Clinical Application and Outcomes of Endoscopic-Assisted Autologous Septal Cartilage Technique in Crooked Nose Correction.

BACKGROUND: Crooked nose deformity is challenging when septal deviation coexists with external malalignment. We evaluated an endoscopic-assisted approach using autologous septal cartilage to improve both function and aesthetics. METHODS: We retrospectively reviewed 186 consecutive patients (January 2023-June 2025) who underwent endoscopic-assisted septorhinoplasty with autologous septal cartilage grafting. Rohrich types were Type I (n = 58), C-type (n = 100), and S-type (n = 28). Outcomes included digital photographic measurements (dorsum/tip deviation angles, nasolabial angle, tip projection

Asymmetric Puzzle Spreader Graft: A Systematic Approach for Correction of Crooked Nose Deformity.

BACKGROUND: Correction of crooked nose deformity remains one of the most challenging aspects of rhinoplasty. C-shaped and reverse C-shaped nasal deviations require asymmetric surgical approaches to achieve optimal aesthetic and functional outcomes. Traditional symmetric spreader graft techniques often fail to address the underlying asymmetric deformity adequately. OBJECTIVES: This study aimed to evaluate the efficacy of a novel asymmetric puzzle spreader graft technique combined with asymmetric osteotomy and osteoplasty in correcting C-shaped and reverse C-shaped nasal deviations. METHODS: B

[A swollen, deformed nose: diagnosis, treatment and ENT referral of nasal fractures].

Nasal fractures are the most common facial fractures, with a higher incidence in males. They typically result from trauma related to sports, accidents, or physical violence, often involving alcohol. Accurate assessment requires knowledge of nasal anatomy, as fractures may involve both bony and cartilaginous structures. Presentation includes swelling, epistaxis, deformity, and nasal obstruction. The trauma's mechanism and patient's age influence the fracture type. Diagnosis is primarily clinical, as imaging has limited diagnostic value and does not alter management. Closed reduction is ideally

One-stage scalp composite tissue transplantation for the correction of pinched nose deformity in Asian patients undergoing revision rhinoplasty.

Pinched nasal deformity is relatively common among Asian patients undergoing revision rhinoplasty and is frequently associated with nasal mucosal contracture, which makes corrective procedures particularly challenging. This study investigates the efficacy of one-stage scalp composite tissue transplantation as a corrective intervention for pinched nose deformities in Asian patients undergoing revision rhinoplasty. Between June 2020 and June 2023, a total of 12 Asian patients with pinched nose deformities resulting from previous rhinoplasties were included in this study. All participants underwe

Endonasal Secondary Rhinoplasty Techniques.

Revision rhinoplasty remains one of the most demanding procedures in facial plastic surgery, challenging not only because of the altered anatomy, deficiencies of structural support, and complex 3-dimensional morphology of the nose, but also because of the varying degrees of scarring. Although the open approach dominates contemporary revision rhinoplasty practice, the endonasal technique remains a valuable option in appropriately selected patients. By preserving the skin-soft tissue envelope and the intrinsic ligamentous support of the nose, endonasal revision offers advantages including reduce

Techniques for the Persistently Crooked Nose.

A persistently crooked nose refers to a condition in which the deviation of the nasal pyramid from the facial midline remains or reappears after the rhinoplasty surgery. Asymmetries in any compartment of the nose are among the main concerns of patients seeking revision rhinoplasty. Therefore, mastering surgical techniques to address these issues is essential to achieve success in revision cases. In this article, we discuss various surgical techniques, present case examples, and provide supplementary revision rhinoplasty videos performed by the senior author to enhance understanding.

Hyaluronic acid injections for alar retraction: a retrospective study on clinical effects and biomechanics.

BACKGROUND: Nasal alar retraction poses a considerable clinical challenge. Conventional correction often relies on open rhinoplasty with cartilage grafting, which carries inherent surgical risks. For mild to moderate cases, hyaluronic acid (HA) fillers represent a promising and minimally invasive alternative. OBJECTIVES: We aimed to evaluate the clinical outcomes of HA injection for correcting nasal alar retraction by integrating objective photographic measurements, patient-reported outcomes, and finite element analysis (FEA). METHODS: A retrospective, single-center study was conducted on pa

Contemporary Management of Caudal Septal Deformities: An Algorithmic Approach.

AIM AND BACKGROUND: Septal deformities are highly prevalent in the population and septoplasties are one of the most common procedures performed by otolaryngologists and facial plastic surgeons. Caudal septal deformities represent a challenging component when managing septal deviations given the caudal septum's critical role as a tip supporting mechanism. As a result, they are a frequent cause for revision procedures. TECHNIQUES: Caudal septal deformities can range from mild to severe and require careful assessment and strategic operative planning. Depending on the deformity, a variety of tech

Association Between Nasal Septal Deviation and Estimated Pulmonary Artery Pressures-Scoping Review.

OBJECTIVE: Nasal septal deviation (NSD) is a common cause of chronic upper airway obstruction (UAO), and surgical correction via nasal septoplasty results in improvements in airflow. Multiple studies have been conducted to assess possible relationships between chronic UAO (including due to NSD) and cardiorespiratory function (such as pulmonary artery pressures). The objective of this scoping review is to identify and examine the current evidence on the potential relationship between nasal septal deviation, estimated mean pulmonary artery pressure (mPAP), and estimated pulmonary artery systolic

Strategies for the Previously Operated Septum.

In a standard septoplasty, most of the quadrangular cartilage is resected while preserving an L-strut to maintain structure of the nose. Deviation may recur due to contractile forces causing flexure or torsion of the L-strut, which may be reduced by modifying the shape of the L-strut chondrotomy. Alternatively, deviations may have been insufficiently addressed by the initial septoplasty. Grafting techniques are often used to address deviations of the L-strut, and repositioning of the posterior septal angle may be required when displaced off the anterior nasal spine. With certain severe sepal d
