Progressive Osteochondral Defect of the Medial Femoral Condyle in a Collegiate Football Player: A Case Report.

CASE: A 21-year-old male Division I American football wide receiver presented with atraumatic anteromedial left knee pain. Initial MRI revealed no osteochondral defect, and he was treated conservatively. As symptoms worsened, serial MRI revealed a progressive medial femoral condyle lesion with subchondral depression. He subsequently underwent an osteochondral allograft transplantation and achieved a full recovery. The time from initial presentation to diagnosis was approximately 7 months. CONCLUSION: Progressive osteochondral defects may not be visible on initial MRI. In athletes with persist

Management and Outcomes of Osteochondritis Dissecans of the Knee in the Pediatric and Adolescent Population.

PURPOSE OF REVIEW: Osteochondritis dissecans (OCD) of the knee is a complex condition in pediatric and adolescent patients, and management relies heavily on lesion characteristics including size, stability, and fragmentation. The purpose of this review is to inform readers of the current understanding, treatment options, and outcomes of the disorder in pediatric and adolescent patients. RECENT FINDINGS: While management of OCD of the knee is tailored highly to patient- and lesion-specific factors, broad management pathways are dictated first by lesion stability, and further by salvageability

Ossification variants of the distal femoral condyle: longitudinal 3 T MRI evidence of progression to juvenile osteochondritis dissecans in asymptomatic siblings of patients with JOCD.

OBJECTIVE: Ossification variants (OVs) of the femoral condyles are traditionally regarded as benign developmental findings distinct from juvenile osteochondritis dissecans (JOCD). We aimed to characterize the longitudinal MRI behavior of OVs and JOCD lesions in asymptomatic siblings of JOCD patients. MATERIALS AND METHODS: In this HIPAA-compliant longitudinal pilot study, seven asymptomatic siblings of JOCD patients underwent serial 3 T bilateral knee MRI. Two fellowship-trained musculoskeletal radiologists independently assessed 56 studies for bone marrow edema, lesion location, and MRI-defi

Vital bone and increased osteoid in equine osteochondrosis dissecans of the proximal phalanx: A controlled study indicating a mineralization deficit rather than vascular failure.

Osteochondrosis and osteochondrosis dissecans (OCD) are among the most common orthopaedic disorders in young horses. Although traditionally explained by vascular failure and ischaemic necrosis, recent findings in human juvenile OCD suggest that impaired bone mineralization may play an important role. Horses are known to have relatively low circulating vitamin D metabolites, raising the question of whether insufficient mineralization contributes to equine OCD. This study investigated the bone vitality and mineralization status of osteochondrosis dissecans fragments (OCDFs) collected from the do

Thirty-six months of experience and 30° of hip internal rotation as critical thresholds for capitellar osteochondritis dissecans in youth baseball players.

BACKGROUND: Capitellar osteochondritis dissecans (OCD) is a serious throwing injury that can lead to long-term elbow dysfunction. While a longer duration of competitive play is a suspected risk factor, no specific cutoff value has been established to identify players at high risk. PURPOSE: To identify independent risk factors for capitellar OCD and determine the optimal cutoff value for the duration of competitive play in youth baseball players. METHODS: This cross-sectional study analyzed 853 youth baseball players (aged 9-12 years) who participated in medical check-ups. OCD was diagnosed u

Factors affecting Union Rates following Fixation of Osteochondritis Dissecans of the Knees: A Systematic Review and Meta-Analysis.

The current systematic review and meta-analysis aims to evaluate the factors that affect union rates following fixation of osteochondritis dissecans of the knees. The systematic review was conducted using the keywords "knee" and "osteochondritis dissecans" or "osteochondral lesion" via a search through PubMed and Cochrane Library. All original human studies that reported on factors affecting union rates following fixation of osteochondritis dissecans of the knees were included. Nineteen studies with 512 knees were included. The union rates following surgical fixation of osteochondritis disseca

Extracellular Matrix With Bone Marrow Aspirate Concentrate for Osteochondritis Dissecans of the Capitellum in Adolescents.

Capitellar osteochondritis dissecans is most frequently encountered in overhead and tumbling adolescent athletes. Traditional management with abrasion chondroplasty or microfracture results in fibrocartilage formation and variable return to sport rates, whereas osteochondral autograft transfer system or osteochondral allograft transplantation may incur unnecessary morbidity for small, shallow lesions. This article therefore describes the application of a biologic scaffold composed of extracellular matrix combined with bone marrow aspirate concentrate to unstable, shallow (<6-7 mm), but contain

Glenoid osteochondritis dissecans in 9 baseball players: arthroscopic management and radiographic outcomes.

Glenoid osteochondritis dissecans (OCD) is a rare condition seen occasionally in throwing athletes. Early diagnosis using conventional radiography is challenging. The aim of this study was to report on a series of baseball players receiving treatment for glenoid OCD. This retrospective case series included 9 baseball players with glenoid OCD who underwent arthroscopic surgery with a minimum of one-year follow-up. Plain radiographs were routinely obtained in the arm-elevated position (140-150°) to compare glenoid morphology between the throwing and non-throwing shoulders. Computed tomography an
