A case of recurrent pleural effusion: tuberculous pleurisy or other disease? A case report.

BACKGROUND: POEMS syndrome (polyneuropathy, organomegaly, endocrinopathy, M-protein, skin changes) is a paraneoplastic syndrome caused by a plasma cell proliferative disorder. Although POEMS syndrome can cause polyserous effusions, the report of pleural effusion as the main manifestation is not common. Here, we describe a case of POEMS syndrome misdiagnosed as tuberculous pleurisy due to massive pleural effusion. CASE PRESENTATION: A 70-year-old man was admitted with chest tightness and was diagnosed with tuberculous pleurisy. Despite receiving anti-tuberculosis treatment, patient exhibited i

Cervical Scrofula Revealing Miliary Tuberculosis and Previously Undiagnosed Advanced HIV: A Case Report.

Cervical tuberculous lymphadenitis, or scrofula, may be the first manifestation of previously undiagnosed advanced HIV, even when concurrent miliary pulmonary tuberculosis produces few or no respiratory symptoms. A 25-year-old male immigrant from Eastern Europe presented with one to two weeks of progressive right cervical and jaw swelling, fever, night sweats, fatigue, and anorexia, without cough, dyspnea, or chest pain. Neck and chest computed tomography demonstrated necrotic bilateral cervical lymphadenopathy, supraclavicular and paratracheal adenopathy, and diffuse miliary pulmonary nodular

Serum hepcidin as a host-derived biomarker of disease severity and risk stratification in tuberculosis.

BACKGROUND: Hepcidin is a key regulator of systemic iron metabolism. Hepcidin may reflect the inflammatory activity and disease burden in tuberculosis. However, prospective evidence on the prognostic value of serum hepcidin and other host-biomarkers in tuberculosis is limited. We evaluated the association of hepcidin with disease severity and outcomes. METHODS: This prospective cohort study enrolled consecutive adults (age ≥ 18 years) with newly diagnosed tuberculosis. Baseline serum hepcidin, adenosine deaminase (ADA), and creatine kinase-MB (CK-MB) levels were measured. Disease severity was

Baseline and longitudinal determinants of survival in end-stage renal disease: A joint longitudinal-survival analysis.

INTRODUCTION: Evidence remains limited regarding how repeated biomarker measurements influence survival in individuals with end-stage renal disease (ESRD). This study examined the prognostic role of both baseline biomarker levels and their longitudinal patterns in ESRD patients. MATERIALS AND METHODS: This retrospective cohort included maintenance hemodialysis patients from Shahid Ayatollah Ashrafi Esfahani Hospital, Tehran, Iran, with at least 3 months of dialysis history. Cox regression assessed baseline predictors, while a joint longitudinal-survival model evaluated repeated albumin, hemog

Evaluation of the BioFire joint infection panel in Taiwan: Impact of regional non-target pathogens and joint type on its performance.

OBJECTIVES: The BIOFIRE® Joint Infection (BJI) panel, a rapid multiplex polymerase chain reaction assay, is used to diagnose joint infections. Here we used it to evaluate the clinical performance and impact of antimicrobial resistance (AMR) detection in Taiwan's unique microbial landscape. METHODS: Adult patients with suspected septic arthritis or prosthetic joint infection were prospectively enrolled from four hospitals in southern Taiwan. Synovial fluid specimens were tested using a BJI panel and routine bacterial culturing. Performance was assessed by calculating the positive percent agree

Remote Body Temperature Monitoring in Infectious Diseases: Pilot Studies in Different Artiodactyla Models.

Thermal microchips are increasingly used in animals as an alternative to rectal temperature measurements because handling is not required. These pilot studies aimed to determine the usefulness of orally administered thermal microchips in infectious disease studies of artiodactyla species when a temperature increase was expected. Thermal microchips were administered subcutaneously to two groups of seven alpacas infected with influenza C and D virus respectively and three alpacas serving as non-challenged controls and orally to one alpaca in each group. Three sheep exposed to border disease viru

A Proposed Convergent Genetic and Molecular Framework for Legg-Calvé-Perthes Disease: A Scoping Review.

Legg-Calvé-Perthes disease (LCPD) is a rare pediatric disorder characterized by avascular osteonecrosis of the femoral head. Its pathogenesis is multifactorial, and genetic, epigenetic, vascular, and inflammatory mechanisms may contribute to disease susceptibility and progression. To map and characterize the available evidence on genetic and epigenetic factors and molecular pathways associated with LCPD, with particular emphasis on potential interactions and convergence across biological pathways. A scoping review was conducted in accordance with the Preferred Reporting Items for Systematic Re

Bayesian Estimates of Prevalence and Likelihood Ratio for Four Carcinosin Symptoms across Varied Clinical Conditions.

BACKGROUND: In homeopathy, symptoms are central to remedy selection and are traditionally validated through clinical experience and materia medica descriptions. Prognostic factor research using likelihood ratios (LRs) offers a quantitative approach to determine how strongly individual symptoms predict treatment response. This study evaluated the prognostic relevance of four commonly described symptoms of Carcinosin-history of tuberculosis, sleeplessness, enjoyment of watching a thunderstorm, and artistic personality-using LR and Bayesian methods. METHODS: This analytical, longitudinal, real-w

Mycobacterium tuberculosis complex Lineage 8 causes drug resistant pulmonary tuberculosis also in South Kivu, Eastern Democratic Republic of Congo.

BACKGROUND: Lineage 8 (L8) occupies a unique phylogenetic position as a sister clade to all other known Mycobacterium tuberculosis complex (MTBC) lineages. Although the initial identification of L8 strains was limited to Rwanda and Uganda, the complete geographical distribution of this lineage remains undetermined due to limited genomic surveillance across eastern and central Africa. METHODS: We performed whole genome sequencing (WGS) on MTBC isolates from rifampicin-resistant tuberculosis (TB) patients' sputum in Bukavu, South Kivu province, eastern Democratic Republic of Congo (DRC). Bedaquiline pharmacokinetics in drug-resistant tuberculosis: from exposure variability to individualized therapy.

Bedaquiline has transformed the treatment of multidrug- and rifampicin-resistant tuberculosis (MDR/RR-TB) and is now a core component of contemporary all-oral regimens. However, translating its distinctive pharmacokinetic properties into individualized clinical management remains challenging. Bedaquiline exhibits food-enhanced absorption, extensive plasma protein binding, broad but heterogeneous tissue distribution, cytochrome P450 3A4-mediated metabolism to the N-monodesmethyl metabolite M2, and prolonged persistence caused by tissue sequestration and slow redistribution. These features contr

Assessing early therapeutic response in extrapulmonary tuberculosis using [18F]FDG-PET/CT imaging.

INTRODUCTION: Monitoring treatment response in extrapulmonary tuberculosis (EPTB) remains challenging due to limited microbiological accessibility and the lack of validated biomarkers. [18F]FDG positron emission tomography/computed tomography [(18F)FDG-PET/CT] enables assessment of metabolic disease activity and has been explored in infectious diseases, but evidence supporting its use in EPTB is limited. METHODS: We conducted a prospective observational study including adults diagnosed with EPTB at a tertiary referral center in Denmark between 2016 and 2018. Participants underwent baseline an

High-throughput screening to identify inhibitors of the MTB CARD•RNA polymerase protein-protein interaction.

In 2023, tuberculosis (TB) once again became the world's deadliest infectious disease, overtaking COVID-19 as the leading cause of death from a single infectious agent, Mycobacterium tuberculosis (MTB). The World Health Organization (WHO) reports that the COVID-19 pandemic has reversed years of progress in providing essential TB services and reducing TB disease burden. In 2025, TB was responsible for an estimated 1.23 million deaths globally (95% uncertainty interval: 1.13-1.33 million), and there were 400 K cases of rifamycin resistant/multidrug resistant TB (RR, RIFR, MDR). While treatment s

Integrated transcriptomic, machine learning, and network pharmacology analyses reveal shared inflammatory biomarkers between intervertebral disc degeneration and osteoporosis.

BACKGROUND: Intervertebral disc degeneration (IDD) and osteoporosis (OP) frequently coexist, but their shared inflammatory molecular features remain unclear. We investigated whether the two disorders share an immune-extracellular matrix axis and sought candidate inflammation-associated biomarkers. METHODS: Public transcriptomic datasets GSE124272 and GSE35956 were analyzed separately using differential expression analysis and gene set enrichment analysis. Shared genes were prioritized using LASSO regression, random forest, and support vector machine algorithms, with model performance estimate

Social Determinants of Health and Behavioral Risk Factors in the Tuberculosis-Type 2 Diabetes Syndemic: A Narrative Review.

The co-occurrence of tuberculosis (TB) and type 2 diabetes mellitus (T2DM) represents a growing global syndemic that disproportionately affects populations with limited access to healthcare and preventive resources. T2DM is an established independent risk factor for TB, while social determinants of health (SDoH) further contribute to disease susceptibility, delayed diagnosis, and poor clinical outcomes. This narrative literature review examines the impact of SDoH and modifiable lifestyle factors on the TB-T2DM syndemic and highlights opportunities for intervention. A literature review was cond
