Endoscopic tracheoesophageal fistula closure-Electrocautery combined with esophageal clip application in pediatric patients.

OBJECTIVE: A tracheoesophageal fistula (TEF) is an abnormal communication between the esophagus and trachea, most often associated with esophageal atresia (EA), a rare congenital malformation affecting 1 in 2400-4500 live births. While surgical repair of EA/TEF is typically completed in infancy, recurrent or missed TEF can be diagnosed later. Open repair can be technically challenging, often requiring re-do thoracotomy or neck dissection. We describe our center's experience, using endoscopic electrocautery in combination with esophageal clips for closure of TEF. METHODS: We conducted a retros

Slide tracheoplasty versus tracheal resection anastomosis for acquired laryngotracheal stenosis: a prospective randomized controlled trial.

BACKGROUND: Tracheal resection with end-to-end anastomosis remains the standard surgical treatment for acquired high-grade laryngotracheal stenosis. Slide tracheoplasty has emerged as an alternative reconstructive technique that enlarges airway caliber while preserving tracheal length and vascularity, yet direct comparative evidence in acquired stenosis is limited. OBJECTIVE: To compare the clinical, surgical, and functional outcomes of slide tracheoplasty versus tracheal resection with end-to-end anastomosis in patients with acquired laryngotracheal stenosis. METHODS: In this prospective pi

Strategies for the Previously Operated Septum.

In a standard septoplasty, most of the quadrangular cartilage is resected while preserving an L-strut to maintain structure of the nose. Deviation may recur due to contractile forces causing flexure or torsion of the L-strut, which may be reduced by modifying the shape of the L-strut chondrotomy. Alternatively, deviations may have been insufficiently addressed by the initial septoplasty. Grafting techniques are often used to address deviations of the L-strut, and repositioning of the posterior septal angle may be required when displaced off the anterior nasal spine. With certain severe sepal d

Building the cancer research workforce through implementation of the RACE21 undergraduate cancer research and education program.

BACKGROUND: Cancer remains the second leading cause of death in the state of Alabama and disproportionately affects communities that are rural, of lower socioeconomic status, and individuals with high-risk, predisposing conditions such as obesity, cardiovascular disease, advanced age, and diabetes. One method to address the increased burden of cancer is to expand the cancer research and clinical workforce. The Roadmap for America's Cancer Explorers for the 21st Century (RACE21) is a National Cancer Institute-funded Youth Enjoy Science program at the University of Alabama at Birmingham (UAB) an

Management of hypoparathyroidism during pregnancy following late maternal diagnosis of DiGeorge syndrome: a case report.

BACKGROUND: Hypoparathyroidism during pregnancy is an uncommon condition, particularly in nonsurgical patients. DiGeorge syndrome (22q11.2 deletion syndrome), a common microdeletion disorder, presents with highly variable features that often delay diagnosis until adulthood. In the absence of hallmark findings such as congenital heart defects or dysmorphic features, diagnosis can be missed or delayed. This case emphasizes the importance of a systematic approach for clinicians facing low calcium levels during pregnancy. Furthermore, it provides valuable guidance for managing and monitoring hypoc

Poikilodermatous Change of Skin: Diagnostic Insights from Clinical and Histopathological Examination.

Poikilodermatous skin changes represent a diagnostic challenge due to their non-specific clinical features and broad differential diagnoses. This report presents the case of a 29-year-old female with symmetrical, variably pigmented macules and patches over sun-exposed areas, including the neck and upper limbs. Histopathological examination revealed epidermal atrophy, pigment incontinence, basal layer pigmentation, dermal telangiectasia and a mild lymphocytic infiltrate - findings consistent with poikilodermatous change. No features suggestive of interface dermatitis, atypical lymphoid cells or

Aspiration-Related Alveolar Disease: From Acute Injury to Chronic Pulmonary Sequelae.

Aspiration-related lung diseases comprise a broad spectrum of disorders resulting from the entry of oropharyngeal or gastric contents into the lower respiratory tract. Clinical manifestations range from acute aspiration pneumonitis and aspiration pneumonia to chronic airway and parenchymal injury, including bronchiolitis, bronchiectasis, fibrosis, and lipoid pneumonia. Aspiration pneumonitis is driven primarily by chemical injury, whereas aspiration pneumonia results from infection caused by aspirated microorganisms in susceptible hosts. Contemporary microbiologic data indicate that aspiration

Two-Stage Reconstruction for Failed Subtrochanteric Femur Fixation with Peri-Implant Infection and Severe Proximal Femoral Bone Loss: A Case Report.

INTRODUCTION: Subtrochanteric femur fractures are difficult to manage due to high mechanical stresses and complex deforming forces. Treatment often involves placing an intramedullary nail (IM nail) for stabilization and early mobilization. Despite being the preferred fixation method, IM nail constructs remain vulnerable to failure, and repeated fixation attempts can compromise remaining bone stock. When fixation failure occurs in conjunction with early post-operative infection, the combined instability and bone loss may necessitate staged reconstruction. CASE REPORT: A 40-year-old female sust

Persistent elevation of procalcitonin following resolution of presumed community-acquired pneumonia revealing sporadic medullary thyroid carcinoma.

Procalcitonin (PCT) is widely utilized as a biomarker of bacterial infection and sepsis and is expected to decline following successful treatment. Persistent elevation in the absence of ongoing infection should prompt consideration of alternative diagnoses. Medullary thyroid carcinoma (MTC), a rare neuroendocrine malignancy arising from parafollicular C-cells, is increasingly recognized as a non-infectious cause of elevated PCT. A 67-year-old woman presented with palpitations, exertional dyspnea, and dizziness. Initial investigations demonstrated a markedly elevated PCT level of 11.7 ng/mL and

Glottic Stenosis.

Glottic stenosis may be congenital, such as anterior glottic webs, or acquired, as a result of surgery or prolonged intubation. Glottic-level obstruction may occur as an isolated lesion or as part of multilevel airway stenosis. Careful evaluation is required to distinguish neural causes of impaired vocal fold motion from true mechanical fixation, as the underlying etiology can influences surgical decision-making. When addressing glottic-level stenosis, particular attention must be paid to the potential effects on voice and swallowing while striving to achieve adequate airway caliber.

Jacob's Syndrome and Hearing Loss: A Case Study.

Jacob's syndrome, or XYY syndrome, is caused by the presence of an extra Y chromosome in each male human cell. Although this extra Y chromosome makes these people taller than the average, they do not show any other unusual physical characteristics. Until now, very few studies have specifically examined the hearing abnormalities of individuals with this syndrome. A four-year-old boy with Jacob's syndrome was referred for an auditory assessment due to speech delay. The test battery results revealed a conductive hearing loss bilaterally based on existing air-bone gaps in pure-tone audiometry, abs

White Matter Microstructural Correlates of Auditory Brainstem Responses in Patients With Charcot-Marie-Tooth Disease.

INTRODUCTION: Some patients with Charcot-Marie-Tooth disease (CMT) exhibit prolonged auditory brainstem response (ABR) latencies or abnormal waveforms, suggesting potential damage to the peripheral auditory nerve or central auditory pathways. Diffusion tensor imaging (DTI), a non-invasive neuroimaging technique, can detect the integrity and functional properties of white matter structures with high sensitivity. However, research on the association between DTI characteristics and ABR changes in patients with CMT remains relatively limited, and whether both modalities reflect synergistic damage

Making Preference Assessments More Acceptable and Effective for People with Dementia.

External auditory canal atresia (EACA) is a common otologic condition. Etiology can vary from congenital to acquired causes. It causes considerable difficulty to the patient. Bilateral ear canal atresia in children can lead to speech delays due to hearing impairment caused by this condition. Though easily diagnosed it is one of the most difficult conditions to treat. Acquired conditions can affect any age group. Restenosis following treatment is very common. This article focuses on the treatment of EACA due to different etiologies and emphasizes on special points of surgical treatment and foll

Fibrolipoma of nasal tip: a patient with bulbous tip deformity.

Bulbous tip deformity of the nose is one of the common nasal tip deformities. In general, it is thought to occur due to an increased tip volume which alters the tip dynamics. Rarely does any tumour arising from the nasal tip produce a voluminous tip. Fibrolipoma is a rare variety of lipoma that can occur at the nasal tip and it can be suspected clinically during palpation of the nasal tip due to its altered consistency. While traditionally, removal of excess soft tissue overlying lower lateral cartilages and cephalic trimming followed by tip-defining sutures are the key steps to correct any bu

The effect of head coil configuration and channel count on the quality of double inversion recovery (DIR) MRI images.

BACKGROUND: Double inversion recovery (DIR) MRI provides high sensitivity for detecting white matter abnormalities but suffers from reduced signal-to-noise ratio (SNR) due to simultaneous suppression of multiple tissue signals. Head-coil configuration and channel count may influence the resulting image quality. METHODS: Seventeen healthy subjects underwent DIR imaging on a 3-T MRI system using both 64-channel and 20-channel head/neck coils. Quantitative image quality was assessed using SNR and contrast-to-noise ratio (CNR) measurements across multiple brain regions, with comparisons performed
