Early fidelity and long-term drift: do we achieve preoperative alignment goals in adult spinal deformity?

OBJECTIVES: To determine how often adult spinal deformity (ASD) surgery meets preoperative sagittal alignment goals and whether these corrections are durable over time, including the impact of planning frameworks and enabling technologies. METHODS: We performed a PRISMA-guided search of MEDLINE, EMBASE, Web of Science, Cochrane Library, and Scopus through June 2025. Eligible studies enrolled adults undergoing ASD surgery where preoperative alignment targets were defined by numeric thresholds (e.g., SVA, PI-LL, PT), algorithmic frameworks (e.g., Roussouly, GAP), or technology-assisted modaliti

Management of Osteomyelitis of the Distal Forearm and Wrist With Function-Preserving Debridement.

PURPOSE: Osteomyelitis of the wrist and distal forearm is rare but functionally devastating because of the extensive debridement needed for infection control and reconstructive surgery needed to regain function. Management guidelines and outcomes are poorly defined and often default to amputation as a last resort strategy for infection control. In this study, we present a series of wrist and distal forearm osteomyelitis patients managed with antibiotics and debridement, with minimal reconstruction in select cases, to help guide decisions on management and prognosis. METHODS: Patients over the

Analysis of Procedural Costs and Outcomes Between Autograft, BMP-2, and Viable Cellular Allograft in Lumbar Interbody Fusions.

STUDY DESIGN/SETTING: Retrospective, multisurgeon cohort at a single academic center with ≥2-year follow-up. OBJECTIVE: To identify dominant, modifiable drivers of intraoperative cost in lumbar interbody fusion and evaluate outcome profiles between graft types. SUMMARY OF BACKGROUND DATA: Instrumentation and biologics are major cost contributors in lumbar fusion, yet prior reports often aggregate spending into broad categories, obscuring which specific supply decisions drive expenditure. Biologics such as bone morphogenic protein (BMP) and viable cellular allografts (VCA) have achieved high

Accuracy of Magnetic Resonance Imaging-Based Vertebral and Endplate Bone Quality in Predicting Cage Subsidence in Anterior Cervical Spine Surgery: A Systematic Review and Meta-Analysis.

INTRODUCTION: Conventional radiographs and bone mineral density are commonly used to predict cage subsidence, though their accuracy can be limited by several factors. Magnetic resonance imaging (MRI)-based measurement of vertebral bone quality (VBQ) and endplate bone quality (EBQ) offer a more detailed assessment of bone quality and subsidence risk. This study aims to evaluate the accuracy of MRI-based VBQ and EBQ in predicting cage subsidence in anterior cervical spine surgery patients. METHODS: A comprehensive search was conducted across databases including PubMed, Europe PMC, ScienceDirect

Long-term return to professional wrestling after ankle arthrodesis for end-stage osteoarthritis: a 6-year follow-up case report.

Post-traumatic ankle osteoarthritis (OA) is a frequent consequence of recurrent injuries among athletes. When conservative management fails, surgical options such as total ankle arthroplasty or arthrodesis are considered. Although ankle arthrodesis (AA) is effective for pain relief, it is generally regarded as incompatible with high-impact sports due to loss of motion and altered biomechanics. We report a 41-year-old professional female wrestler with end-stage post-traumatic AO who underwent arthroscopic AA. She returned to professional competition 6 months postoperatively and continued to per

Percutaneous modified iliac screw fixation: technique and clinical experience.

PURPOSE: S1 pedicle screws are often associated with loosening and breakage. Conventional iliac screw (CIS) or S2 alar-iliac (S2AI) techniques improve stability but have their own drawbacks. The percutaneous modified iliac screw (PMIS) technique was developed to overcome these issues with a less invasive approach. METHODS: This study is a retrospective case series conducted by a single surgeon. The PMIS technique uses two virtual fluoroscopic reference lines to avoid critical structures and allows percutaneous insertion of the screw into the intra-iliac cancellous corridor. Between 2014 and 2
