Variation in assessment, diagnosis and outcome measurement in Perthes disease: a scoping review.

PURPOSE: Perthes disease is a rare self-limiting avascular necrosis of the developing proximal capital femoral epiphysis. Identifying methods used to assess, diagnose and determine outcomes is essential to improve consistency in research and clinical care for this patient population. METHODS: The Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for scoping reviews was used. Four electronic databases (Scopus, MEDLINE, CINAHL and Embase) were searched with articles included if clinical and radiological assessments were conducted on more than 10 participants, aged 0 t

miR-671-5p-enriched exosomes derived from human embryonic stem cells under hypoxia balance oxidative stress homeostasis and macrophage reprogramming to alleviate Legg-Calvé-Perthes disease by targeting NOX2.

BACKGROUND: Legg-Calvé-Perthes disease (LCPD) remains a pediatric condition that causes hip joint deformities, with complicated pathogenesis. This study explored the influence of hypoxia-preconditioned human embryonic stem cells (hESCs)-derived exosomes on LCPD and its related mechanism. METHODS: Spontaneously hypertensive rats (SHRs) and High mobility group B1 (HMGB1) -stimulated chondrocytes were used to mimic LCPD disease-like lesion in vivo and in vitro. Femoral head damage was examined by hematoxylin-eosin (HE) and Saffron-O and fast green staining. Target molecule expression was determi

The MTHFR C677T polymorphism and protection against Legg-Calvé-Perthes disease in children: an updated systematic review and meta-analysis.

BACKGROUND: Legg-Calvé-Perthes disease (LCPD) is a multifactorial pediatric hip disorder with complex genetic underpinnings. The methylenetetrahydrofolate reductase (MTHFR) C677T (rs1801133) polymorphism influences folate metabolism and vascular function and has been investigated as a potential genetic modifier of LCPD susceptibility, though individual study findings remain inconsistent. METHODS: A comprehensive systematic search was conducted across PubMed, EMBASE, Web of Science, Cochrane Library, and Chinese biomedical databases (China National Knowledge Infrastructure [CNKI], Wanfang, VIP)

Late-onset Legg-Calvé-Perthes Disease Treated with Varus Derotation Femoral Osteotomy: A Case Report.

INTRODUCTION: Legg-Calvé-Perthes disease (LCPD) is categorized as idiopathic avascular necrosis of the femoral head, most common in children between the ages of 5 and 7. Prognosis is strongly influenced by age and lateral pillar classification, with children older than 8 years and those with Herring Group C disease typically experiencing poor outcomes. Delayed diagnosis represents a significant barrier to care, often limiting treatment options and worsening prognosis. This case highlights such a scenario while demonstrating that favorable functional and radiographic results remain achievable t

[Translated article] Clinical and radiological study of the final outcome of Legg-Calvé-Perthes disease.

BACKGROUND AND OBJECTIVE: The prediction of outcomes in Legg-Calvé-Perthes disease (LCPD) has traditionally relied on femoral head sphericity and hip joint congruence as determinants of the risk of osteoarthritis in adulthood. However, other less explored radiographic parameters may enhance the ability to anticipate functional prognosis. The aim of this study was to analyze the relationship between novel radiographic parameters of LCPD and patient-reported outcome measures (PROMs) in adulthood, including hip function, sports activity level, and quality of life. MATERIAL AND METHODS: A retrosp

Concomitant Bilateral Kienböck and Legg-Calvé-Perthes Diseases.

Avascular necrosis involving more than one bone either simultaneously or sequentially is uncommon. We report a rare case of concomitant bilateral Kienböck disease and Legg-Calvé-Perthes disease. No definitive predisposing or precipitating factor could be identified. Because of persistent wrist pain, radial shortening osteotomy was performed sequentially on both wrists. At the latest follow-up, the patient's wrist pain had resolved, and he was satisfied with the clinical outcomes.

Improved Gait and Patient-Reported Outcomes Following Hip Preservation Procedures Via Surgical Hip Dislocation Approach in Adolescents With Residual Legg-Calvé-Perthes Disease.

BACKGROUND: Adolescent patients with hip dysfunction due to residual deformity in the healed stage of Legg-Calvé-Perthes disease (LCPD) remain a treatment decision-making challenge. Some advocate hip preservation via the surgical hip dislocation (SHD) approach, with concomitant procedures to address various hip pathologies. Currently, hip movement and gait analysis data before and after the SHD approach are lacking. This study aimed to determine the gait parameters, patient-reported outcomes, and radiographic outcomes of adolescent patients with LCPD who had hip preservation procedures via SHD

IL-6-induced endothelial microparticles exacerbate juvenile ischemic osteonecrosis by promoting Osteoclastogenesis: Implications for Perthes disease.

Perthes disease is a debilitating idiopathic osteonecrosis of the pediatric femoral head and is characterized by femoral head deformity driven by uncoupled bone remodelling, with excessive osteoclastic activity. However, the systemic signals orchestrating this localized bone destruction remain poorly understood. In this study, we found that circulating microparticles (MPs) isolated from the plasma of patients with Perthes disease at the necrotic stage exhibited a greater capacity to promote osteoclast differentiation than MPs from healthy controls. Given the elevated interleukin-6 (IL-6) level
