Robot-Assisted Partial Nephrectomy in a High-Complexity Renal Tumor in a Horseshoe Kidney.

INTRODUCTION: Horseshoe kidney is an uncommon congenital fusion anomaly that can make renal tumor surgery especially challenging because of altered rotation, limited mobility, variable vascular supply, and an unpredictable collecting system (1-7). This video presents a robot-assisted partial nephrectomy for a high-complexity renal tumor in this setting. CASE PRESENTATION: A 33-year-old man, with ECOG 0 and no relevant comorbidities, was diagnosed with a 7.5-cm solid renal mass in the central posterior portion of the left moiety of a horseshoe kidney. The lesion had a RENAL score of 10p. Contr

The Hidden Hitchhiker: Retrocaval Ureter Masquerading as Isolated Pelviureteric Junction Obstruction.

The coexistence of true pelviureteric junction obstruction (PUJO) and a retrocaval ureter in a single, non-duplicated collecting system is exceptionally rare, and this case represents a "hidden retrocaval ureter," in which an upstream PUJO masked the classical radiological features, transforming the diagnosis into an unexpected intraoperative discovery. Our case highlights such a peculiar case, where a 70-year-old man with longstanding intermittent right loin pain was found to have right hydronephrosis and a serum creatinine of 1.7 mg/dL. Computed tomography showed a markedly dilated extra-ren

Biopsy-Proven Prostatic Metastasis From Diffuse-Type Gastric Adenocarcinoma Presenting With Bilateral Malignant Ureteric Obstruction: A Case Report.

Metastatic involvement of the prostate and bladder trigone from gastric adenocarcinoma is exceedingly rare. We report the case of a 38-year-old man with previously treated diffuse-type gastric adenocarcinoma who developed bilateral hydroureteronephrosis during surveillance. Cystoscopy demonstrated an Inflamed prostatic urethra with early papillary changes and abnormal trigonal mucosa with bilaterally narrowed ureteric orifices. Retrograde studies revealed bilateral mid-ureteric strictures without intraluminal lesions. Biopsies from the prostatic urethra and trigone confirmed metastatic gastric

Robot-assisted advancing spiral flap pyeloplasty for complex ureteropelvic junction obstruction and ureteral atresia: a preliminary experience in 27 patients.

BACKGROUND: Long-segment ureteropelvic junction obstruction (UPJO) (typically 12≥3 cm) and ureteral atresia present major reconstructive challenges due to fixed-length limitations of conventional flap techniques. This study evaluates the feasibility and safety of a robot-assisted advancing spiral flap pyeloplasty (RASFP) with dynamically adjustable flap design as a proof-of-concept for complex cases. METHODS: A single-center retrospective study (August 2022-July 2025) included 27 patients with long-segment UPJO (median stricture length 3 cm) or ureteral atresia. All patients underwent RASFP u

Small but Mighty Magnet: A Minimally Invasive Revolution in the Management of Ureterostenosis After Kidney Transplantation.

BACKGROUND: Ureterostenosis affects 1%-8.3% of kidney transplant recipients, causing graft hydronephrosis, dysfunction, and potential loss. Traditional surgical repairs are invasive, technically challenging, and have a high recurrence risk. Magnetic compression anastomosis (MCA) presents a novel, minimally invasive alternative. METHODS: A systematic case series was conducted of 14 transplant recipients with ureterostenosis who were treated with MCA between January 2018 and January 2023. All patients underwent initial percutaneous nephrostomy decompression. Under ureteroscopic guidance, a daug

Prophylactic ureteral suturing after ureterolysis in deep endometriosis: description of a conservative laparoscopic technique.

BACKGROUND: To describe a novel laparoscopic technique, prophylactic ureteral suturing (PUS), developed to reinforce structurally weakened ureteral segments following extensive ureterolysis for deep endometriosis, to avoid segmental resection or reimplantation. METHODS: In selected patients with deep ureteral endometriosis, circumferential periureteral fibrosis was dissected after cystoscopic insertion of a double-J ureteral stent. Laparoscopic ureterolysis restored ureteral mobility and revealed a mucosa-only segment, considered at high risk of postoperative stricture or rupture, resulting f

Advancing urological practice: the value of combined antegrade and retrograde ureterography for the management of complex ureteral strictures.

BACKGROUND: Complex proximal and mid-ureteral strictures, caused by calculi, inflammation, fibrosis, or iatrogenic injury, can result in hydronephrosis and renal function loss. Traditional ureterography, when used alone, has limitations in accurately assessing stricture length and detecting multiple segments. We evaluated the diagnostic value of the combined antegrade and retrograde ureterography (CARU) technique in complex strictures, focusing on stricture length, location, and severity. METHODS: This retrospective study collected data from 110 patients diagnosed with proximal and mid-ureter

Staged transurethral surgery for complete stone clearance in a rare giant infected ureteral stone: a case report.

BACKGROUND: Giant ureteral calculi are uncommon but pose significant management challenges. When associated with infection, they heighten the risk of urosepsis and complicate surgical planning, as conventional single-stage procedures may be hazardous. Optimal treatment for this rare combination remains undefined. We present a case of a giant infection-related ureteral stone successfully managed through a planned staged endourological strategy, achieving complete clearance and preservation of renal function. This report highlights a tailored approach providing a practical staging algorithm base

Post-Operative Upper Tract Reconstruction Renal Ultrasound Protocol: Safe and Effective Ureteral Stricture Surveillance and Outcomes.

INTRODUCTION: There is no standardized post-operative surveillance for upper tract reconstruction, with frequent use of nuclear medicine renal scans (NMRS). We propose a protocol utilizing renal ultrasonography (RUS) as the initial assessment tool, reserving secondary studies to evaluate patients with worse hydronephrosis or persistent symptoms. METHODS: Retrospective analysis of a prospectively designed imaging protocol at a single-institution, 2016-2022. The protocol uses screening RUS at 3 and 12 months, with NMRS or ureteroscopy (URS) as clinically indicated. Diagnostic accuracy of 3-mont

Adolescent idiopathic ureteral stricture: A multimodal diagnostic and surgical approach in resolving hydronephrosis.

RATIONALE: Idiopathic ureteral strictures in adolescents are rare and poorly understood, posing diagnostic and therapeutic challenges due to their unclear etiology and lack of pediatric-specific management guidelines. Existing evidence predominantly derives from adult studies, leaving gaps in understanding pathophysiology, optimal interventions, and long-term outcomes in younger populations. PATIENT CONCERNS: A 16-year-old male presented with asymptomatic left hydronephrosis detected during routine physical examination. Laboratory tests revealed elevated white blood cell count (12 × 10⁹/L) an
