Association Between Serum Parathyroid Hormone Levels and Femoral Bone Mineral Density in Patients with Chronic Kidney Disease Stage 3.

Background: Chronic kidney disease (CKD) is associated with early disturbances in mineral metabolism that contribute to bone fragility. Secondary hyperparathyroidism is a key component of chronic kidney disease-mineral and bone disorder (CKD-MBD) and may affect bone even during the early stages of CKD. This study evaluated the association between serum parathyroid hormone (PTH) concentrations and bone mineral density (BMD) in patients with stage 3 CKD. Methods: A multicenter cross-sectional observational study was conducted in 203 patients with stage 3 CKD. Blood and 24 h urine samples were co

Severe Euvolemic Hyponatremia Consistent With Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH) During Localized Herpes Zoster in an Older Adult.

Syndrome of inappropriate antidiuretic hormone secretion (SIADH) is an uncommon but recognized complication of herpes zoster (varicella-zoster virus, VZV) infection, and diagnostic uncertainty may arise when hyponatremia develops after antiviral therapy is initiated. We report an 89-year-old patient with hypertension, chronic kidney disease stage 3, hyperlipidemia, and prior left nephrectomy for renal cell carcinoma who presented with right neck and upper arm pain, weakness, and a unilateral dermatomal vesicular eruption consistent with herpes zoster. Oral acyclovir was started at 800 mg five

Recurrent hydronephrosis and catheter blockage, secondary to an organised bacterial mass within the bladder.

We present a frail man in his 80s with a medical history of hypertension, oesophageal cancer (2018), chronic kidney disease stage 3, hiatus hernia and a long-term catheter. He was admitted with urosepsis and repeated catheter blockages. Imaging confirmed bilateral hydronephrosis secondary to a large gas-containing bladder mass. Operative removal of the amorphous mass was performed and microbiological analysis revealed the presence of microorganisms consistent with Proteus and Aerococcus Our patient has recovered well and has been discharged back to residential care. We have found a few similar

Machine learning models to predict osteoporosis in patients with chronic kidney disease stage 3-5 and end-stage kidney disease.

Chronic kidney disease-mineral bone disorder is a common complication in patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD), and it increases the risk of osteoporosis and fractures. This study aimed to develop predictive machine-learning (ML) models to identify osteoporosis risk in patients with CKD stages 3-5 and ESKD. We retrospectively analyzed a de-identified osteoporosis database from a Taiwanese hospital, including 6614 patients with CKD stages 3-5 and ESKD who underwent bone mineral density (BMD) scans between January 2011 and June 2022. Nine ML algorithms we

Complete Revascularization of Multiple Coronary CTOs in a Single Session Using Hydrodynamic Contrast Recanalization.

BACKGROUND: Percutaneous coronary intervention (PCI) of chronic total occlusions (CTO) is technically more challenging than non-CTO PCI. The development of novel CTO-specific PCI techniques, such as hydrodynamic contrast recanalization (HDR), has led to improved technical success with low complication rates. We present a case of successful single-session PCI of multiple CTOs using HDR in an elderly patient. Our report highlights the feasibility and advantages of single-session complete revascularization in patients with multiple CTOs. CASE PRESENTATION: A 71-year-old woman with multiple comor

Variable Phenotypic Expression of PAX2 Variants in Two Lithuanian Families with Kidney Disease.

Background and Objectives: Pathogenic variants in the PAX2 gene have been associated with a spectrum of eye and kidney disorders, ranging from papillorenal syndrome (known as renal coloboma syndrome) to isolated nephrosis without kidney morphological anomalies (focal segmental glomerulosclerosis), inherited in an autosomal dominant manner. However, due to the growing number of reports of pathogenic variants in the PAX2 gene, it is observed that genotype-phenotype correlation is not always consistent. We present patients from two unrelated families with PAX2 pathogenic variants c.685C>T and c.2

Early Initiation of Guideline-Directed Medical Therapy Improves Outcomes in Heart Failure with Reduced Ejection Fraction Patients without Significant Risk of Nephrotoxicity.

BACKGROUND: Current evidence supports the early initiation of all guideline-directed medical therapy (GDMT) in patients with heart failure with reduced ejection fraction (HFrEF), but clinical inertia persists in real-world practice. METHODS: Clinical data of 343 HFrEF patients hospitalized from January 2018 to December 2019 were collected, and they were followed for at least 3 years. We analyzed the benefits of optimizing GDMT at hospital discharge, the reasons for underprescription of GDMT, and factors associated with different outcomes after multivariate adjustments. RESULTS: Starting at l
