High-resolution genomics uncovers region-specific evolution and virulence of ocular Streptococcus pneumoniae.

UNLABELLED: Streptococcus pneumoniae is the leading cause of sight-threatening ocular infection. However, comprehensive molecular epidemiological analyses that focus on ocular pneumococci are limited. We performed a high-resolution whole-genome analysis (WGS) of ocular pneumococcal isolates from the Zhejiang province of China and a global data set (n = 58). Using latex agglutination and the Quellung reaction, nine serotypes were identified, with PCV13 covering 54.5% of the Chinese isolates, and no non-typeable (NT) strains were found in conjunctivitis. Illumina short-read data were assembled d

A Diagnostic Challenge: Recurrent Uveitis Leading to the Diagnosis of Hyper-IgD Syndrome in a 28-Year-Old Patient.

INTRODUCTION: Mevalonate Kinase Deficiency (MKD), which includes the clinical phenotype known as Hyperim munoglobulin D Syndrome (HIDS), is an autosomal recessive autoinflammatory disorder caused by mutations in the MVK gene. While typically characterized by childhood-onset recurrent febrile episodes and systemic symptoms like lymphadenopathy and abdominal pain, ocular involvement is usually limited to conjunctivitis. Uveitis is a rare manifestation, often associated with more severe forms of the disease. CASE PRESENTATION: This report describes a 28-year-old male who presented with bilateral

Refractory Kawasaki Disease With a Coronary Artery Aneurysm in a Three-Month-Old Infant: Diagnostic Challenges and Rescue With Infliximab.

Kawasaki disease (KD) is an acute systemic vasculitis and the leading cause of acquired heart disease in children. Diagnosis is particularly challenging in early infancy due to incomplete presentations. We describe a three-month-old infant with persistent fever, irritability, and lymphadenopathy who was initially treated for bacterial infection. By Day 7, the patient developed conjunctivitis, cracked lips, and pharyngeal congestion, meeting criteria for incomplete Kawasaki disease. Laboratory findings included leukocytosis, anemia, thrombocytosis, elevated C-reactive protein, and hypoalbuminem

Biologic therapy is associated with reduced ocular disease in psoriasis: a real-world study.

BACKGROUND/OBJECTIVES: Psoriasis is a systemic immune-mediated disease with ocular involvement. While biologic therapies reduce cardiovascular and musculoskeletal comorbidities, their impact on ocular health is not well characterised. We aimed to assess whether biologic therapy is associated with reduced ocular disease risk in psoriasis patients. SUBJECTS/METHODS: We performed a large-scale, retrospective cohort study using the TriNetX Global Collaborative Network (>160 million patients worldwide). Adults with psoriasis initiating biologic therapy were compared with those receiving non-biolog

Clinical Management of Dupilumab-related Conjunctivitis, Erythema, and Arthralgia in Atopic Dermatitis: Evidence From Two Cases Treated with Nemolizumab.

Atopic dermatitis (AD) is the most common chronic inflammatory skin condition and typically presents with pruritus, xerosis, eczematous lesions, and lichenification. Treatment options vary depending on severity; however, core management involves trigger avoidance, daily skin care, and anti-inflammatory therapies. Dupilumab, a monoclonal antibody that blocks interleukin-4 (IL-4) receptor α, is approved for the treatment of moderate-to-severe AD in patients aged 6 months or older. While this approval represents a significant advancement in AD therapy, the full extent of its side effect profile c

Approach to the patient: therapeutic mitigation of lacrimal manifestations in thyroid eye disease.

CONTEXT: Thyroid eye disease (TED) is an autoimmune disease involving periocular and orbital tissue. The manifestations of TED include proptosis, diplopia, visual disturbance, and lacrimal symptoms. Dry/gritty eyes, light sensitivity, and tearing are prominent lacrimal system-related symptoms. They can be misdiagnosed as dry eye syndrome, chronic conjunctivitis, or allergies. Multiple therapeutic interventions can be utilized in the management of lacrimal symptoms of TED. EVIDENCE DESCRIPTION: Lacrimal system dysfunction occurs from immune cell infiltration of glandular tissues as well as fro

Case Report: Dupilumab in difficult-to-treat bullous pemphigoid at the University Hospital Schleswig-Holstein, Campus Kiel.

Bullous pemphigoid (BP) is the most common autoimmune blistering disease affecting the skin. It is characterized by a type 2 immune-mediated pathogenesis. BP predominantly affects elderly patients who have multiple comorbidities, and treatment can be particularly challenging in refractory cases or when standard immunosuppressive therapies are limited due to toxicity or contraindications. Here, we present a case series of five patients with confirmed BP who were treated with off-label dupilumab. All five patients achieved sustained clinical improvement with dupilumab treatment, including reduct

Bacilli to books: The life and legacy of John Elmer Weeks.

The story of John Elmer Weeks (1853-1949) is as impressive as it is inspiring. Based on a New Year's resolution with a friend, Weeks decided to pursue a career in medicine and worked as a full-time mechanic to fund his medical school preparation and admissions process. The field of ophthalmology, and the study of medicine at large, has much to thank for that fateful New Year's resolution. From culturing the Koch-Weeks bacillus, the causative agent of a form of infectious conjunctivitis, to publishing a landmark textbook titled "A Treatise on Diseases of the Eye," Weeks has left his mark on the

Cutaneous Clues in Kawasaki Disease: Clinical Implications and Differential Diagnosis with Multisystem Inflammatory Syndrome in Children.

Kawasaki disease (KD) and multisystem inflammatory syndrome in children (MIS-C) are pediatric inflammatory conditions with overlapping mucocutaneous features that may complicate early diagnosis. We performed a narrative review of the literature to characterize and compare cutaneous manifestations reported in children with KD and MIS-C and to assess their diagnostic relevance. Published studies describing dermatologic findings in patients aged 0-18 years were reviewed. The analysis revealed a broad heterogeneity of skin manifestations in both conditions, ranging from classic polymorphous rash a

Global Perspectives on Therapy for Noninfectious Conjunctival Hyperemia: A Narrative Review.

Conjunctival hyperemia is one of the most frequent ophthalmologic presentations and may adversely affect quality of life due to associated discomfort and aesthetic concerns. In many countries, the prevalence of noninfectious conjunctival hyperemia is increasing due to lifestyle factors and the shift to technology-based employment. Management of noninfectious conjunctival hyperemia is typically aimed at addressing the underlying cause while alleviating signs and symptoms. However, an underlying cause might not be identified, treatment may not immediately reduce redness, or residual redness migh

Navigating Potential Dengue and Its Mimics in a Febrile Returning Traveller: A Case Report.

Fever is a common symptom in returning travellers. This prompts healthcare providers to consider and investigate potentially serious infections requiring timely diagnosis and management. The following case describes a traveller returning from Bangladesh presenting with symptoms indicative of dengue. These include fever, rash, nausea, vomiting, myalgia and conjunctivitis. However, his symptoms defied the expected pattern of presentation, posing a diagnostic challenge. No laboratory confirmation of prior dengue infection was available. He was discharged as having a viral fever with a rash and th

Biological characterization, sequence type distribution and drug resistance profiling of Mycoplasma hyorhinis field isolates from pigs in Chongqing, China.

Mycoplasma hyorhinis is a ubiquitous pathogen of swine that causes polyserositis and polyarthritis and is also associated with conjunctivitis, meningitis, pneumonia, and abortions. This microorganism is a high prevalence pathogen in Chinese swine herds. However, few studies on M. hyorhinis have been reported in Chongqing, China. The overuse of antimicrobials has led to an increased risk of antimicrobial resistance, but a series of Chinese herbal monomers exhibited antibacterial activity to drug-resistant bacteria, including mycoplasmas. The aim of the study was to determine the prevalence, seq
