MITF-Overexpressing Perivascular Epithelioid Cell Tumor of the Urinary Bladder Lacking TSC1/TSC2 Alterations and TFE3 Rearrangement: A Case Report and Review of the Literature.

Perivascular epithelioid cell tumors (PEComas) are mesenchymal neoplasms characterized by melanocytic and smooth muscle differentiation, commonly associated with TSC1/TSC2 alterations or TFE3 rearrangements. Herein, we report a rare case of urinary bladder PEComa with MITF overexpression and without identifiable TSC1/TSC2 mutations or TFE3 rearrangement. A 50-year-old man underwent partial cystectomy for a bladder mass. Histologically, the tumor was well circumscribed and composed of mixed spindle and epithelioid cells with cytologic atypia and low mitotic activity. Immunohistochemistry reveal

Exosomal circ55033 promotes cigarette smoke-induced bladder cancer by modulating EMT and stem cell properties.

Bladder cancer has high morbidity and mortality, and cigarette smoke is a major risk factor for this disease. Exosomal circRNAs are widely involved in tumor progression, while their roles in smoke-related bladder cancer remain unclear. In the present study, we identified and validated circ55033, a highly expressed circRNA in clinical samples from smoking patients. Functional experiments demonstrated that cigarette smoke induced EMT and cancer stemness in bladder cancer cells via exosome-transported circ55033. Mechanistically, circ55033 acts as a molecular sponge for miR-1286 to exert its oncog

Urine-derived induced pluripotent stem cells for non-invasive diagnosis of bladder cancer.

Bladder cancer (BLCA) remains one of the most prevalent and costly malignancies worldwide, largely owing to its high recurrence rate and the invasive nature of conventional surveillance modalities. Induced pluripotent stem cell (iPSC) technology offers a new framework for disease modeling and, potentially, precision diagnostics. Urine provides a non-invasive, renewable source of exfoliated urothelial and renal epithelial cells amenable to direct reprogramming. Here, we review the biological rationale, reprogramming methodology, and diagnostic potential of urine-derived iPSCs (UDiPSCs) in the c

Piezo1 Activation by Yoda1 Inhibits Mechanosensitive ATP Release in Human Urothelial Cells and Bladder Overactivity in Rats with Bladder Outlet Obstruction.

Urothelial ATP release during bladder filling modulates afferent nerve activity and detrusor contractility to regulate the micturition reflex, yet the upstream mechanosensory mechanism linking distension to ATP release remains unclear. Here we investigated the role of Piezo1 in regulating mechanosensitive ATP release in human urothelial TRT-HU1 cells and in a rat model of bladder outlet obstruction (BOO). In TRT-HU1 cells, mechanical stress increased ATP release 60% above baseline. Bath-applied Yoda1 (1-10 µM) suppressed mechanically stimulated ATP release in a dose-dependent manner, an effect

Multidisciplinary Consensus on Improving the Care Pathway for Overactive Bladder from an Adherence Perspective: The claVHes Consensus.

Overactive bladder management within the Spanish healthcare system faces significant challenges, including suboptimal therapeutic adherence. This consensus proposes recommendations to improve the care process (both clinical and organizational) from an adherence perspective. A Multidisciplinary Working Group of 14 members critically reviewed and discussed the literature (individually and in joint sessions) and formulated recommendations around five pillars: (1) strategic planning, with a realistic medium-term horizon and progress toward common protocols; (2) technological innovation and digital

Peritoneal loose body: an incidental finding, a challenging diagnosis and multimodal assessment to avoid resections.

A man in his early 40s presented with urinary symptoms and was diagnosed with a right ureteric calculus, hydronephrosis and urinary tract infection. A non-contrast CT scan of the kidneys, ureters and bladder revealed an incidental pelvic mass in the rectovesical pouch, initially suspected as a gastrointestinal stromal tumour/fibroma. Multidisciplinary management prioritised cystoscopy-guided double-J stenting for obstruction and infection. In a second admission, ureteroscopic lithotripsy with stenting and a laparoscopic low anterior resection were planned simultaneously. Following ureteroscopi

Celastrol Sensitizes Bladder Cancer to Gemcitabine by Regulating LINC00839-Mediated Homologous Recombination DNA Repair.

Gemcitabine resistance remains a major obstacle in the treatment of advanced bladder cancer. Celastrol, a bioactive triterpenoid derived from traditional Chinese medicinal plants, has been reported to enhance chemosensitivity in multiple malignancies. However, the molecular mechanisms by which celastrol modulates gemcitabine response in bladder cancer remain largely unclear. Human bladder cancer cell lines (T24 and J82) and a gemcitabine-resistant T24 model (T24GEM) were used to investigate the chemosensitizing effects of celastrol. Cell proliferation, colony formation, and drug sensitivity we

The effect of ureteral stenting on the risk of metachronous upper tract urothelial carcinoma in patients with bladder tumors involving the ureteral orifice.

BACKGROUND: Ureteral stenting in patients with bladder cancer involving the ureteral orifice may increase the rate of metachronous upper tract urothelial carcinoma (UTUC). Nevertheless, evidence on the matter is scarce and often contradictory. MATERIALS AND METHODS: We performed an observational study comparing ureteral stenting versus no stenting with respect to the risk of developing metachronous UTUC in patients who underwent transurethral resection of bladder tumors (TURBT) involving the ureteral orifice between January 2015 and December 2024. We performed time-to-event analyses to estima

Adamantinoma-like Ewing sarcoma of the bladder.

Adamantinoma-like Ewing sarcoma (ALES) is an exceedingly rare neoplasm, typically arising in the head and neck. We present a case of bladder tumour initially diagnosed as urothelial carcinoma on transurethral resection, which after neoadjuvant dose-dense methotrexate, vinblastine, doxorubicin and cisplatin and subsequent radical cystoprostatectomy was reclassified as ALES with no residual urothelial carcinoma.

External validation and comparison of nomograms for prediction of long-term disease recurrence and survival for bladder cancer patients who undergo radical cystectomy.

BACKGROUND: This study aimed to compare existing nomograms and identify which models best predict long-term survival outcomes, namely disease-free survival (DFS) or recurrence-free survival (RFS), and cancer-specific survival (CSS), in patients undergoing radical cystectomy (RC) for bladder cancer (BC). METHODS: Six nomograms (three for DFS, three for CSS) were selected from 20 studies and evaluated using data from 730 RC patients treated at University Hospitals Leuven. Discriminatory performance was assessed using area under the curve (AUC) of receiver operating characteristics (ROC) curve,

Impact of intravesical BCG Therapy on quality of life in non-muscle-invasive bladder cancer: a prospective evaluation using the FACT-Bl questionnaire.

INTRODUCTION AND OBJECTIVE: Intravesical bacillus Calmette-Guérin(BCG) remains the standard treatment for high-risk non-muscle-invasive bladder cancer(NMIBC) and is also considered an option for intermediate and very high-risk groups. Despite over five decades of clinical use, limited information is available regarding patients' quality of life(QoL) during therapy. The objective was to evaluate QoL patients receiving BCG therapy using the Functional Assessment of Cancer Therapy-Bladder Cancer(FACT-Bl) questionnaire. MATERIALS AND METHODS: Prospective multicentre observational study from Febru

Hypoxia-Induced MicroRNAs Confer Survival Advantage in Bladder Cancer by Fine-Tuning Oncogenic Pathways.

INTRODUCTION: Bladder cancer (BC) is influenced by hypoxic conditions, which promote tumor progression and resistance to therapy. MicroRNAs (miRNAs) play key roles in regulating gene expression under hypoxia. METHODS: This study utilized an miRNA array to compare expression profiles under normoxic and hypoxic conditions. We analyzed 1,024 miRNAs, focusing on differentially expressed miRNAs. Survival analysis was performed using a cohort of 408 BC patients from TCGA (The Cancer Genome Atlas). Target prediction for hypoxia-inducible factors (HIFs) and cancer-related genes was performed. RESULT

Advancements in Bladder-Sparing Strategies for Muscle-Invasive Bladder Cancer: A Comprehensive Review.

Advancements in the management of muscle-invasive bladder cancer have increasingly focused on strategies that balance oncologic control with preservation of quality of life. The therapeutic landscape for neoadjuvant therapy is rapidly evolving as immunotherapy and novel agents are integrated into systemic therapy regimens. The utility of tumor tissue, plasma, and urine biomarkers in selecting patients for bladder preservation remains uncertain and warrants further evaluation in prospective clinical trials before incorporation into routine clinical practice. Clinicians should consider both tumo

When Small Cells Matter: Diagnosing Bladder Small Cell Carcinoma on Urine Cytology.

Small cell carcinoma of the urinary bladder (SCCB) is a rare and highly aggressive neuroendocrine neoplasm that poses significant diagnostic challenges, particularly on urine cytology. We report a case of a 68-year-old man with painless hematuria in whom SCCB was initially detected on voided urine cytology. Cytologic evaluation revealed small malignant cells with high nuclear-to-cytoplasmic ratios, nuclear moulding, finely granular chromatin, necrotic background and apoptotic debris, raising suspicion for a neuroendocrine carcinoma. Immunohistochemical studies performed on the urine cell block

An Integrin β6-Targeted Antibody-Drug Conjugate Optimized for Intravesical Delivery to Treat Non-Muscle-Invasive Bladder Cancer.

Non-muscle-invasive bladder cancer (NMIBC) accounts for 75% of all new bladder cancer diagnoses. Despite new therapeutic treatment options aiming to replace Bacillus Calmette-Guérin (BCG), the standard of care for 40+ years, high rates of failure and recurrence of disease remain in patients with NMIBC. Integrins are a large family of cell surface receptors involved in key biological processes such as cellular adhesion, motility, and cytokinesis, and integrin β-6 (IB6) has emerged as a clinically relevant target due to its restricted expression in normal adult epithelial tissues and elevated le

A Spatial Atlas of Muscle-Invasive Bladder Cancer Reveals Lineage-Specific Vulnerabilities and Immune Architecture.

UNLABELLED: Muscle-invasive bladder cancer (MIBC) is clinically heterogeneous, and current molecular subtyping does not capture the spatial organization of tumor states and microenvironmental context. In this study, we construct a spatial atlas of MIBC by integrating spatial transcriptomics from 22 tumors with matched bulk RNA and whole-exome sequencing data. We identify a continuous, spatially organized luminal-to-basal axis within individual tumors that is associated with greater chromosomal instability and transcriptional plasticity. Luminal tumor cores are enriched for FGFR3 and NECTIN4, w
