Serologic screening for anti-rubella immunoglobulin G antibodies in Romanian pregnant women: an unexpected rate of seronegativity.

OBJECTIVES: This study assessed the risk of rubella infection among pregnant women in Romania. METHODS: Serologic testing was conducted between 2022 and 2023 in 1,279 women to determine the presence of specific anti-rubella immunoglobulin (Ig)G antibodies using the Elecsys rubella IgG assay designed for Cobas e 411 analyzers. RESULTS: The overall seroprevalence of IgG antibodies was 87.02%, indicating protective immunity developed after past infection or vaccination. However, in 12.98% of the pregnant women, anti-rubella antibodies were not detected. Pregnant women aged 12-19 years, who shou

Molecular diagnostics in cancer patients with suspected respiratory mold infections.

Invasive fungal infections (IFIs), including aspergillosis and mucormycosis, are serious complications in cancer patients. Reliable detection of etiologic agents is challenging but important for optimal patient management. In a prospective multicenter study, we compared fungal detection by culture, specific and unspecific qPCRs, and fluorescence in situ hybridization (FISH) targeting Aspergillus and Mucorales in bronchoalveolar lavage fluid (BALF) and serum of adult cancer patients with suspected fungal pneumonia. In subgroups of patients, fungal DNA was amplified from serum. We evaluated 210

Emergency department management of measles, mumps, rubella, and varicella in pediatric patients.

Public health programs have helped decrease the prevalence and sequelae of measles, mumps, rubella, and varicella; however, misinformation regarding vaccinations and recent disruptions to vaccination programs have caused outbreaks to occur. Emergency clinicians must be able to readily identify the characteristic presentations of measles, mumps, rubella, and varicella, with the goals of optimizing supportive care, limiting the spread of disease, and giving appropriate precautionary advice for complications to patients and their families. This issue provides a review of the clinical features, di

Post-mumps encephalitis mimicking metabolic disease on brain magnetic resonance imaging: a case report.

Post-infectious encephalitis is a rare manifestation of mumps. We report the case of an 11-year-old boy who presented with pseudobulbar syndrome and parkinsonism, 1 week following mumps parotitis. Magnetic resonance imaging of the brain showed bilateral and symmetric changes in the basal ganglia with signal abnormalities in the white matter surrounding the lentiform nuclei. Lumbar puncture confirmed the presence of mumps antibodies in the cerebrospinal fluid, confirming the diagnosis of post-mumps encephalitis. Symmetric basal ganglia involvement has been described in mumps encephalitis. Howev

Measles Matters: A Clinical Overview and Update.

Measles is a highly transmissible viral disease that is preventable through vaccination. Despite global initiatives, outbreaks persist, demonstrating the need for high immunization coverage. Spread occurs via respiratory droplets and airborne particles, reinforcing the need for herd immunity. Symptoms include fever, cough, and a distinctive rash. Complications may include pneumonia, encephalitis, and in rare cases, acute disseminated encephalomyelitis and subacute sclerosing panencephalitis. Diagnosis relies on reverse transcription-polymerase chain reaction and serology tests. Two doses of th

[Subacute sclerosing panencephalitis].

Subacute sclerosing panencephalitis (SSPE) is a rare, fatal complication of measles, manifesting years after infection due to persistent measles virus in the central nervous system. An 11-year-old Syrian boy with prior measles and measles - mumps - rubella (MMR) vaccination developed progressive encephalopathy and a vegetative state. MRI showed diffuse white matter, thalamic, and pontine changes; SSPE was confirmed post-mortem. This case emphasises the need for high MMR vaccine coverage and the importance of clinical awareness of rare infectious diseases in patients from crisis-affected countr

Modeling Reemergence of Vaccine-Eliminated Infectious Diseases Under Declining Vaccination in the US.

IMPORTANCE: Widespread childhood vaccination has eliminated many infectious diseases in the US. However, vaccination rates are declining, and there are ongoing policy debates to reduce the childhood vaccine schedule, which may risk reemergence of previously eliminated infectious diseases. OBJECTIVE: To estimate the number of cases and complications in the US under scenarios of declining childhood vaccination for measles, rubella, poliomyelitis, and diphtheria. DESIGN, SETTING, AND PARTICIPANTS: A simulation model was used to assess the importation and dynamic spread of vaccine-preventable in

Vertical Transmission of SARS-CoV-2-Specific Antibodies and Cytokine Profiles in Pregnancy.

Despite intensive characterization of immune responses after COVID-19 infection and vaccination, research examining protective correlates of vertical transmission in pregnancy are limited. Herein, we profiled humoral and cellular characteristics in pregnant women infected or vaccinated at different trimesters and in their corresponding newborns. We noted a significant correlation between spike S1-specific IgG antibody and its RBD-ACE2 blocking activity (receptor-binding domain-human angiotensin-converting enzyme 2) in maternal and cord plasma (P < .001, R > 0.90). Blocking activity of spike S1
