Antibiotic therapy in adult patients with infectious mononucleosis in the prehospital period - a single-center experience.

Introduction: Some viral pharyngitis can mimic streptococcal pharyngitis. The clinical similarity of infectious mononucleosis (IM) and streptococcal pharyngitis makes differential diagnosis based on the clinical picture alone tricky, and patients with IM may be treated empirically with antibiotics (ATBs).

Aim: To assess prehospital ATB use in adult patients with IM admitted to the hospital. In addition, to compare antibiotic-treated individuals and patients not treated with ATBs using selected demographic, clinical, and laboratory param

Comparison of the Association of Area-Based Deprivation Indices With Hypertension and Diabetes Control Outcomes.

BACKGROUND: Area-based deprivation indices have been linked to hypertension and diabetes outcomes, but limited information exists about their relative performance. We compared 2 area-based deprivation measures (Area Deprivation Index and Yost Index) as risk factors for hypertension and diabetes control. METHODS: This cohort study identified adults with hypertension or diabetes in a 19-county region of Southeast Minnesota covered by the Rochester Epidemiology Project. The deprivation indices were operationalized as quintiles for state and national ranks. We assessed agreement using the weighte

Hemopoietic stem cell transplantation for infectious mononucleosis-related aplastic anemia.

We report the case of a young woman who developed aplastic anemia (AA), following a serologically confirmed primary Epstein–Barr virus (EBV) infection, occurring with fever and pharyngotonsillitis, in the absence of either palpable lymph nodes or enlarged spleen. Pancytopenia persisted after EBV DNA clearance, requiring multiple red blood cell and platelet transfusions. Given the availability of a human leukocyte antigen (HLA)-matched sibling donor (MSD), hematopoietic stem cell transplantation (HSCT) from bone marrow source was performed after a non-myeloablative conditioning regimen with cyc

Epstein-Barr virus-associated lymphoma: current understanding and treatment strategies.

Epstein-Barr virus (EBV) infection is well-known for its high prevalence rate, association with several diseases including cancer and autoimmune conditions, and a wide variety of symptoms and prognosis. When acquired at a young age, primary infections are often asymptomatic; however, in adolescence and young adulthood, symptomatic infections develop, such as in infectious mononucleosis. A special feature of EBV infection is its ability to establish a latent infection in B cells which can lead to long-term infection. Subsequent cellular transformation and viral protein expression can result to

Infectious Mononucleosis: An Uncommon Presentation of a Common Disease-a Case Report.

Infectious mononucleosis (IM), caused by the Epstein-Barr virus (EBV), typically presents with fever, pharyngitis, and lymphadenopathy. However, atypical presentations may occur, complicating the diagnostic process. A 19-year-old male presented with high-grade episodic fever, cervical lymphadenopathy, and severe constitutional symptoms. The clinical presentation raised concern for serious underlying conditions due to prominent lymphadenopathy. Laboratory evaluation demonstrated marked lymphocytosis with atypical lymphocytes and positive EBV serology, confirming the diagnosis of IM. The patient

Clinical characteristics in hospitalized children with infectious mononucleosis in Shanghai China: a single-center retrospective study.

OBJECTIVE: To investigate clinical features and peripheral blood inflammatory markers in children with Epstein-Barr virus (EBV)-induced infectious mononucleosis (IM), analyze their correlation with disease severity, enable early identification of severe cases, and provide a theoretical basis for early clinical intervention. METHODS: A retrospective analysis was performed on 156 hospitalized children with EBV-related IM (January 2019-December 2024). EBV nucleic acid was detected via real-time PCR. Patients were grouped by blood EBV-DNA load (positive/negative). Demographics, clinical manifestat

A Rare Complication of Gastric Volvulus in Infectious Mononucleosis: A Case Report.

Infectious mononucleosis (IM), typically resulting from Epstein-Barr virus infection, is characterized by the clinical triad of fever, pharyngitis, lymphadenopathy, and hepatosplenomegaly. Acute gastric volvulus secondary to massive splenomegaly represents an exceedingly rare yet life-threatening complication that requires urgent intervention. We reported the case of a five-year-old girl who was diagnosed with IM. Subsequently, the patient identified a 180° gastric volvulus associated with splenomegaly. Laparoscopic surgery derotation was promptly performed, with immediate resolution of sympto

Splenic Infarcts in an Adult Male with Epstein-Barr Virus Infection: A Rare Complication.

Splenic infarction is an extremely rare complication of Epstein-Barr virus (EBV) infection. We report a case of a healthy young man who presented with fever and pharyngitis and was diagnosed with acute EBV infection. During his hospital stay, he developed sudden left upper quadrant abdominal pain. Computed tomography scan of the abdomen demonstrated hepatosplenomegaly and multiple splenic hypodense lesions suggestive of infarcts. The patient was managed conservatively and discharged on prophylactic anticoagulation therapy. He is currently under close clinical follow up to minimize the risk of

Outcomes of ME/CFS following infectious mononucleosis: seven-year follow-up of a prospective study.

BACKGROUND: Many individuals with Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) report experiencing an infectious illness prior to disease onset. Approximately 30% of cases are linked to Epstein-Barr virus (EBV) infection resulting in Infectious Mononucleosis (IM). METHODS: We examined the progression of ME/CFS following IM among a cohort of college students who were recruited before they developed the infection. This sample represented a socioeconomically and ethnically diverse population of young adults who were monitored over a 7-year period. Assessments of health status, psy

A Rare Presentation of Cytomegalovirus Mononucleosis in a Nine-Year-Old Girl: A Case Report.

Epstein-Barr virus (EBV) and cytomegalovirus (CMV) are common viruses from the Herpesviridae family. While EBV is the classic cause of infectious mononucleosis (IMN), characterized by fever, pharyngitis, cervical lymphadenopathy, and atypical lymphocytes, CMV can also cause a similar mononucleosis syndrome, though it is less frequently reported. Mononucleosis caused by both EBV and CMV is more common in preadolescent children or young adults. Here, we present a case of a nine-year-old girl presenting with a two-week history of sore throat, fever, cervical lymphadenopathy, and cough. Additional

Long-term Outcomes of Chronic Active Epstein-Barr Virus Infection in Childhood After Dominantly Infected Lymphocyte Subpopulation Management.

BACKGROUND: Chronic active Epstein-Barr virus (EBV) disease (CAEBV) is an uncommon lethal infection involving EBV-infected T cells and/or natural killer (NK) cells, often complicated by hemophagocytic lymphohistiocytosis (HLH). Patients need curative hematopoietic cell transplantation (HCT), but the indication and timing remain unclear. This study aimed to assess the prognostic value of EBV-infected lymphocyte subsets. METHODS: We analyzed 52 pediatric/young-adult patients diagnosed with persistently EBV infection from 2003 to 2025 at a single tertiary center in Japan, excluding acute infecti

Adolescent Epstein-Barr virus-associated cholestatic jaundice with porta hepatis/pancreatic head lymphadenopathy and transient CA 19-9 elevation: a case report.

BACKGROUND: Cholestatic jaundice is uncommon in adolescents. Epstein-Barr virus (EBV)-related inflammatory lymphadenopathy can mimic malignant biliary obstruction and cause a transient rise in carbohydrate antigen 19-9 (CA 19-9), complicating diagnosis. CASE PRESENTATION: A previously healthy 15-year-old girl presented with 6 days of jaundice and dark urine without fever. Physical examination showed moderate generalized jaundice with mild scleral icterus; multiple mobile, non-tender lymph nodes in the bilateral cervical regions and right post-auricular area (largest ∼2 × 2 cm); palpable liver
