Regional variations and temporal trends of atopic diseases in Germany.

BACKGROUND AND OBJECTIVES: Prevalence rates of atopic diseases (atopic dermatitis [AD], allergic asthma [AA], and allergic rhinoconjunctivitis [ARC]) have increased in Germany. However, there is a lack of simultaneous consideration and of precise data on the spatiotemporal variation of these diseases. This study investigates the spatiotemporal variation of atopic diseases in Germany. METHODS: Analyses based on outpatient care data (2011-2020) of all statutory health insured persons in Germany. Spatiotemporal analyses of prevalence rates were performed on county levels and statistical smoothin

Antiviral effect of poly(styrene 4-sulfonate) (PSSNa) on feline calicivirus oral infections in cats-field study.

Feline calicivirus (FCV) infection causes nasal discharge, oral mucosa inflammation, ulcerations, gingivitis, and conjunctivitis, often progressing to chronic gingivostomatitis, severe pneumonia, and fatal systemic infections. With no antivirals currently available, poly(sodium 4-styrene sulfonate) (PSSNa) was identified in 2019 as a safe inhibitor in vitro. In this preliminary single-center, randomized, double-blind, placebo-controlled field study, we further characterized the PSSNa's safety profile and tested its efficacy in cats after topical oral application. Twenty-eight cats were enrolle

Non-congenital HSV-1 Keratitis in an Immunocompetent 9-Month-Old Infant.

Isolated, non-congenital herpes simplex virus type 1 (HSV-1) keratitis in immunocompetent infants is exceedingly rare and may be easily misdiagnosed, leading to delays in appropriate antiviral therapy. This case is unique in that it describes HSV-1 keratitis in a healthy 9-month-old infant presenting without systemic illness, initially treated as bacterial conjunctivitis. The patient presented with unilateral eye pain, redness, and corneal opacification. Subsequent evaluation revealed HSV-1 keratitis, and the patient was treated with intravenous acyclovir in combination with topical moxifloxac

Cutaneous Manifestations of Emerging Arbovirus Infections Including West Nile, Dengue, Zika, Chikungunya, Usutu, and Toscana Viruses: A Clinical Overview for Dermatologists.

Arboviral infections, notably those caused by West Nile virus (WNV), Dengue virus (DENV), Zika virus (ZIKV), and Chikungunya virus (CHIKV), represent a growing global health concern due to their expanding geographic distribution and evolving clinical spectrum. While systemic and neurologic complications are well recognized, cutaneous manifestations are frequently overlooked despite their diagnostic and prognostic value. These viruses, transmitted primarily by Aedes or Culex mosquitoes, share overlapping symptoms such as fever, headache, arthralgia, myalgia, and malaise but can often be disting

The research potential of A20 in psoriatic arthritis.

Psoriasis, a systemic inflammatory disorder, extends beyond its classical dermatological presentation to encompass multiple manifestations including arthritis, inflammatory bowel disease, ocular inflammation (conjunctivitis/uveitis), and cardiovascular manifestations such as aortic valve pathology. While cutaneous manifestations have been extensively characterized, psoriatic arthritis (PsA) remains challenging to investigate, primarily due to the paucity of suitable experimental models that accurately recapitulate human disease. The ubiquitin-editing enzyme A20 (encoded by TNFAIP3) emerges as

Fecal-related self-inflicted conjunctivitis in a pediatric patient with major depressive disorder.

A 16-year-old girl with major depressive disorder suffered from a recurrent polymicrobial conjunctivitis, culture-positive for Escherichia coli, that persisted over several months despite evaluation by multiple specialists and appropriate antibiotic treatment. Ultimately, a brown malodorous mass, concerning for possible fecal matter, was noted in the inferior fornix of the left eye and sent for pathology review. Pathology results of the mass revealed degenerated vegetable matter debris with associated bacterial colonies consistent with fecal matter. Discussions were held with the patient, her

Perfluorohexyloctane Ophthalmic Solution in Patients with Dry Eye Disease Undergoing Cataract Surgery: A Prospective Multicenter Study.

INTRODUCTION: Dry eye disease (DED) is a common comorbidity in patients undergoing cataract surgery. Perfluorohexyloctane ophthalmic solution (PFHO) forms a protective layer on the tear film surface to reduce evaporation and is approved for treatment of signs and symptoms of DED. Because PFHO has a long ocular surface residence time, it was of interest to assess whether PFHO interfered with preoperative biometry and keratometry measurements to affect postoperative refractive accuracy in patients undergoing cataract surgery. This study evaluated the perioperative use of PFHO in patients undergo

Seasonal variations and environmental factors associated with allergic ocular surface disease: A prospective observational study of clinical presentation patterns in South Asian region.

BACKGROUND: Allergic ocular surface disease affects 20-40% of populations globally, with seasonal patterns undocumented in south Asian populations, limiting evidence-based resource planning in resource-constrained settings. METHODS: This prospective cross-sectional study enrolled 201 consecutive patients (mean age 33.1 years, 49.8% female) from a tertiary care ophthalmology outpatient department over 12 months. Participants underwent standardized assessment: Ocular Surface Disease Index (OSDI ≥ 23), Schirmer's test (< 5 mm/5 min), and tear break-up time (TBUT < 5 s). Environmental data were o

Temperature- and pH-responsive cetirizine hydrochloride in situ gels for enhanced ocular delivery in allergic conjunctivitis.

Cetirizine eye drops provide rapid symptom relief in allergic conjunctivitis but are limited by short precorneal residence and frequent dosing. This study aimed to develop and compare dual pH- and temperature-responsive in situ gels incorporating Poloxamer and two Carbopol grades to enhance ocular retention and improve therapeutic efficacy. Two formulations were prepared using Poloxamer 407/188 with either Carbopol AA-1 (AA-Gel) or Carbopol TR-1(TR-Gel). Sol-gel transition, microstructure, rheology, erosion, drug release, irritation, precorneal retention, and pharmacodynamic performance were e

[Adult Kawasaki disease: A diagnostic case report].

Kawasaki disease (KD) is an acute systemic vasculitis mainly affecting medium-size blood vessels and primarily affecting children under five years old. The typical clinical presentation associates fever, skin rash, mucous membrane abnormalities, and lymphadenopathies. Occurrence in adults is rare, with fewer than 100 cases reported and limited histological descriptions available, particularly regarding the skin. We report herein the case of a 22-year-old man hospitalized for fever, odynophagia, and aphagia since five days. Clinical examination revealed enanthem with palatal ulcerations, maculo

Molecular requirements of pathogenic Th2 cell differentiation in allergic airway disease.

Th2 cells were originally described as a homogeneous population capable of simultaneously producing interleukin (IL)-4, IL-5, and IL-13, thereby playing a central role in allergic asthma and related conditions. Subsequent studies have revealed substantial heterogeneity within the Th2 lineage, with distinct subpopulations defined by unique surface markers and cytokine profiles. Of particular interest are pathogenic Th2 subsets, referred to as peTh2, Tpath2, or Th2A, that exhibit specialised effector functions and actively drive allergic disease. These pathogenic Th2 (pTh2) cells have been ident

Eyelid margin tattoo granuloma in an individual with systemic sarcoidosis.

Systemic sarcoidosis rarely presents as tattoo granulomas. We present a case of an eyelid tattoo granuloma as a prodrome to the diagnosis of sarcoidosis; this case is unique in its onset being several decades following the initial tattooing event, in addition to its resolution with intralesional triamcinolone. A 63-year-old female presented with ipsilateral upper and lower lid margin tattoo granulomas, 6 months post bilateral conjunctivitis. A punch biopsy of the granuloma revealed dermal non-caseating granulomatous inflammation. Serum angiotensin-converting enzyme levels were elevated, and a

Progressive resolution of reactive arthritis treated with Upadacitinib.

Reactive arthritis (ReA) is a postinfectious inflammatory disorder that may be difficult to treat in refractory cases. We describe a 20-year-old man with HLA-B27-positive ReA presenting with arthritis, conjunctivitis, balanitis, and psoriasiform skin lesions. Conventional therapies, including antibiotics, nonsteroidal anti-inflammatory drugs, corticosteroids, and sulfasalazine, failed to control disease activity. Treatment with upadacitinib (15 mg/day), a selective Janus kinase 1 inhibitor (JAK1), led to rapid improvement of joint, ocular, and cutaneous manifestations within three weeks. Corti

Identification of Treatment Targets in Allergic Conjunctivitis Through Proteome-Scale Mendelian Randomization Analysis.

PURPOSE: Allergic conjunctivitis (AC) is a condition with a rising prevalence that occasionally leads to irreversible visual impairment. However, few novel therapeutic targets have been identified for AC. The investigation aims to utilize the Mendelian randomization (MR) technique to explore the causal impacts exerted by the plasma proteome on AC. METHODS: An evaluation was conducted on a two-sample MR study utilizing 2,940 plasma proteins from the UK Biobank Pharma Proteomics Project (UKB-PPP) to investigate their causal links with AC. Confirmation of these MR results was achieved using Summ

Case Series: Combination of dupilumab and omalizumab as a way to reduce dupilumab-associated adverse events in severe atopic dermatitis.

Dupilumab is a fully human monoclonal antibody directed against the interleukin-4 receptor subunit α, the common chain of the IL-4 and IL-13 receptors. Dupilumab has been demonstrated to be highly effective in the treatment of severe atopic dermatitis, asthma, and chronic rhinosinusitis with nasal polyps. Despite the favorable tolerability of dupilumab, several publications and reviews have reported various adverse events, including conjunctivitis, keratitis, and face and neck treatment-resistant dermatitis, which reduce patients' quality of life and may necessitate the cancellation of therapy

Molecular landscape connecting complications of dry eye disease: mechanisms, therapeutic targets, biomarkers, and future innovations.

PURPOSE: This review paper is a sightseeing on the molecular pathways of the DED, which culminate in the cascades of inflammatory cytokines, apoptosis, oxidative stress, as well as the breakdown of homeostasis of the ocular surface. The article also discusses complications, and it explains the connection between conjunctivitis, keratitis, corneal ulcers, blepharitis, dysfunction of meibomian glands, recurrent corneal erosion, ocular neuropathic pain, and accentuating the systemic effects of conditions, such as Sjögren syndrome. Within the directions of such molecular pathways, the article anal
