Repeated Transurethral Interventions Increase Elastin Degradation and Stricture Severity.

OBJECTIVE: To investigate the role of elastin in the progression of recurrent urethral strictures induced by repeated transurethral endoscopic interventions, using a preclinical animal model with translational relevance. METHODS: Urethral strictures were induced in middle-aged male New Zealand White rabbits (n = 6) via endoscopic electrocoagulation (50 W, 600 Hz, 10 seconds) at two sites located 4 cm and 5 cm from the urethral meatus. A subset of animals (n = 3) subsequently underwent repeated urethral dilation (UD) using Foley balloon inflation for 10 minutes. Ultrasound-guided imaging was p

Simultaneous surgery for NMIBC and BOO: evaluating surgical safety to oncological benefit.

BACKGROUND: Current evidence demonstrates that simultaneous surgical management of bladder cancer (BCa) and bladder outlet obstruction (BOO) is oncologically safe, despite well-known concerns. This systematic review and meta-analysis primarily evaluates oncological outcomes of concurrent transurethral resection of bladder tumor (TURBT) and BOO surgery compared to TURBT alone or staged treatments; perioperative safety is descriptively summarized. METHODS: A systematic search of PubMed, Web of Science, and the Cochrane Central Register of Controlled Trials was conducted from January 2000 to Mar

No urethral recurrences in complete responders after neoadjuvant chemotherapy and cystectomy for muscle-invasive bladder cancer.

OBJECTIVE: The Gold standard treatment for muscle-invasive bladder cancer (MIBC) in medically fit patients is neoadjuvant chemotherapy (NAC), followed by radical cystectomy (RC), usually urethra-sparing. Swedish guidelines recommend yearly urethroscopies for 5 years post-RC to detect urethral recurrences (URs). Evidence is limited regarding optimal urethral surveillance in the NAC-era for patients with MIBC. The goal was to evaluate UR-frequency across NAC-response groups: complete response (CR), i.e. pT0N0M0, and Non-CR-responses (partial response, stable disease, and progressive disease), co

Urethrovaginal Fistula Repair with Vaginal Reconstruction: A Complex Post-Traumatic Case.

INTRODUCTION AND HYPOTHESIS: Complex urethrovaginal fistula after childhood pelvic trauma is rare and may be compounded by severe vaginal stenosis, scarring, and difficult fistula localization. This video demonstrates a multimodal strategy for restoring vaginal access, identifying a urethral fistula near the bladder neck, and performing layered repair with vaginal reconstruction. METHODS: A 27-year-old woman presented with a 10-year history of cyclic hematuria after a childhood pelvic crush injury and previous urethral and vaginal repair. Examination revealed a blind-ending, stenotic vagina,

Multicentric evaluation of a lateral-based anterior vaginal wall flap for female urethral stricture repair.

BACKGROUND: Female urethral stricture (FUS) is an uncommon pathology. Success of the different treatments has not been compared and the published series have low sample size and short follow-up, so the evidence is not enough to support one technique over another. Treatment options include urethral dilatation and urethroplasty. Urethroplasty techniques include vaginal flap and graft urethroplasty. The objective is to evaluate the efficacy and safety of the lateral-based anterior vaginal wall flap for the treatment of FUS, and to describe our experience in etiology, diagnosis and treatment. Evaluation of lighted catheter phototherapy in endoscopic treatment of urethral strictures: a rabbit pilot study.

PURPOSE: To improve the success of minimally invasive urethral stricture treatments with red and near-infrared (NIR) light emitted via catheters after direct vision internal urethrotomy (DVIU). We hypothesized that catheter-emitted light therapy would improve success rate and histologic wound characteristics. METHODS: Twenty-four male New Zealand rabbits underwent stricture induction by electroresection and then were randomized into three groups: red light catheter (650 nm wavelength), NIR light (810 nm wavelength), or control (no light). After DVIU, a catheter was inserted for 7 days providi

Sequential thulium laser vaporization and catheter-guided 5-aminolevulinic acid photodynamic therapy for long-segment anterior urethral condyloma acuminatum: A case report.

Long-segment intraurethral condyloma acuminatum (CA) is difficult to treat because the urethral lumen limits visual access, instrument maneuverability, and uniform contact between a topical agent and the mucosa. Treatment must balance immediate removal of visible disease against cumulative mucosal injury: repeated or deep destructive procedures can cause bleeding, ulceration, pain, scarring/fibrosis, stenosis, and urethral stricture [1,2]. Electrocautery can rapidly destroy discrete accessible lesions but may produce thermal injury when applied deeply or circumferentially. Cryotherapy is usefu

Postvoided Residual/Bladder Emptying: A Mini Review.

Postvoid residual (PVR) urine volume is widely used in the evaluation of patients with lower urinary tract symptoms (LUTS) and benign prostatic obstruction (BPO), yet its clinical role remains debated. This mini review critically appraises current evidence on the definition, pathophysiology, measurement, and clinical utility of PVR. PVR reflects impaired bladder emptying and is influenced by multiple mechanisms, including bladder outlet obstruction, detrusor underactivity, and altered sensory function. However, its diagnostic accuracy for identifying specific conditions is limited by high inte

Caudal versus penile block and urethrocutaneous fistula formation following hypospadias repair in children <2 years: a multicenter randomized clinical trial.

BACKGROUND: Retrospective studies report conflicting associations between caudal block and urethrocutaneous fistula (UCF) formation after hypospadias repair, often without control for hypospadias severity. Our multicenter randomized, non-inferiority trial compared UCF incidence following caudal versus penile block for single-stage hypospadias surgery. METHODS: Children ≤2 years undergoing primary single-stage midshaft or distal hypospadias repair at 16 Pediatric Regional Anesthesia Network centers were randomized to caudal (group C) or penile block (group P) with general anesthesia. The prima

Combination therapy for multidrug-resistant Mycoplasma genitalium infections: a case series.

OBJECTIVES: In Belgium, approximately a quarter of Mycoplasma genitalium infections are resistant to both macrolides and fluoroquinolones-termed multidrug-resistant (MDR) infections. The optimal treatment approach for these MDR infections remains uncertain. Combination therapy has shown promise in treating other MDR pathogens by enhancing efficacy and reducing resistance development. We report the first five cases of MDR M. genitalium urethritis successfully treated with a novel combination therapy regimen consisting of minocycline, metronidazole, methenamine and pristinamycin ('M3P'). Treatment of bulbar urethral strictures with Optilume drug-coated balloons in a previously untreated population (FIRST-CARE): protocol for a single-blind multicentre randomised controlled trial.

INTRODUCTION: Minimally invasive endoscopic procedures constitute the cornerstone of first-line treatment for bulbar urethral strictures, although their long-term effectiveness is limited by high recurrence rates. The Optilume drug-coated balloon (DCB) is a novel intervention combining mechanical dilation with localised delivery of paclitaxel to reduce recurrence by inhibiting scar tissue formation. While its efficacy has been demonstrated in patients with recurrent strictures, its potential as a first-line option in treatment-naïve patients remains unexplored. The FIRST-CARE trial aims to ass

Treatment and outcomes of primary urethral cancer treated with radiation therapy.

BACKGROUND: Primary urethral cancer (PUC) is a heterogeneous and aggressive malignancy that accounts for less than 1% of all genitourinary malignancies. Due to its rarity, no prospective data exists to guide treatment paradigms. The purpose of this study is to report our clinical experience treating PUC with external beam radiation therapy (EBRT) with or without high-dose rate brachytherapy (HDR-BT) as part of curative-intent therapy. METHODS: Adult patients who had received treatment for PUC between January 2000 and December 2023 within a multi-center academic hospital system were queried in
