Kidney transplant recipients are at increased risk for Pneumocystis jirovecii pneumonia (PJP). We aimed to determine risk factors and outcomes of the disease using a large national database. We first conducted a nested case-control study to identify risk factors associated with PJP in patients who received a kidney transplant from January 1, 1998, through December 31, 2019. We then performed a matched cohort study to evaluate clinical outcomes following the diagnosis of PJP. We matched 407 patients with PJP to 1625 uninfected controls. PJP was diagnosed at a median time of 55 months after tran

Chronic kidney disease (CKD) is common and a major contributor to increased morbidity and early mortality in people with sickle cell anemia (SCA). Urine albumin-to-creatinine ratio (uACR) is recommended to identify patients with SCA-related CKD but its utility in predicting long-term kidney dysfunction remains unclear in this patient population. In two independent, longitudinal cohorts of patients with hemoglobin SS or Sβ0-thalassemia (USA: n = 268, median follow-up 6 years; France: n = 310, median follow-up 8.2 years) we investigated the utility of uACR, as well as other clinical and modifiab

BACKGROUND: Chronic kidney disease affects around 10% of the global population and is associated with significant risk of progression to end-stage renal disease and vascular events. Aldosterone receptor antagonists such as spironolactone have shown prognostic benefits in patients with heart failure, but effects on patients with chronic kidney disease are uncertain.
OBJECTIVES: To determine the effect of low-dose spironolactone on mortality and cardiovascular outcomes in people with chronic kidney disease stage 3b.
DESIGN: Prospective randomised open blinded end-point trial.
SETTINGS: Three

Increasing evidence suggests that branched-chain amino acids (BCAAs) are associated with frailty. However, whether there is a causal relationship between them remains to be seen. In addition, the metabolism of amino acids in the body is complex, and many previous studies have neglected the effects of their metabolites. The aim of this study was to investigate the relationship between BCAAs and their metabolites and indicators of frailty through Mendelian randomization (MR) methods, so as to determine the causal relationship between the 2, and to provide potential information for preventive and

KEY POINTS: We evaluated the association between the four categories of left ventricular geometry and all-cause mortality in patients undergoing hemodialysis. Concentric hypertrophy was the most prevalent, followed by concentric remodeling, and these were associated with a high risk of mortality. Echocardiography screening for evaluating the four categories of left ventricular geometry may be useful for risk prediction in patients undergoing hemodialysis.
BACKGROUND: Left ventricular (LV) geometry, which is classified into four categories on the basis of left ventricular mass index and relati

We report the case of a 72-year-old man with end-stage renal disease on maintenance dialysis and advanced right heart failure with severe tricuspid regurgitation, chronic atrial fibrillation, and cardiac cachexia. The patient presented with profound hypotension and cardiogenic shock, leading to recurrent failure of renal replacement therapy despite inotropic support. Given the prohibitive surgical risk, transcatheter edge-to-edge repair was deemed unsuitable due to extensive annular dilation, and the patient underwent urgent percutaneous caval valve implantation with the TricValve® system. This study assessed the association between chronic kidney disease (CKD) and presbycusis and investigated whether inflammatory neutrophil percentage-to-albumin ratio (NPAR) indicators influence this relationship. This study used data from the National Health and Nutrition Examination Survey and employed methods such as restricted cubic spline analysis, multiple logistic regression analyses, and mediation analyses to explore these relationships. High-frequency hearing impairment (HI) was correlated with moderate CKD (OR = 3.25, 95% CI:1.25- 8.46, P = 0.02). HI grades were correlated with modera

Despite important advances over the past few decades, chronic kidney disease-mineral and bone disorder remains a major clinical therapeutic challenge. Traditional interventions targeting hyperphosphatemia, impaired vitamin D metabolism, and secondary hyperparathyroidism overall failed to meet expectations. This calls for a paradigm shift. The 2023 Madrid Chronic Kidney Disease-Mineral and Bone Disorder Kidney Disease: Improving Global Outcomes (KDIGO) controversies conference advocated a holistic approach to replace the current parathyroid hormone-calcium phosphate-centric approach. In this co

Chronic kidney disease (CKD) is a growing global public health concern, marked by increasing prevalence and substantial economic burden. Notably, CKD demonstrates significant physiological sex differences. Epidemiological evidence indicates that men are more susceptible to developing CKD in early to middle adulthood due to accelerated declines in renal function, whereas postmenopausal women exhibit a sharp rise in CKD incidence and progression. These findings suggest that sex hormones may play a critical regulatory role in CKD pathogenesis. The "bidirectional effects" of sex hormones are consi

BACKGROUND: Podocytopathies are a varied set of renal diseases in which podocytes are unable to perform their typical filtration function within the glomerulus. This typically leads to edema, proteinuria, and hypoalbuminemia early in life. Among podocytopathies, focal segmental glomerulosclerosis (FSGS) is characterized by histology demonstrating segmental and focal sclerosis of the glomerular tuft. FSGS affects an estimated 1-20 per one million individuals and leads to significant morbidity and mortality related to renal failure. While FSGS can be attributed to many causes, such as drug react

OBJECTIVES: Postoperative atrial fibrillation (POAF) is a frequent entity after cardiac surgery. However, its potential relationship with adverse outcomes and permanent arrhythmias remains uncertain. We sought to assess the relationship between the occurrence of atrial fibrillation (AF) in the postoperative period and its long-term recurrence.
METHODS: Patients who underwent aortic valve replacement (AVR) with a biological prosthesis between 2005 and 2023 were analyzed at our center. The incidence of atrial fibrillation in the postoperative period and the factors associated with its occurrenc

Pediatric palliative care is a narrow subspeciality dealing with specialized comprehensive care approach for children living with chronic life-limiting illness and their families. Moyamoya disease with extracranial involvement is one such rare disease, where the most appropriate curative treatment is not yet determined. It is a vaso-occlusive disorder affecting intracranial vessels commonly, and rarely extracranial vessels. It is considered as one of the infrequent causes of acute stroke in children. We report a severe case of Moyamoya disease in a child, affecting both cerebral and extra-cran

Coral reef aorta represents a distinct vascular pathology characterized by large calcified exophytic plaques that grow into the aortic lumen typically at the level of the visceral and renal arteries. In most cases, these arteries are also heavily calcified and stenosed or occluded. Symptoms vary depending on the affected artery and differ from renovascular hypertension, and renal insufficiency to cardiac decompensation and chronic limb threatening ischemia. This report describes two patients with a coral reef aorta and stenosed aortic side branches. Both patients had experienced episodes of pu

Renal artery stenosis (RAS) is a common yet under-recognized cause of secondary hypertension and acute kidney injury, especially in elderly patients with multiple comorbidities. We report the case of a 74-year-old woman with longstanding hypertension, chronic kidney disease stage 3, and tobacco use who presented with flash pulmonary edema and hypertensive emergency. Despite a negative renal duplex ultrasound, persistent clinical suspicion prompted further evaluation with renal angiography, which revealed significant right RAS. Following successful stenting, the patient showed marked improvemen

Introduction Timely diagnosis of iron deficiency is crucial for managing anemia in patients with non-dialysis-dependent chronic kidney disease (ND-CKD). However, iron status results are not always immediately available. We aimed to examine the performance of mean corpuscular volume (MCV) in diagnosing iron deficiency as a primary objective and of iron status in predicting the onset of iron deficiency as a secondary objective. Methods This retrospective cohort study included adult patients with ND-CKD from the BRIGHTEN (oBservational clinical Research In chronic kidney disease patients with ren

The dominant focus on rheumatic aetiology of mitral regurgitation (MR) often overshadows other causes. This challenges optimal management when the aetiology is non-rheumatic, as exemplified below in an 8-year-old girl, hailing from Uganda, who was diagnosed and managed as rheumatic MR with unusually elevated and persistent systemic inflammatory state with chronic renal dysfunction disproportionate to her cardiac status. Further workup led towards the possibility of early onset systemic lupus erythematosus (SLE). The valvular involvement in SLE closely resembles rheumatic heart disease (RHD). A

Axial spondyloarthritis (axSpA) is a chronic inflammatory arthritis affecting the spine and sacroiliac joints, often accompanied by extra-musculoskeletal manifestations involving the eyes, gut, and skin. Other organ systems, including the heart (aortic insufficiency), lungs (upper-lobe predominant interstitial fibrosis), and kidneys (nephritic syndrome), may also be affected. Immunoglobulin A vasculitis (IgAV), formerly known as Henoch-Schönlein purpura, is the most common systemic vasculitis in children and is typically self-limited. It is characterized by palpable purpura, arthritis or arthr
