[UROTHELIAL CARCINOMA WITH SARCOMATOID SUBTYPE OF THE BLADDER ACCOMPANIED BY TROUSSEAU'S SYNDROME: A CASE REPORT].

A 56-year-old male patient underwent transurethral resection of a bladder tumor (TURBT), the pathological diagnosis of which was urothelial carcinoma with sarcomatoid subtype, stage T1. Despite the absence of lesions in the upper urinary tract, multiple slightly enlarged para-aortic lymph nodes measuring less than 1 cm were detected on computed tomography. One year after TURBT, the patient exhibited recurrent thromboembolic events despite receiving anticoagulant therapy. As the para-aortic lymph nodes continued to enlarge, an open lymph node biopsy was performed, which revealed urothelial carc

Tumor-intrinsic execution of interferon-γ-associated immunity in bladder cancer: checkpoints in signaling, recruitment, and recognition.

Interferon-γ (IFN-γ) is a central mediator of antitumor immunity in bladder cancer, but an IFN-γ-rich tumor microenvironment does not necessarily translate into tumor control. Tissue-level inflammatory signatures capture overall immune activity and cannot determine whether malignant urothelial cells remain responsive to immune-derived IFN-γ or preserve the downstream functions required for immune elimination. This Mini Review examines tumor-intrinsic IFN-γ-associated immunity at three functional checkpoints: proximal IFN-γ sensing and signal transduction, chemokine-mediated effector-cell recru

[MALACOPLAKIA OF THE URINARY BLADDER: A CASE REPORT WITH DIAGNOSTIC DIFFICULTY].

We herein report a rare case of malacoplakia of the urinary bladder that demonstrated significant diagnostic difficulty. The patient was a 65-year-old woman with a history of breast cancer, dermatomyositis, steroid-induced diabetes mellitus, and cutaneous cryptococcosis. She presented with repeated episodes of dysuria and hematuria since the age of 62. Computed tomography (CT) revealed progressive bladder wall thickening, and she was therefore diagnosed with cystitis by her primary care physician and treated with antibiotics. Her symptoms worsened at 63 years of age, and further bladder wall t

CYP24A1 rs1570669 and rs2762934 polymorphisms and bladder cancer susceptibility: a Turkish case-control study.

BACKGROUND: Bladder cancer (BC) is one of the most common malignant tumours of the urinary system and represents an important public health problem. Single nucleotide polymorphisms (SNPs) are common human genetic variations and may affect cancer risk by altering various genetic processes such as vitamin D metabolism and tumour suppression. CYP24A1 rs1570669 and rs2762934 polymorphisms may be relevant to cancer biology through their potential effects on the degradation of active vitamin D metabolites. These polymorphisms have been associated with the risk of urinary cancer, including BC, in an

Phenotypically diverse Klebsiella pneumoniae urinary tract infection isolates consistently cause pyelonephritis.

Urinary tract infections (UTIs) are a significant public health burden that impact millions of people every year and are highly prevalent among hospital-acquired infections. Klebsiella pneumoniae is globally the second most common cause of UTIs after uropathogenic Escherichia coli (UPEC). Thus far, the molecular mechanisms underlying pathogenesis are better understood in UPEC than in K. pneumoniae. UPEC is known to have virulence phenotypes such as fimbrial adhesion and evasion of complement-mediated killing. In other infection types, K. pneumoniae fitness has been associated with mucoidy and

Quantitative Evaluation of Pharmacologic Synergy for Neoadjuvant Enfortumab Vedotin Plus Pembrolizumab in Muscle-Invasive Bladder Cancer: A Copula-Based Secondary Analysis.

OBJECTIVES: Perioperative treatment with enfortumab vedotin (EV) plus pembrolizumab has demonstrated a high pathological complete response (pCR) rate in patients with cisplatin-ineligible muscle-invasive bladder cancer (MIBC). However, whether this therapeutic benefit reflects true pharmacologic synergy or can be explained by independent drug action (IDA) remains uncertain. METHODS: We analyzed data from EV-103 cohort H (EV monotherapy), the cisplatin-ineligible subset of PURE-01 (pembrolizumab monotherapy), and KEYNOTE-905/EV-303 (EV plus pembrolizumab). The combined medication approach for treating overactive bladder: assessing safety and efficacy.

RESEARCH QUESTION: What is the latest evidence on the efficacy and safety of combining medications to treat overactive bladder? THE PROBLEM: Overactive bladder symptoms negatively impact quality of life and, with an ageing population, overactive bladder prevalence is expected to rise. Current treatments do not adequately address all patients' needs, with concerns about dementia risk from antimuscarinics, and not all patients wish to proceed to invasive procedures. This highlights the need for alternative pharmacological strategies. LITERATURE REVIEW: The literature emphasises the detrimental

MITF-Overexpressing Perivascular Epithelioid Cell Tumor of the Urinary Bladder Lacking TSC1/TSC2 Alterations and TFE3 Rearrangement: A Case Report and Review of the Literature.

Perivascular epithelioid cell tumors (PEComas) are mesenchymal neoplasms characterized by melanocytic and smooth muscle differentiation, commonly associated with TSC1/TSC2 alterations or TFE3 rearrangements. Herein, we report a rare case of urinary bladder PEComa with MITF overexpression and without identifiable TSC1/TSC2 mutations or TFE3 rearrangement. A 50-year-old man underwent partial cystectomy for a bladder mass. Histologically, the tumor was well circumscribed and composed of mixed spindle and epithelioid cells with cytologic atypia and low mitotic activity. Immunohistochemistry reveal

Exosomal circ55033 promotes cigarette smoke-induced bladder cancer by modulating EMT and stem cell properties.

Bladder cancer has high morbidity and mortality, and cigarette smoke is a major risk factor for this disease. Exosomal circRNAs are widely involved in tumor progression, while their roles in smoke-related bladder cancer remain unclear. In the present study, we identified and validated circ55033, a highly expressed circRNA in clinical samples from smoking patients. Functional experiments demonstrated that cigarette smoke induced EMT and cancer stemness in bladder cancer cells via exosome-transported circ55033. Mechanistically, circ55033 acts as a molecular sponge for miR-1286 to exert its oncog

Urine-derived induced pluripotent stem cells for non-invasive diagnosis of bladder cancer.

Bladder cancer (BLCA) remains one of the most prevalent and costly malignancies worldwide, largely owing to its high recurrence rate and the invasive nature of conventional surveillance modalities. Induced pluripotent stem cell (iPSC) technology offers a new framework for disease modeling and, potentially, precision diagnostics. Urine provides a non-invasive, renewable source of exfoliated urothelial and renal epithelial cells amenable to direct reprogramming. Here, we review the biological rationale, reprogramming methodology, and diagnostic potential of urine-derived iPSCs (UDiPSCs) in the c

Piezo1 Activation by Yoda1 Inhibits Mechanosensitive ATP Release in Human Urothelial Cells and Bladder Overactivity in Rats with Bladder Outlet Obstruction.

Urothelial ATP release during bladder filling modulates afferent nerve activity and detrusor contractility to regulate the micturition reflex, yet the upstream mechanosensory mechanism linking distension to ATP release remains unclear. Here we investigated the role of Piezo1 in regulating mechanosensitive ATP release in human urothelial TRT-HU1 cells and in a rat model of bladder outlet obstruction (BOO). In TRT-HU1 cells, mechanical stress increased ATP release 60% above baseline. Bath-applied Yoda1 (1-10 µM) suppressed mechanically stimulated ATP release in a dose-dependent manner, an effect

Multidisciplinary Consensus on Improving the Care Pathway for Overactive Bladder from an Adherence Perspective: The claVHes Consensus.

Overactive bladder management within the Spanish healthcare system faces significant challenges, including suboptimal therapeutic adherence. This consensus proposes recommendations to improve the care process (both clinical and organizational) from an adherence perspective. A Multidisciplinary Working Group of 14 members critically reviewed and discussed the literature (individually and in joint sessions) and formulated recommendations around five pillars: (1) strategic planning, with a realistic medium-term horizon and progress toward common protocols; (2) technological innovation and digital

Peritoneal loose body: an incidental finding, a challenging diagnosis and multimodal assessment to avoid resections.

A man in his early 40s presented with urinary symptoms and was diagnosed with a right ureteric calculus, hydronephrosis and urinary tract infection. A non-contrast CT scan of the kidneys, ureters and bladder revealed an incidental pelvic mass in the rectovesical pouch, initially suspected as a gastrointestinal stromal tumour/fibroma. Multidisciplinary management prioritised cystoscopy-guided double-J stenting for obstruction and infection. In a second admission, ureteroscopic lithotripsy with stenting and a laparoscopic low anterior resection were planned simultaneously. Following ureteroscopi
