INTRODUCTION: Relapsing polychondritis is a rare, systemic autoimmune condition characterized by recurrent inflammation of cartilaginous tissues. Its symptoms are often nonspecific and can lead to misdiagnosis or delayed treatment. This case report emphasizes the need for clinicians to recognize the potential for relapsing polychondritis in patients presenting with recurrent episodes of cartilaginous inflammation and sudden hearing loss, as early intervention is crucial in preventing irreversible damage.
CASE PRESENTATION: The patient, a 24-year-old Ethiopian male, presented with a history of

A 64-year-old male presented with progressive breathlessness and wheezing, initially suspected to be worsening asthma. Inhaled therapies were commenced and further investigations were conducted, including CT thorax, bronchoscopy, pulmonary function tests and autoimmune screening. Imaging revealed tracheobronchomalacia and bronchoscopy confirmed a grossly abnormal endobronchial tree with appearances suggestive of relapsing polychondritis. A multidisciplinary discussion led to a diagnosis of tracheobronchomalacia secondary to relapsing polychondritis. Worsening symptoms of breathlessness led to

Genetic contributions to systemic autoimmunity are often considered more significant in children than in adults. As such, genetic evaluation may be more frequently pursued in pediatric rheumatology patients. Motivated by the discovery of a STING-associated vasculopathy with onset in infancy (SAVI) mutation in a patient with adult-onset relapsing polychondritis and systemic lupus erythematosus, we hypothesized that STING gain-of-function mutations might underlie a broader spectrum of autoimmune disease in adults. We systematically screened 43,731 exomes from the Penn Medicine Biobank, revealing

BACKGROUND: VEXAS syndrome is a recently recognized, acquired monogenic adult onset hemato-inflammatory syndrome characterized by somatic mutations within the UBA1 gene. The acronym VEXAS stands for vacuoles, E1 enzyme, X-linked inheritance, autoinflammatory tendencies, and somatic mutations. It presents as a severe progressive disease displaying varied characteristics that bridge hematologic and rheumatologic domains. Herein, we describe a series with a detailed evaluation of 11 cases of VEXAS syndrome.
MATERIALS AND METHODS: A comprehensive retrospective analysis of patients diagnosed with

Relapsing polychondritis (RP) is a rare autoimmune disorder with minimal reported renal involvement. We describe the first case of tip lesion variant of focal segmental glomerulosclerosis (FSGS) in a 60-year-old male with familial RP. The patient initially presented with nephrotic syndrome concomitant with RP, which was diagnosed 17 years ago; renal biopsy revealed minimal change disease at that time. Over the subsequent 17 years, he experienced four RP flares without nephrotic syndrome recurrence until his recent presentation with severe nephrotic syndrome. Repeated renal biopsy confirmed tip

OBJECTIVE: Auricular hematomas commonly result from soft tissue trauma and require prompt drainage and pressure dressing to prevent recurrence and deformity. Both bolster and quilting suture techniques are used post-drainage, but direct comparisons are lacking. The objective of this study is to compare outcomes between bolster dressings and through-and-through mattress quilting sutures in the management of auricular hematomas.
METHODS: A 15-year retrospective chart review was conducted for patients age > 10 years presenting to the Emergency Department with auricular hematoma. Exclusion criter

OBJECTIVES: Relapsing polychondritis is characterized by recurrent flares, and comparative evidence for relapse prevention using biologics remains limited. We evaluated the association between biologic exposure and relapse incidence.
METHODS: This single-centre retrospective cohort study (Kyoto University Hospital, 2000-23) investigated adults with relapsing polychondritis. Follow-up was divided into periods without biologics (No-Bio), with TNF-α inhibitors (TNFi) or with IL-6 receptor inhibitor (IL-6Ri). Primary outcomes were relapsing polychondritis relapse and hospitalized infection. Perio

BACKGROUND: Antiphospholipid syndrome (APS) is an autoimmune thrombophilia characterized by arterial and venous thrombosis that commonly affects the deep veins of the lower extremities and cerebral vasculature. Inferior vena cava (IVC) thrombosis is an extremely rare manifestation of APS and is often associated with hypercoagulable states. Relapsing polychondritis (RPC) is an autoimmune disorder affecting cartilaginous structures, sometimes occurring alongside APS. However, the coexistence of primary suspected APS and RPC has rarely been reported.
CASE PRESENTATION: A 33-year-old previously h

Relapsing polychondritis (RP) is a rare and complex autoimmune disease primarily affecting cartilage tissue. In particular, RP leads to inflammatory damage of airway cartilage, resulting in severe complications such as airway narrowing and tracheomalacia, which has a significant effect on patients' quality of life and prognosis. Although the clinical manifestations of RP are diverse, airway involvement has become a focal point of research and clinical attention because of its high incidence and potential lethality. Currently, the early recognition and accurate diagnosis of airway lesions remai

OBJECTIVES: Relapsing polychondritis (RP) is a rare, multisystem inflammatory disease characterized by heterogeneous clinical manifestations and a substantial impact on patients' daily functioning and well-being. Health-related quality of life (HR-QoL) in RP remains insufficiently characterized, and no disease-specific measurement tool is available so far. This study aimed to develop and validate the RP-QoL, a multilingual, patient-reported outcome instrument specifically designed to assess HR-QoL in individuals with RP.
METHODS: The RP-QoL was developed within the European Reference Network

We report the case of a 30-year-old female patient with Crohn's disease who was receiving infliximab and mesalazine therapy. She presented with left ear pain accompanied by bloody diarrhea and abdominal pain. Initial treatment with ciprofloxacin and aluminum acetate was ineffective, requiring hospitalization. She was treated with intravenous meropenem, clindamycin, and prednisolone, which led to symptom resolution after 12 days. However, 7 months later, the patient developed similar symptoms in the right ear without intestinal involvement. She was hospitalized and treated with intravenous anti

BACKGROUND: Large language models (LLMs) are increasingly applied in clinical decision support, yet their diagnostic performance in Chinese-language settings and under realistic clinical workflows remains unclear. In particular, how LLMs perform across diseases with different prevalence and under stepwise diagnostic processes has not been well characterized.
OBJECTIVE: This study aimed to evaluate the diagnostic capabilities of LLMs for common diseases and rare diseases using clinical vignettes within a hypothetico-deductive framework and to identify their potential and limitations for clinic

Relapsing polychondritis (RP) is a rare immune-mediated inflammatory disorder primarily affecting cartilaginous tissues throughout the body that may involve other connective tissues. The initial manifestation is often auricular chondritis, and cutaneous presentations are relatively uncommon. Approximately 30% of RP cases have an association with autoimmune diseases, but it is incredibly rare for there to be any coexistence with inflammatory bowel disease (IBD). Sweet syndrome (SS), which is also known as acute febrile neutrophilic dermatosis, also has an association with autoimmune disorders a

OBJECTIVE: Relapsing polychondritis (RP) is a rare inflammatory disease characterized by recurrent cartilaginous inflammation with unclear pathogenesis. The precise alterations in the peripheral immune landscape driving RP pathogenesis remain incompletely defined.
METHODS: Single-cell RNA sequencing was performed on peripheral blood mononuclear cells from six patients with RP and five matched healthy controls. Bioinformatic analyses characterized cellular composition, transcriptomic profiles, differentially expressed genes, pathway enrichment, metabolic states, and intercellular communication
