Rare disease gene discovery is limited by small cohorts and the frequent absence of matched controls. We present the Case-Only Burden Test (COBT), a gene-based burden test for case-only designs accounting for multiple variants per individual and additive effects. COBT uses a Poisson model to test for excess variants in a gene relative to expectations from population mutation rates. Simulations show high power and competitive performance versus case-control burden tests. Validation on 1000 Genomes data demonstrated good model fit and low false-positive rates. Applied to 478 ciliopathy patients,

Hereditary tubulointerstitial kidney diseases typically manifest as slowly progressive chronic kidney disease. Rapidly progressive kidney failure with non-cystic nephromegaly is an exceptionally rare presentation posing significant diagnostic challenges. We describe a 64-year-old man presenting with rapidly progressive kidney failure and bilateral non-cystic kidney enlargement. Initial imaging, including computed tomography (CT) and magnetic resonance imaging (MRI), revealed no cysts. Kidney biopsy showed tubular ectasia and basement membrane abnormalities consistent with tubulointerstitial di

PURPOSE OF REVIEW: Transthyretin amyloid cardiomyopathy (ATTR-CM) is a type of progressive cardiomyopathy that is characterized by the accumulation of misfolded transthyretin protein within the myocardium. Cardiorenal dysfunction is common among patients with ATTR-CM, and its presence is associated with worse outcomes. As such, monitoring kidney dysfunction has important implications for disease prognostication. There remains an unmet need to better characterize renal biomarkers to enhance risk stratification and assessment of treatment response in affected patients. The goal of this review is

Familial hypomagnesaemia with hypercalciuria and nephrocalcinosis (FHHNC) is a rare autosomal recessive tubulopathy caused by mutations in the CLDN16 or CLDN19 genes, patients usually develop hypomagnesemia, hypercalciuria, nephrocalcinosis and renal failure early in life, and those with CLDN19 gene mutations have ocular findings in addition. Five children from four non-consanguineous Chinese Han families presenting with nephrocalcinosis and renal insufficiency were enrolled in our study. Metabolic profiling and radiology examinations were performed, in addition to whole exome sequencing (WES)

Steroid-resistant nephrotic syndrome (SRNS) in childhood frequently reflects monogenic podocytopathies in which immunosuppression is ineffective. Biallelic variants in MYO1E, encoding the class I myosin Myo1E, cause a distinctive form of focal segmental glomerulosclerosis (FSGS) often accompanied by "Alport-like" multilamination of the glomerular basement membrane (GBM). Early recognition has therapeutic and prognostic implications. A previously healthy 4-year-old boy presented with generalized edema and nephrotic-range proteinuria. Glucocorticoids induced no remission; sequential calcineurin

Diabetic nephropathy (DN) is one of the most common microvascular complications of diabetes and is the primary cause of end-stage renal disease, imposing a significant burden on patients' health, the medical and economic systems. The traditional view holds that the pathogenesis of DN is mainly related to metabolic disorders caused by hyperglycemia, genetic susceptibility, and abnormal hemorheology. However, with the deep integration of immunology and nephrology research, more and more evidence indicates that immune factors play a core role in its pathogenesis. The renal immune microenvironment

Increased concentrations of neuroblastoma suppressor of tumorigenicity 1 (NBL1) in the blood have been associated with disease progression in diabetic kidney disease (DKD) and IgA nephropathy. However, it is unclear whether NBL1 is a causal factor for kidney disease and what is driving these increased concentrations in the blood. To test this, we evaluated Nbl1 heterozygous knockout (Nbl1+/-) mice in two models of kidney injury, X-linked Alport syndrome (XLAS) and chronic low-dose cisplatin treatment, and compared them with wild-type (WT) controls. In parallel, we assessed serum NBL1, kidney f

BACKGROUND: Bifid nose with or without anorectal and renal anomalies (BNAR) is a rare autosomal recessive genetic congenital disorder characterized by bifid nose and renal agenesis, with or without anorectal malformations. Our research identified the genetic factors associated with BNAR in a Chinese pedigree. METHODS: Muscle tissue specimens from the fetus and peripheral blood specimens from the parents were collected, and genomic DNA was extracted for whole-exome sequencing. Sanger sequencing was used to verify the emergence of variants in the pedigree. Minigene assays and structural modeling

Autosomal dominant polycystic kidney disease is a systemic, hereditary disorder requiring life-long, multidisciplinary management. Approximately half of affected individuals progress to kidney failure by age 60 years. Recent advances in genetics, imaging, and disease pathophysiology have enabled earlier diagnosis and improved risk stratification and led to the development of disease-modifying therapies (such as tolvaptan) for high-risk patients. Despite the availability of comprehensive clinical practice guidelines, real-world implementation is hindered by disease heterogeneity, psychosocial i

BACKGROUND: Diabetic nephropathy (DN) is a major complication of Type 2 diabetes (T2D). Transforming growth factor-beta 1 (TGF-β1), a profibrotic cytokine, promotes fibrosis in DN, and its expression is affected by gene polymorphisms. This study aimed to determine the genotypic and allelic frequencies of the TGF-β1 gene (rs1800469 and rs1800470) as well as to test whether there is an association between TGF-β1 single-nucleotide polymorphisms (SNPs) and DN development in Saudi patients with T2D.
METHODS: This case-control study involved 204 samples, including 76 controls, 81 patients with T2D

KEY POINTS: Phenotypic variability remains significant among patients with the monoallelic COL4A3 p.Gly407Arg variant, despite shared environmental backgrounds. Proteinuria ≥500 mg/g is a strong predictor of renal function decline and adverse outcomes, highlighting its value as a key prognostic marker. Early nephrologic monitoring and timely initiation of renoprotective therapies, such as renin-angiotensin-aldosterone system inhibitor may delay progression to kidney failure.
BACKGROUND: Alport syndrome is a hereditary nephropathy caused by pathogenic variants in the COL4A3 , COL4A4 , or COL4A

BACKGROUND: Alport syndrome (AS) is a hereditary kidney disease characterized by hematuria, proteinuria, and progressive kidney failure. Podocyte injury is an important feature of AS, but the underlying molecular mechanisms remain incompletely understood. Secreted protein acidic and rich in cysteine (SPARC) has been implicated in kidney injury and fibrosis; however, its role in AS-associated podocyte injury is unclear. METHODS: We examined SPARC expression and localization in kidneys from Col4a3−/− AS mice and analyzed SPARC expression in urine and kidney samples from patients with AS. Mouse p

Familial adenomatous polyposis (FAP) is a genetic disorder primarily driven by mutations in the adenomatous polyposis coli (APC) gene, leading to hereditary colorectal cancer. Without intervention, FAP almost invariably progresses to colorectal carcinoma. Due to the large molecular weight of APC protein, traditional drugs fail to restore its function. In this study, we conducted a statistical analysis of APC gene mutations, optimizing key functional peptide sequences for therapeutic delivery. Using a clinically validated ionizable lipid-based lipid nanoparticle (LNP) formulation, we reproduced

Dent disease 1, an X-linked recessive proximal tubulopathy most commonly caused by CLCN5 variants, often presents with heterogeneous and nonspecific phenotypes that hinder clinical diagnosis in the absence of molecular data. We investigated a Chinese kindred with suspected hereditary renal disease using whole-exome sequencing and comprehensive in silico analyses and identified a novel frameshift variant in CLCN5 (NM_001127899: c.2359dupG/p.R788Afs*24). Segregation analysis showed the proband to be hemizygous, with his mother and daughter as heterozygous carriers. Pathogenicity prediction, doma

Autosomal dominant tubulointerstitial kidney disease (ADTKD) is a rare hereditary disorder characterized by slowly progressive loss of kidney function with absent or mild proteinuria and tubulointerstitial fibrosis. Mutations in the UMOD gene represent one of the primary genetic etiologies of ADTKD. This study presented the clinical data and genetic variant interpretation of three ADTKD patients diagnosed via a next-generation sequencing (NGS)-based diagnostic workflow at the Department of Nephrology, Zhejiang Provincial People's Hospital, along with a review of relevant literature. The most frequent causative genes in podocytopathies are NPHS1 and NPHS2 that encode the main slit diaphragm components nephrin and podocin, respectively. The significance of the nephrin-podocin interaction has remained elusive. The NPHS2 R229Q variant is unique in human genetics as it is only pathogenic when trans-associated to specific 3' variants causing R229Q podocin to form distorted heterooligomers. To examine the effect of podocin on the distance between nephrin molecules, nephrin constructs labeled with YPet or mCherry/mRuby were transitionally coexpressed in HEK-293 cells. We assessed

BACKGROUND: Dent disease is a hereditary kidney tubular disorder caused by pathogenic variants in either the CLCN5 (Dent disease 1) or OCRL1 (Dent disease 2) genes. As a rare genetic disorder, Dent disease often presents with variable clinical manifestations, leading to frequent misdiagnosis or underdiagnosis, especially in milder cases. Consequently, limited research has addressed the long-term clinical outcomes of Dent disease.
METHODS: This retrospective multicenter cohort study was conducted at 9 hospitals in Korea. A total of 48 male patients from 44 unrelated families with genetically c

Autosomal dominant Alport syndrome (ADAS) is the least common form of Alport syndrome, the second most common monogenic cause of chronic kidney disease after autosomal dominant polycystic kidney disease. We report a 14-year follow-up of a patient with ADAS carrying a likely pathogenic heterozygous variant in COL4A3 (Gly366Arg substitution). The disease initially presented with microhematuria and no renal impairment on analysis or ultrasound. The patient progressively developed bilateral renal cysts and renal failure -a classic renal pattern of the syndrome- and bilateral hearing loss. Incident
