Kidneys of 118 children of all age groups, and of 2 adults were investigated by means of senography. Normal kidneys could be identified on account of a typical echo-pattern. Deviations from this pattern were interpreted as evidence for pathological changes in the kidneys; they were correlated to the radiological and in some cases to the pathologic-anatomical findings. The results presented show that horseshoe kidney is a non-fatal anomaly of renal development that can be managed successfully by standard urologic procedures. Most clinical problems are caused by hydronephrosis and renal calculi. An analysis of forty-two cases of hydronephrosis occurring in a series of 3,400 post mortem examinations (1.2%) shows that bilharzia could not be regarded as a cause of death in any of them. Not all patients with meatal stenosis require a complete urologic study. In those asymptomatic patients presenting with a small meatus found on routine examination, a urologic study is required only for clear historical or laboratory evidence of infection. Complete evaluation is still required for those patients with hematuria or a suggestive history on physical examination. 3 cases of ureteric obstruction secondary to aortic femoral bypass grafting are presented. This complication has been infrequently reported and we feel that it may be more common than is recognised. In all other published cases the ureter lay behind the prosthesis but the authors present a surgically proved case where the ureter lay in its normal position. The aseptic vascular reconstructive surgical procedures are to be preferred to ureteric reconstruction.

The urographic findings in 59 cases of horseshoe kidney are reviewed. Significant degrees of rotation are commoner and more pronounced in the left kidney in these cases than in the right kidney. Isotope renography has been carried out in 32 babies with neurogenic bladder due to spina bifida or with other urological lesions. The tracings can be interpreted according to the standards applicable to renography in older children. Comparison of renography and intravenous urography showed a close correlation between the two tests, but renography proved to be more sensitive and informative.
