Surgical Treatment of Multifocal Nonbilateral Juvenile Osteochondritis Dissecans in the Ankle and Shoulder: A Case Report.

CASE: We report a unique case of a 14-year-old girl with juvenile osteochondritis dissecans (JOCD) lesions of the talus and humeral head. She presented with painful motion of the ankle and shoulder that failed conservative treatment and was treated with surgical intervention. Four-year follow-up was achieved. CONCLUSION: A paucity of literature exists regarding the incidence and treatment of JOCD lesions occurring in multiple joint areas of a single patient. Our case highlights a unique presentation of contralateral, multijoint JOCD lesions and describes the treatment modalities subsequently

Histological analysis of quadriceps tendon collagen composition across physical maturity levels and anatomical layers.

PURPOSE: To investigate whether patient physical maturity and histological layer influence the collagen composition of the quadriceps tendon (QT). METHODS: QT samples were collected intraoperatively from 25 patients undergoing primary anterior cruciate ligament reconstruction using a QT autograft or during knee osteochondral autograft harvesting for osteochondritis dissecans, with one sample obtained from each of the three QT layers. Sections were immunostained for type I and III collagen. The relative area of each collagen type (type I + type III = 100%) was quantified. Patients were grouped

Preoperative Proximal Migration of the Radial Head as an Independent Predictor of Suboptimal Outcomes After Osteochondral Autograft Transplantation for Capitellar Osteochondritis Dissecans: A Retrospective Cohort Study.

BACKGROUND: Osteochondral autograft transplantation (OAT) is widely performed for capitellar osteochondritis dissecans (OCD). However, preoperative predictors of suboptimal postoperative outcomes remain unclear. PURPOSE/HYPOTHESIS: The authors aimed to evaluate clinical outcomes after OAT for capitellar OCD and identify preoperative risk factors associated with suboptimal outcomes. They hypothesized that radiographic indicators of disease severity, including preoperative proximal migration of the radial head, lesion size, and lateral wall disruption, would be associated with suboptimal postop

Prevalence of Capitellar Osteochondritis Dissecans and Medial Epicondylar Abnormalities in Female Youth Baseball Players: A Large-Scale Ultrasonographic Screening Study.

BACKGROUND: Capitellar osteochondritis dissecans (OCD) and medial epicondylar apophyseal abnormalities are recognized throwing-related elbow injuries in youth baseball. However, most epidemiological data have been derived from male athletes and the prevalence of these conditions among female youth baseball players has not been well characterized. PURPOSE: To determine the prevalence of capitellar OCD and medial epicondylar abnormalities among female youth baseball players using large-scale ultrasonographic screening data and to compare these findings with those of male players. STUDY DESIGN:

Dysplasia epiphysealis hemimelic in the knee: a case report.

BACKGROUND: Dysplasia epiphysealis hemimelica (DEH) is a rare congenital bone dysplasia characterized by overgrowth of the epiphyseal cartilage on one side, with its etiology remaining unclear. The condition predominantly affects weight-bearing joints of the lower limbs, typically presenting with localized pain due to a rapidly growing mass around the joint. In severe cases, symptoms such as limb length discrepancy and limb deformity may occur. Early diagnosis is challenging, and delayed diagnosis and treatment can easily lead to limb deformity and functional impairment. CASE PRESENTATION: Th

Arthroscopic Technique for Autologous Minced Cartilage Implantation for Focal Lesions of the Capitellum Humeri of the Elbow: A Technical Note.

Focal cartilage defects of the capitellum humeri are frequently underestimated, despite their substantial contribution to elbow pain and functional impairment in young and physically active patients. Arthroscopic autologous minced cartilage implantation is a single-stage biological cartilage repair technique that has shown favorable clinical and imaging outcomes in larger joints. Osteochondral lesions of the capitellum humeri of the elbow, particularly in the setting of osteochondritis dissecans, represent a challenging condition in young and physically active patients. However, detailed descr

Radiographic Anomalies of the Tarsal Navicular.

INTRODUCTION: Radiographic abnormalities of the tarsal navicular are frequently encountered incidentally in children undergoing plain radiography of the foot for pain or deformity. These abnormalities are rarely deemed to be associated with the child's chief complaint or physical examination findings. Whether these radiographic anomalies are due to prior trauma, history of unrecognized osteochondritis dissecans, or avascular necrosis earlier in life remains unclear as the numerous subtle navicular abnormalities have never been characterized. The purpose of this study was thus to describe and c

Autologous chondrocyte implantation in osteochondral defects of the capitulum: clinical and radiological outcome in a case series.

BACKGROUND: Osteochondritis dissecans (OCD) of the capitellum is a cartilage and subchondral bone disorder predominantly affecting young, physically active individuals. While established surgical techniques such as microfracturing and osteochondral autograft transfer offer acceptable outcomes, limitations in cartilage quality or procedural invasiveness remain. Autologous chondrocyte implantation (ACI) has been proposed as a less invasive, biologically restorative alternative, though clinical data on its use in the elbow are not available. Therefore, the aim of this pilot study was to evaluate

Screw Versus Suture Bridge Fixation for Medial Femoral Condyle Osteochondritis Dissecans Demonstrate Comparable Healing Rates.

BACKGROUND: Fixation of unstable medial femoral condyle (MFC) osteochondritis dissecans (OCD) lesions aims to achieve osseous union while preserving the articular cartilage. Suture bridge fixation has emerged as an alternative to bioabsorbable implants or metallic headless compression screws. HYPOTHESIS: The authors hypothesized that rates of union, conversion to a secondary cartilage restoration procedure, and patient-reported outcomes would not differ significantly between screw and suture bridge fixation, with fewer implant-related complications after suture bridge fixation. STUDY DESIGN:

More Horizontal ACL Posterolateral Bundle Position in Osteochondritis Dissecans of the Lateral Femoral Condyle.

BACKGROUND: The pathogenesis of osteochondritis dissecans (OCD) is multifactorial. An association of the horizontal anterior cruciate ligament (ACL) position with the anatomic proximity of its posterolateral (PL) bundle to the OCD of the lateral femoral condyle (LFC) has not been previously found. HYPOTHESIS: In OCD of the LFC, a more horizontal ACL and PL bundle position can be found, compared to that of control patients. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: The authors retrospectively analyzed magnetic resonance imaging scans of 90 knees in 90 patients, creatin

Osteochondritis dissecans of the glenoid: A case report and review of the literature.

BACKGROUND: Osteochondritis dissecans (OCD) is an articular disorder characterized by focal subchondral bone involvement that may progress to cartilage instability and fragment detachment. Although OCD most commonly affects the knee, ankle, and elbow, shoulder involvement is rare, and glenoid lesions are particularly uncommon. The limited literature consists primarily of isolated case reports, most frequently describing male overhead athletes. Optimal management strategies for glenoid OCD remain poorly defined, particularly in adolescent athletes with big chondral lesions. PURPOSE: To report

Avascular Necrosis Secondary to Traumatic Posterior Hip Dislocation.

BACKGROUND: Avascular necrosis of the femoral head, a known long-term complication following traumatic posterior hip dislocation, can result in femoral head collapse, osteochondral defects, and eventual posttraumatic osteoarthritis. This condition is especially problematic in young, active patients for whom total hip arthroplasty survivorship is a concern. Biologic treatments are therefore preferable to delay or avoid total hip arthroplasty. CASE REPORT: A 16-year-old male developed progressive left hip pain after a posterior dislocation sustained while playing football. The hip was emergentl

Research in OsteoChondritis of the Knee-ROCK Research Study Group.

Knee osteochondritis dissecans (OCD) is a condition affecting the articular cartilage and subchondral bone, predominantly in young athletes, which has historically suffered from a lack of high-quality evidence to guide diagnosis and treatment. In response to the 2010 American Academy of Orthopaedic Surgeons (AAOS) clinical practice guideline, which identified critical gaps in OCD research, the Research in OsteoChondritis of the Knee (ROCK) group was established as an international, multicenter research consortium with the goal of advancing the clinical understanding and treatment of knee OCD.

Age, Sex, and Location Impact Osteochondritis Dissecans Lesion Depth and Their Relation to the Physis in Skeletally Immature Knees.

PURPOSE: To characterize the size of medial and lateral femoral condyle osteochondritis dissecans (OCD) lesions and their relation to the physis in skeletally immature patients. METHODS: Skeletally immature patients from 2008 to 2023 with medial or lateral femoral condyle OCD lesions on magnetic resonance imaging were included. At the center of each OCD lesion, 2 reviewers independently measured on coronal and sagittal cuts: (1) lesion depth, (2) shortest distance from subchondral bone to open physis, and (3) shortest distance from cartilage to open physis. Intraclass correlation coefficients

Editorial Commentary: Beyond the Lesion: Individualizing Safe Implant Depth During Osteochondritis Dissecans Fixation in Skeletally Immature Knees.

Successful fixation of unstable osteochondritis dissecans in skeletally immature patients requires precise implant placement as well as careful appreciation of the distal femoral physis beyond the lesion. Although 16-mm drill or screw length can reliably provide a 3 mm or greater margin of safety between the implant and distal femoral physis, isotropic 3-dimensional magnetic resonance imaging sequences allow surgeons to incorporate multiplanar reformatted imaging into preoperative planning to measure patient- and lesion-specific safe implant depths and optimize implant trajectories. Return to Sport After Surgical Treatment of Capitellar Osteochondritis Dissecans Lesions in Baseball Players: A Systematic Review and Meta-analysis.

BACKGROUND: Because of the difficulty of baseball players with capitellar osteochondritis dissecans (OCD) lesions to return to sport (RTS), multiple operative strategies have been used for these lesions. However, there has yet to be a systematic review of RTS outcomes in specifically baseball players. PURPOSE/HYPOTHESIS: The purpose of this study was to compare the RTS rates between surgical procedures for capitellar OCD lesions in youth baseball players, with a secondary purpose of comparing improvements in elbow range of motion (ROM) and improvements in Timmerman-Andrews scores between surg

Femoral Osteochondritis Dissecans and Tibial Osteochondral Defect in an Adult Revealed by Bone SPECT/CT.

A 46-year-old woman presented with persistent right knee pain and swelling six months after a fall. MRI initially showed a lateral meniscus tear, leading to meniscus repair and later meniscectomy, but symptoms persisted. Retrospective review of the MRI revealed edema in the tibial plateau and distal femoral condyle. Arthroscopic debridement demonstrated severe synovitis, marked cartilage loss of the lateral femoral condyle with a loose body, and tibial plateau cartilage damage. Bone SPECT/CT showed bony destruction, cleft formation, and focal tracer uptake in the distal femur and proximal tibi
