Traumatic Occipitocervical Distraction Injuries in Children: A Systematic Review.

BACKGROUND: Occipitocervical distraction injuries (OCDI) in children occur on a wide spectrum of severity, and decisions about treatment suffer from a lack of rigorous guidelines and significant inter-institutional variability. While clear cases of frank atlanto-occipital dislocation (AOD) are treated with surgical stabilization, the approach for less severe cases of OCDI is not standardized. These patients require a careful assessment of both radiographic and clinical criteria, as part of a complex risk-benefit analysis, to establish whether occipitocervical fusion (OCF) is indicated. Manipulation Under Anesthesia in Infants With Arthrogenic Newborn Torticollis: A Retrospective Case Series.

OBJECTIVE: The purpose of this report is to describe the manipulation under anesthesia (MUA) treatment of 6 infants with newborn torticollis with a segmental dysfunction at C1/C2. CLINICAL FEATURES: Six infants aged 4 1/2 to 15 months previously diagnosed with newborn torticollis were referred to a doctor of chiropractic owing to a failure to respond adequately to previous conservative therapies. Common physical findings were limited range of motion of the upper cervical spine. Radiographs demonstrated rotational malpositions and translation of atlas on axis in all 6 infants, and 1 had a subl

Cervical Spine Involvement in a Challenging Case of Accidental Strangulation.

Although strangulation is generally homicidal, the scientific literature reports several cases in which it is suicidal or even accidental. The former eventuality is particularly interesting because extremely atypical ligatures (such as locks of hair), complex dynamics, and uncommon findings can be involved. Only a meticulous evaluation of the body and of both direct and circumstantial evidence can help in the complex differential diagnosis that includes murder, suicide, and accident. In accidental strangulation, the number and severity of injuries depend on the magnitude of applied force. This

Nonmetallic posterior monosegmental cervical fusion of a dislocated C6/7 fracture in a 4-year-old girl : A case report.

PURPOSE: Pediatric cervical spine injuries constitute approximately 1-2% of all pediatric trauma cases. Usually pediatric vertebral injuries appear as stable A type fractures, whereas B and C type injuries are relatively uncommon. In contrast to adults, the appropriate treatment strategy in children is still controversial and places spine surgeons in complex situations. This article reports the case of a 4-year-old girl with an unstable B type injury at the C6/7 level (AOSpine C6-7: B2 [F4 BL, C7:A1]) with bilateral locked fractures of the facet joints after falling down a flight of stairs. Biomechanical Rationale for the Development of Atlantoaxial Instability and Basilar Invagination in Patients with Occipitalization of the Atlas: A Finite Element Analysis.

OBJECTIVE: Occipitalization of the atlas (OA) often is associated with atlantoaxial dislocation and basilar invagination. The purpose of this study is to determine the biomechanical difference between normal and OA conditions in the craniovertebral junction and to further explore the rationale for development of atlantoaxial dislocation and basilar invagination using the finite element model (FEM). METHODS: A ligamentous, nonlinear, sliding-contact, 3-dimensional FEM of the occipitoatlantoaxial complex was generated. Validation of the model was accomplished by comparing kinematic predictions

Management of Unique Basilar Invagination Combined with C1 Prolapsing into the Foramen Magnum in Children: Report of 2 Cases.

BACKGROUND: Basilar invagination (BI) is a developmental anomaly and commonly presents with neurologic findings. The incidence of BI associated with other osseous anomalies of the craniovertebral junction is high, including incomplete ring of C1 with spreading of the lateral masses, atlanto-occipital assimilation, hypoplasia of the atlas, basiocciput hypoplasia, and occipital condylar hypoplasia. However, BI combined with C1 prolapsing into the foramen magnum (FM) is an extremely rare condition. CASE DESCRIPTION: A previously healthy 6-year-old girl (case 1) presented with extremities numbnes

Traumatic Atlantoaxial and Fracture-Related Dislocation.

Traumatic atlantoaxial dislocation due to ligamentous and combined osseous injuries rarely occurs in adults. There are only few cases published in the literature. In this level 4 study, a cohort of nine consecutive patients suffering from traumatic atlantoaxial dislocation has been analyzed regarding morphology of injury, trauma mechanism, and outcome since 2007. Three types of those injuries have been found regarding direction of dislocation indicating the underlying ligamentous injuries as well as the accompanying grade of instability. Firstly, there was rotatory dislocation, if the alar lig

The Chiropractic Vertebral Subluxation Part 3: Complexity and Identity From 1908 to 1915.

OBJECTIVE: The purpose of this paper is to review and discuss the history of chiropractic vertebral subluxation (CVS) between 1908 and 1915. DISCUSSION: Evidence from the works of Daniel D. Palmer, Bartlett J. Palmer, Joy Loban, Willard Carver, James Firth, Alva Gregory, John Howard, Arthur Forster, and Harold Swanberg demonstrates that chiropractic during this period was characterized by increasingly complex theories of CVS, which contributed to defining the profession. Critiques of CVS as a central identifier of the profession begin with this early period when students of D. D. Palmer's ear

The Chiropractic Vertebral Subluxation Part 4: New Perspectives and Theorists From 1916 to 1927.

OBJECTIVE: The purpose of this paper is to review and discuss the history of chiropractic vertebral subluxation (CVS) between the years 1916 and 1927. DISCUSSION: Theories during this period were shaped by many chiropractic school leaders and instructors. Unique contributions to theories during this period come primarily from 4 authors, John Craven, Jim Drain, Shelby Riley, and Ralph Stephenson. This period included the first thermographic instrumentation in chiropractic, which led to one of Craven's modifications of CVS theory. He also added to the literature about spinal cord pressure and d

The Chiropractic Vertebral Subluxation Part 5: The First Research Era From 1928 to 1949.

OBJECTIVE: The purpose of this paper is to review and discuss the history of chiropractic vertebral subluxation theories between 1928 and 1949. DISCUSSION: Theories during this period grew in complexity and developed in 4 primary ways: upper cervical models, reflex models, global models, and models based on the work of Speransky. Authors, theorists, and technique developers during this time included B. J. Palmer, R. J. Watkins, Galen Price, John Hurley, Hugh Logan, Major Bertrand DeJarnette, Richard Van Rumpt, Roy Ashton, Joseph Janse, Henri Gillet, James Firth, and J. R. Verner. At least 8 p

The Chiropractic Vertebral Subluxation Part 7: Technics and Models From 1962 to 1980.

OBJECTIVE: The purpose of this paper is to review and discuss the history of chiropractic vertebral subluxation (CVS) theories and models between 1962 and 1980. DISCUSSION: This period was marked by several innovative texts from Weiant, Homewood, and Harper, and Canadian Memorial Chiropractic College's Segmental Neuropathy. Technique developers during this period increased the complexity of models from upper cervical to full spine. The textbooks built upon previous theories, the scientific literature, work by Speransky, and instrumentation research. The texts influenced decades of research an

The Chiropractic Vertebral Subluxation Part 9: Complexes, Models, and Consensus From 1979 to 1995.

OBJECTIVE: The purpose of this paper is to review and discuss the history of chiropractic vertebral subluxation (CVS) between 1979 and 1995, with an emphasis on complexes, models, and consensus. DISCUSSION: New models were developed and critiqued during this period. The first subluxation complex model was proposed by Faye. Other theorists such as Lantz and Dishman built upon his model. The complex models were integrated into consensus statements by the International Chiropractic Association and the American Chiropractic Association, and later by profession-wide processes. The plurality of the

The Chiropractic Vertebral Subluxation Part 10: Integrative and Critical Literature From 1996 and 1997.

OBJECTIVE: The purpose of this paper is to review and discuss the history of chiropractic vertebral subluxation (CVS) during 1996 and 1997. The literature during this period offered critical and integrative models emphasized by a need for research into operational and functional definitions. DISCUSSION: Several integrative approaches emerged, from Rome's 296 synonyms to Bergman's Pain/Tenderness, Asymmetry/Alignment, Range of Motion Abnormality, Tissue Tone, Texture, Temperature Abnormality, and Special Tests (PARTS) analysis adopted by the profession in the United States. Other noteworthy co

Spontaneous Temporomandibular Joint Dislocation.

Dislocation of the temporomandibular joint (TMJ) is the displacement of the head of the condyle from its normal position in the glenoid fossa. Anterior dislocations are the most common type of TMJ dislocations. Prognathism of the lower jaw, anterior crossbite, and open bite are the classic features in bilateral TMJ dislocations. The treatment of acute TMJ dislocation involves manual reduction with or without general anesthesia. The use of maxillomandibular fixation is reserved for chronic, more complex dislocations. Determining the presence of a predisposing factor is very important in guiding

Hip Radiograph Findings in Patients Aged 40 Years and Under with Posterior Pelvic Pain.

BACKGROUND: Several sacroiliac joint (SIJ) provocative tests used to assess posterior pelvic pain involve moving and stressing the hip. It is unknown if there is a subgroup of patients with posterior pelvic pain who have underlying hip deformity that could potentially influence performance and interpretation of these tests. OBJECTIVE: To describe the prevalence of radiographic hip deformity and hip osteoarthritis in a group of adults 40 years old and under who met the clinical diagnostic criteria for treatment of posterior pelvic pain with an image guided intra-articular SIJ injection. DESIG

Isolated multiple lumbar transverse process fractures with spinal instability: an uncommon yet serious association.

PURPOSE: Isolated vertebral transverse process fractures of thoracolumbar spine without other vertebral injuries and neurological deficit are generally considered as minor injuries with no concern for associated spinal instability. This report describes a case of multiple lumbar transverse fractures associated with an unexpected yet clinically significant spinal instability. METHODS: A young male presented with right flank pain following being pushed and trapped against the ground by a reversing truck. The neurological examination was normal, and computed tomography (CT) imaging revealed mult

Management of an odontoid synchondrosis fracture causing chronic translational anterior atlanto-axial subluxation in a child with autism: case report.

The authors report an unusual case of an odontoid synchondrosis fracture causing chronic translational anterior atlanto-axial subluxation and present a discussion of the unique management of this case. Traumatic translational anterior atlanto-axial subluxation is a rare manifestation within pediatrics. Patients with preexisting abnormalities in ligamentous or bony structures may present with unusual symptomatology, which could result in delay of treatment. A 6-year-old male patient with autism who presented with acute respiratory arrest was noted to have an odontoid synchondrosis fracture and

Implications of Spinopelvic Mobility on Total Hip Arthroplasty: Review of Current Literature.

Understanding the impact of pathologic spinopelvic mobility on total hip arthroplasty instability requires an appreciation of the dynamic interplay between and the spine, hip and pelvis. This complex interdependent relationship changes with position, pathology and surgical intervention. Spinal pathology may prevent normal dynamic motion leading to spinopelvic stiffness and abnormal pelvic position. Patients at high risk for pathologic spinopelvic motion and subsequent total hip arthroplasty (THA) dislocation should be assessed with a functional imaging series with lateral standing, sitting and

Repeat Radiography in Monitoring Structural Changes in the Treatment of Spinal Disorders in Chiropractic and Manual Medicine Practice: Evidence and Safety.

There is substantial evidence for normal relationships between spine and postural parameters, as measured from radiographs of standing patients. Sagittal balance, cervical lordosis, thoracic kyphosis, lumbar lordosis, pelvic tilt, and the more complex understanding of the interrelations between these essential components of normal stance have evolved to where there are known, established thresholds for normalcy. These spinal parameters are reliably measured from X-ray images and serve as goals of care in the treatment of spine and postural disorders. Initial and follow-up spinal imaging by X-r
