Polygenic risk score from steroid-sensitive nephrotic syndrome GWAS indicates overlapping genetic basis with steroid-resistant cases.

Pediatric idiopathic nephrotic syndrome (INS) includes steroid-sensitive (SSNS) and steroid-resistant (SRNS) forms, which differ in treatment response and prognosis. We constructed a polygenic risk score (PRS) using summary statistics from the largest pediatric SSNS GWAS meta-analysis (2,440 cases and 36,023 controls) and evaluated it in an independent Chinese cohort of 2,507 controls, 123 SRNS patients, and 493 SSNS patients. ANOVA showed significant between-group differences (F (2, 3120) = 59.42, P < 2 × 10⁻¹⁶). Post-hoc tests revealed higher PRS in SSNS than controls (P < 0.001), with SRN

Corticosteroids augment cyclosporine nephrotoxicity in pediatric nephrotic syndrome: potential role of alternatively activated macrophages.

BACKGROUND: Cyclosporine A (CsA) is an effective steroid-sparing agent for steroid-dependent nephrotic syndrome (SDNS); however, long-term use can cause chronic kidney injury (CsA nephropathy). We previously reported that alternatively activated macrophages (M2-type) are linked to interstitial fibrosis in progressive kidney disease. This study aimed to investigate the potential involvement of M2-type macrophages in CsA nephropathy in SDNS. METHODS: Thirty-three children with SDNS treated with CsA for more than 2 years were investigated. Fourteen age-matched SDNS children without CsA served as

C3 glomerulonephritis with light chain crystalline podocytopathy and non-crystalline proximal tubulopathy: a case report and integrative clinicopathologic analysis.

This report describes a novel renal triad consisting of C3 glomerulonephritis (C3GN), light chain crystalline podocytopathy (LCCP), and non-crystalline light chain proximal tubulopathy (NC-LCPT) in a 41-year-old male presenting with nephrotic syndrome and rapidly progressive renal failure. Investigations revealed monoclonal IgG kappa paraproteinemia and isolated C3 hypocomplementemia. Renal histopathology demonstrated three distinct lesions: (1) C3GN with mesangial and capillary wall C3 deposits; (2) LCCP, featuring intralysosomal κ-restricted crystalline deposits in podocytes; and (3) NC-LCPT

Role of renal doppler in children with idiopathic nephrotic syndrome.

BACKGROUND: Renal Doppler can measure intrarenal vascular resistance and may help determine the degree of intrarenal damage, as well as predict subsequent kidney function impairment. However, its utility in children with an underlying kidney disease such as nephrotic syndrome has not been widely examined. This work aimed to measure serial renal resistive index (RI) in pediatric patients with idiopathic nephrotic syndrome to assess its predictive value for steroid resistance and disease outcome. METHODS: This prospective cohort study included 60 patients with idiopathic nephrotic syndrome aged

Development of machine learning-driven non-invasive diagnostic models for idiopathic membranous nephropathy in Chinese patients.

BACKGROUND: Idiopathic membranous nephropathy (IMN) is a major cause of nephrotic syndrome and end-stage renal disease, but the gold-standard diagnostic method is invasive. This study aims to develop a non-invasive diagnostic model for IMN, focus on the diagnostic value of anti-phospholipase A2 receptor antibody (anti-PLA2R-Ab). PATIENTS AND METHODS: In this single-center retrospective study,we included 9524 patients with chronic kidney disease patients who received renal biopsies, extracted 139 clinicopathological data from their records, and divided them into two groups based on pathologica

Severe Diabetic Gastroparesis and Complex Discharge Planning: Home IV Cyclizine As the Only Solution.

Gastroparesis is a chronic disorder characterised by delayed gastric emptying in the absence of a physical blockage. Gastroparesis can be clinically classified as mild (Grade 1), moderate (Grade 2), and severe (Grade 3), based on the severity of symptoms, with grade 3 being refractory or intractable symptoms that are not controlled despite medical therapy. Gastroparesis can occur in both type 1 and type 2 diabetes; it is more common in type 1 diabetes. Severe gastroparesis in type 1 diabetes mellitus (T1DM) can be very difficult to manage, especially when accompanied by multi-organ complicatio

Single relapse at ≥ 0.5 mg/kg alternate-day prednisone predicts course of childhood nephrotic syndrome.

BACKGROUND: Around half of children with steroid-sensitive nephrotic syndrome are eventually diagnosed with frequently relapsing or steroid-dependent disease. To minimize steroid toxicity, guidelines usually recommend switching these patients to steroid-sparing agents. Unfortunately, morbidity from steroids can be irreversible, and patients may suffer even after steroid discontinuation. METHODS: This is a multicenter observational study of children with steroid-sensitive nephrotic syndrome. We compared patients that were eventually diagnosed with frequently relapsing/steroid-dependent nephrot

ST-segment elevation myocardial infarction as the first clue to membranous nephropathy: A case report.

RATIONALE: Acute ST-segment elevation myocardial infarction (STEMI) typically results from abrupt coronary occlusion, most often caused by rupture of an atherosclerotic plaque. Membranous nephropathy (MN) typically presents with nephrotic syndrome, characterized by proteinuria. STEMI as the initial manifestation of MN is exceptionally rare. This case underscores the importance of considering systemic hypercoagulable states when evaluating atypical myocardial infarction in young adults. PATIENT CONCERNS: A 35-year-old man with no prior history of renal impairment, cardiovascular disease, or ri

Clinical value of miR-551b-5p in children with primary nephrotic syndrome and its regulatory role in disease progression.

BACKGROUND: Primary nephrotic syndrome (PNS) is a common glomerular disorder in children. Despite significant advances in glucocorticoid therapy, considerable challenges remain in the early diagnosis and prognostic management of PNS. Elevated miR-551b-5p expression has been detected in the PNS. This study aims to further explore the role of miR-551b-5p in the onset and progression of PNS and its potential mechanisms. METHODS: A total of 107 patients with primary nephrotic syndrome (PNS) and 99 healthy volunteers (HV) were included in this study. And the PNS group was further divided into subgr

Nomogram-Based Admission Model to Predict Early Hypovolaemic Shock in Paediatric Nephrotic Syndrome: Derivation and Internal Validation.

AIM: Hypovolaemic shock (HS) is an uncommon but life-threatening complication in paediatric nephrotic syndrome (NS), yet current guidelines lack a quantitative admission-time instrument to flag high-risk children. We aimed to develop and internally validate a simple model for early HS using routinely available clinical-laboratory data. METHODS: We performed a single-centre observational study including 258 children with NS admitted to a tertiary paediatric hospital. HS within 24-h of admission was the outcome. Candidate predictors were routine admission variables. Multivariable logistic regre

Efficacy and Safety of Therapies for Pediatric Steroid-Resistant Idiopathic Nephrotic Syndrome: A Systematic Review of the Last Decade.

Steroid-resistant nephrotic syndrome (SRNS) represents a major therapeutic challenge in pediatric nephrology, being associated with poor prognosis and increased risk of progression to chronic kidney disease. Over the past decade, several therapeutic strategies have been evaluated, but evidence regarding their efficacy and safety remains heterogeneous. The main objective of this study is to systematically review the efficacy and safety of therapies for pediatric idiopathic SRNS. We systematically searched PubMed/MEDLINE, SCOPUS, Web of Science, ScienceDirect, BMC Pediatrics, and Cochrane Librar

Opportunities and challenges in recurrent diffuse podocytopathy post-transplantation: the critical value of the definition.

Diffuse podocytopathy (DP) is a clinical and pathological entity, which comprises minimal change disease and primary focal segmental glomerulosclerosis (FSGS). It is characterized by diffuse podocyte foot process effacement resulting in nephrotic syndrome. Cumulative evidence supports that DP is a complex disease caused by circulating permeability factors. Following kidney transplantation, DP may recur and severely compromise graft survival. However, prior studies aiming to define immune and genetic factors implicated in disease recurrence have been limited by small cohorts and lack of utilizi

FSTIV score is essential for clinical management and predicts outcome of idiopathic membranous nephropathy.

BACKGROUNDS: The FSTIV score, an acronym derived from Focal, Segmental, Tubular, Interstitial, and Vascular, is a histopathological scoring system based on light microscopy that incorporates four parameters: focal segmental glomerulosclerosis (FSGS), tubular atrophy (TA), interstitial fibrosis (IF), and vascular hyalinosis (VH). This study aimed to investigate the associations among the FSTIV score, clinicopathological characteristics, and initial treatment response in patients with idiopathic membranous nephropathy (IMN). METHODS: We retrospectively enrolled 139 patients with biopsy-proven IM

Efficacy of iptacopan in idiopathic immune complex-mediated membranoproliferative glomerulonephritis refractory to conventional treatment.

Membranoproliferative glomerulonephritis due to immune complexes (MPGN-IC) idiopathic is a diagnosis of exclusion, made after ruling out the most common etiological processes associated with this pattern of glomerular injury (infectious, autoimmune diseases, gammopathies, among others). Idiopathic MPGN-IC shares with C3 glomerulopathy the activation of the alternative complement pathway, often evidenced by decreased serum C3 levels. Currently, there is no specific treatment for this type of glomerulonephritis; however, various immunosuppressive agents (such as prednisone, mycophenolate, cyclop
