Kidney parenchymal size was estimated on urograms from 22 children with unilateral vesico-ureteral reflux (VUR), 14 children with bilateral VUR and seven children with unilateral heminephrectomy. In the bilateral VUR group, one kidney was roentgenologically normal and the other was growth-retarded. The GFR was estimated in 19 of the children. The authors report a study of 37 kidneys in 30 patients with staghorn calculi undergoing isotopic investigation of renal function by sequential scintigraphy and isotopic nephrogram by gamma-camera. 7 of the patients had bilateral staghorn calculi. Tracers used: Hippuran labelled with I131 and DPTA labelled with Technetium 99 m. 62% of kidneys poorly tolerated their staghorn calculus as shown by severe impairment of the isotopic nephrogram involving the uptake phase. Five neonates with renal venous thrombosis (RVT) were studied at the onset of the disease, and four have been followed up for from 24 to 32 months after the time of diagnosis. From July 1971 until December 1978 we treated 19 male and 5 female children less than 1 year old for ureteropelvic junction obstruction. Diagnosis was based on high dose excretory urography with delayed films. Ultrasound was performed in some cases to confirm hydronephrosis. Cystoscopy and retrograde pyelography were not required. Of 32 obstructed kidneys 27 were repaired by dismembered reduction pyeloureteroplasty. A patient with tuberous sclerosis and a normal intravenous urogram 5 years previously presented with a large and palpable upper pole renal mass. Since patients with tuberous sclerosis have small bilateral hamartomas, a Wilms' tumor was suspected. In retrospect, inhomogenous nephrograms should have alerted the radiologist to the multiplicity of other small lesions. Also, a partially lucent rim should have sustantiated that the lesion was not a Wilms' tumor.

A 22-month-old boy had a Tc-99m-MDP bone scan. This revealed multiple urinary tract anomalies (bladder distention, ectopic and small right kidney, hypertrophied left kidney, and bilateral hydronephrosis). The defects, fortuitously appreciated by means of the bone images, led to the correct diagnosis of urinary retention and infection as the cause of the child's symptoms. The urinary tract anomalies were associated with the Rubinstein-Taybi syndrome.

Cis-platinum (II) diaminedichloride (CPDD) is a new chemotherapeutic agent whose major side effect is renal toxicity. Serial urography may show evidence of a diminution of renal size before there is any clinical evidence of renal failure in patients being treated with CPDD. We recommend that these patients be followed closely with serial radioisotope scanning and/or urography to detect early renal damage.

In neonates, thrombosis beginning in small renal veins and progressing to larger veins is best termed renal venous thrombosis (RVT) since the renal vein is not usually concerned. Four patients on long-term follow-up for Hippel-Lindau syndrome (HLS) are presented. All had undergone nephrectomy for renal carcinoma; in 2, carcinoma subsequently developed in the remaining kidney. Because of the tendency of these tumors to develop bilaterally, wide local resection is preferable to radical nephrectomy if the tumors can be diagnosed when small and noninvasive. CT is currently the most accurate method of regularly screening patients with HLS for asymptomatic renal carcinoma.

Proximal propagation of an occlusive distal aortic thrombus to the suprarenal level is rare, probably resulting from diminished renal blood flow, and is invariably accompanied by renal failure. The case of a 40-year-old patient who underwent an unsuccessful cadaver kidney transplantation and was treated with ethambutol and isoniazid is reported. A bilateral retrobulbar neuropathy with an unusual central bitemporal hemianopic scotoma was found. Ethambutol was stopped and only small improvement of the visual acuity followed. Isoniazid was discontinued later, followed by a dramatic improvement in the visual acuity. The hazards of optic nerve toxicity due to ethambutol are known. We emphasize the potential danger in the use of ethambutol and isoniazid.

The radiological findings in 14 female patients with the loin plain--haematuria syndrome are described. The abnormal features are found on renal arteriography, the changes being bilateral in 13 of the 14 patients. In the smaller branches of the arterial tree there are changes of irregularity, tortuosity, stasis, and occlusion. Although not all these changes are present in every patient, the nephrogram is always abnormal, with multiple small defects in the renal cortex and blurring of the corticomedullary differentiation.
