Double jeopardy: conflict and natural disasters-driven cholera outbreaks in Sudan.

Cholera continues to be a global threat, particularly in regions affected by conflict and environmental shocks, such as Sudan. Such humanitarian crises disrupt sanitation, water, and health systems, exacerbating cholera transmission. Overcrowded camps from displacement, poor hygiene, and weak surveillance further contribute to the outbreaks even beyond national borders. This paper explores armed conflict-driven crises and population movement as epidemiological drivers of cholera. It highlights the critical need for integrated, multisectoral strategies combining vaccination, water, sanitation,

Recurrent outbreaks of cholera in Nigeria: a narrative review on the role of conflict and climate change.

Cholera remains a major public health problem in Nigeria, with recurrent outbreaks linked to weak water and sanitation services. We conducted a narrative review of published studies and reports and examined how conflict and climate variability contribute to cholera risk and how these drivers interact. Evidence shows that conflict increases cholera transmission mainly through population displacement, overcrowding, and the breakdown of water, sanitation and hygiene (WASH) systems, surveillance, and health services. Climate hazards, especially heavy rainfall, flooding, and drought, increase expos

Spatiotemporal cholera dynamics with antibiotic resistance and vaccination via demographic-epidemic data in Zimbabwe.

The diffusion of cholera epidemics and the emergence of drug-resistant strain pose significant challenges to cholera control and treatment, emphasizing the need for more effective interventions. By establishing a reaction-diffusion model of cholera with vaccination and two strains (wild and drug-resistant), we study the spatiotemporal dynamics of cholera transmission in this paper. In a spatially heterogeneous case, we derive R 0 and establish a threshold result: the disease-free steady state is globally stable if R 0 < 1 , and the disease persists if R 0 > 1 . In addition, we prove the g

[The institutionalization of public health in Spain].

This article reviews the long and complex process of institutionalizing public health in Spain, from the mid-nineteenth century to the present, highlighting its persistent structural fragility despite the accumulation of regulatory advances. Beginning with the 1855 Health Act, conceived as a response to cholera epidemics, the paper describes the gradual assumption by the State of responsibilities for protecting collective health, in parallel with the late consolidation of the Spanish state itself. It shows how, throughout the twentieth century, public health was often subordinated to a view of

Antimicrobial Resistance in Somalia: A Silent Public Health Crisis Demanding Immediate Action.

Antimicrobial resistance (AMR) is a critical global public health threat, particularly in fragile and conflict-affected settings. In Somalia, decades of armed conflict, large-scale displacement, and limited healthcare infrastructure have created conditions conducive to the emergence and spread of resistant pathogens. Available evidence indicates alarmingly high resistance rates, including reports of up to 97% methicillin resistance among Staphylococcus aureus isolates and over 90% resistance in Escherichia coli to commonly used antibiotics. These findings suggest that many standard treatment r

Biosynthesis of a Polysaccharide-based Glycoconjugate Vaccine against Neonatal Meningitis-Causing Escherichia coli O45.

Extraintestinal pathogenic Escherichia coli O45 is an emerging multidrug-resistant serotype. Herein, we developed a glycoengineered E. coli strain for efficient biosynthesis of O45 O-polysaccharide (OPS45) and the OPS45-based glycoconjugate vaccine. Systematic optimization enhanced the production of OPS45, a polysaccharide containing the rare sugars 6-deoxy-l-talose (6d-l-Tal) and N-acetylfucosamine (FucNAc), with structure confirmed by NMR. Using oligosaccharyltransferase-mediated conjugation in this glyco-optimized chassis strain, we generated a homogeneous cholera toxin B subunit (CTB)-OPS4

Population structure and phylogenetic analysis of Vibrio cholerae non-O1/O139 by whole genome sequencing.

Toxigenic Vibrio cholerae serogroups O1 and O139 are well known for causing excessive diarrhea leading to devastating cholera epidemics and pandemics. Over 200 other serogroups, usually lacking the cholera toxin, are denoted non-O1/O139 V. cholerae (NOVC), and cause vibriosis leading to sporadic gastroenteritis and other extraintestinal infections. NOVC infections are not a notifiable disease in Canada and thus underreported. From 2010 to 2023, 160 cases and a small 2018 outbreak were reported in Canada caused by NOVC, provoking considerable public health concern. In this study, 242 Canadian V

Strain-dependent toxT expression, rather than ToxT activity, governs virulence gene regulation in Vibrio cholerae.

The AraC-type transcriptional activator ToxT is a central regulator of Vibrio cholerae virulence, directly controlling expression of the major virulence genes ctxAB and tcpA. Although biotype-specific culture conditions have been widely used to study virulence gene regulation, virulence gene expression patterns are not conserved across V. cholerae strains, particularly within the El Tor biotype. In this study, we investigated strain- and allele-dependent control of toxT expression using isogenic derivatives harboring chromosomally encoded His-tagged toxT alleles, enabling direct comparison of

RND-mediated efflux couples antimicrobial resistance and hypervirulence in contemporary Vibrio cholerae.

The prevailing view in bacterial pathogenesis is that antimicrobial resistance and virulence are constrained by evolutionary trade-offs, with resistance mechanisms imposing fitness costs that attenuate pathogenic potential. Herein we document that contemporary Vibrio cholerae clinical isolates from the ongoing seventh pandemic have circumvented this paradigm by coupling multidrug resistance with hypervirulence. We examined five geographically diverse Wave 3 isolates collected between 2017 and 2019 and compared them to early pandemic strains. These contemporary isolates exhibited both broad-spe

Crystal structure of Vibrio cholerae polysaccharide lyase RbmB bound to Vibrio polysaccharide (VPS) fragments provides insights into substrate recognition and cleavage.

Exopolysaccharides are carbohydrate polymers secreted by bacteria to perform various roles including adhesion to surfaces, protection from environmental stressors, and as key components in the production of biofilms. The human pathogen Vibrio cholerae produces an exopolysaccharide called VPS, which is essential for the formation of its biofilm matrix through crosslinking interactions with a series of secreted accessory proteins. VPS consists of a repeating tetrasaccharide unit consisting of a uniquely modified α-L-gulose moiety (with N-acetyl, O-acetyl, and amide-linked glycine modifications).

Genomic and phylogenomic exploration of Vibrio cholerae strain SU129B isolated from Penaeus vannamei: Resistance, virulence, and evolutionary dynamics in bangladeshi aquaculture.

Vibrio cholerae, a globally significant pathogen, causes both endemic and epidemic cholera and has also been associated with sporadic gastroenteritis and foodborne infections. Shrimp exports are a key source of revenue and employment in Bangladesh. However, Vibrio outbreaks pose serious public health and socio-economic risks through seafood consumption and the spread of antibiotic-resistant genes from aquaculture. The current study investigates the genomic and pathogenic features of the V. cholerae strain associated with seafood, with particular focus on the aquaculture environment of Banglade

Fecal Contamination in Drinking Water: A review of detection techniques, pathogen dynamics, and sustainable remediation approaches.

Fecal contamination poses a persistent global threat to drinking water safety, especially in low- and middle-income countries with limited sanitation and treatment infrastructure. Fecal matter transmits a wide array of enteric pathogens like bacteria (Escherichia coli, Vibrio cholerae, Salmonella, Shigella), viruses (norovirus, rotavirus, adenoviruses), protozoa (Giardia, Cryptosporidium), and occasionally fungi and helminths-driving outbreaks of diarrheal diseases, cholera, and other waterborne illnesses that cause substantial morbidity and childhood mortality. This review synthesizes current

Program Evaluation of the WASHmobile PICHA7 Mobile Health and Chlorine E-Voucher Program in the Democratic Republic of the Congo.

Targeted water treatment and hygiene (WASH) programs for those residing near diarrhoea patients can be a cost-effective approach during outbreaks to reduce diarrhoea spread by targeting those at highest risk. We designed the WASHmobile mobile health (mHealth) program for high risk populations for diarrhoea. In our previous randomised controlled trials in the Democratic Republic of the Congo (DRC) (PICHA7) and Bangladesh (CHoBI7), WASHmobile delivery to diarrhoea patient households through a healthcare facility visit and mobile messages from a doctor significantly reduced diarrhoea, cholera, an

Prioritisation and risk ranking of epidemic-prone diseases for emergency preparedness and response in Eastern Africa using a multi-criteria decision analysis framework, 2023.

BACKGROUND: Resources to respond to emerging and re-emerging infectious disease emergencies are limited. AIM: To support preparedness planning and resource allocation, a prioritisation methodology was applied to rank epidemic-prone diseases in Eastern Africa. SETTING: The study took place in the eastern region of Africa representing 14 member states. METHODS: A multi-criteria decision analysis (MCDA) combined with a modified Delphi approach, adapted from the Africa Centres for Disease Control and Prevention (CDC's) continental prioritisation framework was employed. A planning team of epidem

Predictive intelligence for future vibriosis risk in the eastern United States employing Bayesian spatial modeling.

UNLABELLED: Vibrio spp. infections have increased significantly over recent decades. Advances in remote sensing and understanding of environmental parameters driving increased proliferation of pathogenic Vibrio spp. have enabled development of predictive risk models. Here, we implement Bayesian spatial modeling (INLA-SPDE) to forecast infection risks for six vibrio species-V. alginolyticus, V. cholerae non-O1/non-O139, V. fluvialis, V. mimicus, V. parahaemolyticus, and V. vulnificus-and their combined burden, i.e., non-cholera vibriosis, along the United States eastern seaboard. Capturing dynamic phage-pathogen coevolution by clinical surveillance.

Bacteria harness diverse defence systems that protect against phage predation1, many of which are encoded on horizontally transmitted mobile genetic elements2. In turn, phages evolve counter-defences3, driving a dynamic arms race that remains underexplored in human disease contexts. For the diarrhoeal pathogen Vibrio cholerae, a higher burden of its lytic phage ICP1 in patient stool correlates with reduced disease severity4. However, direct molecular evidence of lytic phages driving selection of epidemic V. cholerae has not been demonstrated. Here, through clinical surveillance in cholera-ende

Post-vaccination seroconversion and lineage-level molecular characterization of classical swine fever virus in five northeastern states of India.

Classical swine fever (CSF) is a highly contagious viral disease of pigs with major economic consequences, necessitating sustained vaccination and surveillance under India’s national control programme. The present study evaluated post-vaccination seroconversion and conducted lineage-level molecular characterization of circulating classical swine fever virus (CSFV) in five northeastern states of India. An Erns-based indirect ELISA was used to assess antibody responses in pig sera collected before and 30 days after vaccination. A marked increase in seropositivity was observed following immunizat

Mucosal-Penetrating Nanovaccine Enhances Mucosal and Systemic Immunity against Respiratory Syncytial Virus Infection.

Eliciting mucosal immunity is crucial for protecting against respiratory pathogen infections. However, the intricate structure of mucosal tissues and their immune tolerance environment pose great challenges for the development of mucosal vaccines. To avoid being expelled by the ciliary clearance system, mucosal adhesion materials, such as chitosan, have been applied to efficiently deliver antigens to the immune system. However, the antigen showed poor mucus penetration due to chitosan's strong adhesion to mucin. Herein, we applied hyaluronic acid (HA)-coated nanoparticulate vaccine, named NHCF
