Signet-Ring Cell Carcinoma Masquerading as Crohn's Disease: A Diagnostic Challenge in Multifocal Colonic Strictures with Ascites.

Gastrointestinal malignancies such as signet-ring cell carcinoma can mimic inflammatory bowel disease, leading to diagnostic uncertainty. We present the case of a 39-year-old woman who presented with cramping abdominal pain, watery diarrhea, and unintentional weight loss. Imaging revealed multifocal colonic strictures and pelvic ascites, initially suspected to represent Crohn's disease. Bidirectional endoscopy showed gastric edema and colonic strictures without typical inflammatory features. Following laparoscopic-assisted subtotal colectomy for progressive obstruction, histopathology revealed

IL10 is a potent modulator of IL1-mediated inflammation in bovine endometrial explants.

CONTEXT: Bovine uterine inflammation impairs fertility, animal welfare and economics. Balanced regulation of interleukin-1 family proteins is crucial to resolve endometrial inflammation. AIM: mRNA expression of interleukin 1B (IL1B) and its receptor antagonist (IL1RA) was examined in bovine endometrial explants. METHODS: Endometrial explants from 26 cows were incubated with heat-inactivated pathogenic Escherichia coli, opportunistic Bacillus pumilus or commensal and potentially protective Lactobacillus buchneri (3, 14 and 24 h) and with cytokines (24 h; IL1B, IL17A, IL10). mRNA expression of

Gender Disparities in Procedural Management of Hunner Lesion Interstitial Cystitis.

INTRODUCTION: Interstitial cystitis/bladder pain syndrome (IC/BPS) is marked by chronic pelvic pain and urinary symptoms, affecting millions. Hunner-type IC (HIC), representing ~20% of cases, is characterized by distinct inflammatory pathology and worse symptom severity. Despite evidence of sex-based differences in IC/BPS, data specific to HIC remains sparse. This study examined sex differences in clinical presentation, management, and outcomes in HIC patients. METHODS: Single-center retrospective analysis of 114 patients with confirmed HIC managed between 2011 and 2024. We assessed sex-based

New perspectives in managing chronic pelvic pain in endometriosis: the role of GnRH antagonists versus dienogest.

INTRODUCTION: Endometriosis is a chronic, estrogen-dependent inflammatory disease affecting about 10% of reproductive-age women. Chronic pelvic pain is its most disabling symptom, and available therapies often show limited long-term efficacy and tolerability. This review examines novel pharmacological strategies to improve outcomes. AREAS COVERED: We compare the pharmacological, pharmacokinetic, and safety profiles of dienogest and oral GnRH antagonists (elagolix, relugolix, linzagolix). Mechanisms of action, levels of estrogen suppression, adverse events, particularly bone mineral density lo

Uterine inflammation did not compromise fertility in young primiparous Bos indicus cows enrolled in postpartum timed AI programs.

Although, previous study indicated that postpartum uterine inflammation was more pronounced in young primiparous (YP) Bos indicus cows, no study evaluated the impact of this inflammation on the fertility of these young cows included in timed artificial insemination (TAI) protocols. The objective of this study was to determine the effect of postpartum uterine inflammatory status on pregnancy per artificial insemination (P/AI) in YP Bos indicus beef cows subjected to TAI protocols. Postpartum Nelore Bos indicus YP cows (n = 151), 26.8 ± 1.8 months old were used in this study. On Day 0 of the Tim

Comparative characteristics of the microbiota of diverticula in complicated diverticulitis.

We present a comparison of the mucosal microbiota within different diverticula in a patient with diverticular disease (DD) complicated by diverticulitis and pelvic abscess. The conventional culture method and the 16S rRNA-based sequencing approach were employed to characterize the microbiota of perforated diverticulum (PD) and adjacent visually intact diverticulum (ID) from the same surgically resected colonic segment. Compared to PD, the microbiota of ID demonstrated depletion in butyrate-producing genera and increased abundances of Proteobacteria, Enterobacteriaceae, and Bacteroides. The pre

Nutrition in the prevention and treatment of pelvic inflammatory disease: a review.

Pelvic inflammatory disease (PID) is a common gynecological condition that causes inflammation in the upper reproductive tract. It affects millions of women globally each year and can lead to long-term complications such as infertility, ectopic pregnancy, and chronic pelvic pain. Although conventional PID treatments rely mainly on antibiotics, nutrition is crucial for preventing and treating PID because it can modulate immune responses, reduce inflammation, and promote tissue repair. This review aims to summarize the link between nutrition and PID by analyzing how dietary antioxidants such as

Uncontrolled Diabetes as the Sole Risk Factor in a Rare Candida glabrata Tubo-Ovarian Abscess.

Fungal tubo-ovarian abscesses (TOAs) are rare and often mimic bacterial TOAs clinically and radiographically, frequently resulting in delayed diagnosis. Uncontrolled type 2 diabetes mellitus (T2DM) significantly increases susceptibility to fungal infection through impaired neutrophil function, tissue ischemia, and enhanced Candida colonization. Early recognition is critical, as standard empiric TOA regimens do not provide antifungal coverage. We describe the case of a 36-year-old patient with poorly controlled T2DM (hemoglobin A1c 15.6%) who presented with fever and right-sided abdominal pain.

The Gut-Reproduction-Oral-Immune Axis: Microbial Dysbiosis as a Shared Driver in Polycystic Ovary Syndrome and Endometriosis.

Polycystic ovary syndrome (PCOS) and Endometriosis (EMs) are common reproductive endocrine disorders in women of reproductive age, characterized by the co-occurrence of infertility, chronic low-grade inflammation, and chronic pelvic pain. In this review, the “Gut-Reproduction-Oral-Immune Axis” (GROI axis) is introduced as a shared upstream pathophysiological mechanism for PCOS and EMs, with microbial dysbiosis playing a crucial role in its dysregulation. Despite distinct core pathologies, with PCOS characterized by metabolic/endocrine dysfunction and EMs by inflammatory/estrogen-dependent lesi

β-Lactam-Induced Fever Mimicking Post-caesarean Infection: A Diagnostic Dilemma.

Fever after a caesarean section is often believed to be caused by infections, which usually require an escalation of antimicrobial treatment. Although caesarean delivery can be lifesaving for both mother and baby, the procedure carries risks, with emergency caesarean sections being associated with a higher chance of complications, including infection and haemorrhage, compared to elective caesarean sections.  Drug-induced fever is an important yet often overlooked alternative diagnosis in obstetrics and can be difficult to recognise in patients receiving antibiotics for suspected postoperative

The dynamic landscape of alternative splicing during silicosis progression.

BACKGROUND: Silicosis is a severe occupational pulmonary fibrosis disease caused by silica dust inhalation, characterized by persistent inflammation and progressive fibrosis. While transcriptomic studies have identified numerous genes dysregulated, molecular mechanisms extend beyond mere changes in gene expression levels. Recent evidence suggests that post-transcriptional regulatory mechanisms, particularly alternative splicing (AS), may play crucial roles in fibrotic processes. However, AS dynamics and functional significance in silicosis remain largely unexplored. METHOD: To investigate AS i

Adenomyosis and chronic endometritis: cause, consequence or coincidence? A systematic review and meta-analysis.

Chronic endometritis is persistent inflammation of the endometrium, often underdiagnosed due to its oligosymptomatic presentation. Both chronic endometritis and adenomyosis share inflammatory and structural alterations of the endometrial-myometrial interface that may impair reproductive outcomes. This systematic review and meta-analysis were conducted in accordance with the PRISMA guidelines, and registered in PROSPERO. MEDLINE, Embase, CENTRAL, Cochrane Database of Systematic Reviews, Scopus and trial registries were searched from 2000 to May 2025 to determine the prevalence of chronic endome
