analysis of individual-level data. Health systems datasets, often distributed across geography and providers, can streamline clinical trials. Data are increasingly accessible in secure data environments (SDEs). Future trial analyses may involve working across multiple SDEs. Row-level data and identifiable data often cannot leave, requiring a 'two-stage approach', where summary data from each SDE are meta-analysed. OBJECTIVE:

Validation of an AI-Driven Sputum Analysis Platform for Accurate and Reliable Tuberculosis Detection in South-Western Uganda.

BACKGROUND: Ziehl-Neelsen (ZN) sputum smear microscopy remains an accessible method for detecting acid-fast bacilli (AFB), but its interpretation is dependent on specimen quality and trained microscopy personnel. Artificial intelligence (AI) may provide a microscopy reading aid that supports consistent identification of AFB-positive and AFB-negative smear images. OBJECTIVE: To evaluate transfer-learning deep learning models for classification of AFB-positive and AFB-negative digitized ZN-stained sputum smear images generated in a Ugandan regional referral laboratory. METHODS: A retrospective

Delays in Tuberculosis Care in the Indian Population: A Systematic Review and Meta-analysis.

INTRODUCTION: Being harmful to patients, delayed diagnosis and treatment can spread tuberculosis (TB) in the community, posing a significant challenge to TB elimination efforts. This systematic review and meta-analysis aimed to estimate the median delays of TB care and the proportion of the Indian population experiencing these delays. METHODS: Data extraction and screening were conducted from August 1, 2024, to September 30, 2024, using PubMed, Scopus, and DOAJ search engines. The pooled proportion of the Indian population experiencing the total delay in TB care was estimated, at a 95% confid

A transcriptional repressor co-regulatory network integrates iron homeostasis and mycobacterial stress tolerance to oxidative stress.

A hallmark of Mycobacterium tuberculosis survival and proliferation within the host is attributable to the effective stress response of the pathogen against hostile environmental conditions. Although controlled iron (Fe) uptake and maintenance of Fe homeostasis are closely associated with intracellular survival of mycobacteria, the underlying mechanism that accounts for the interdependence of mycobacterial Fe regulation and oxidative stress response remains unknown. In this study, we discovered that Fe accumulation connects intra-bacterial redox environment via the phoP locus. Gene expression
