Comprehensive Imaging Review of the Ureter.

The ureter is an important conduit between the kidney and the bladder and is frequently imaged to rule out cancer in the upper genitourinary tract. However, while malignancy can occur, a spectrum of other pathologic ureteral findings can exist, and these can be divided into congenital, intrinsic, and extrinsic abnormalities that range from postoperative and neoplastic conditions to inflammatory disease. This article provides a comprehensive overview of the ureter, including the ureteral anatomy, ureteral imaging strategies, and various ureteral conditions and their associated imaging features.

Pathogenicity and virulence of NEISSERIA GONORRHOEAE.

Neisseria gonorrhoeae causes the sexually transmitted infection gonorrhea and is an obligate human pathogen, colonizing mucosal epithelia of the male urethra and the female endo/ectocervix, resulting in urethritis and cervicitis. These manifestations represent uncomplicated gonorrhea and highlight the gonococcus as a primarily mucosal pathogen. However, gonococci can ascend the female reproductive tract to cause pelvic inflammatory disease and may disseminate to distant anatomical sites. The gonococcus expresses a diverse array of virulence factors that mediate transmission, mucosal colonizati

Bacterial infections and their association with female infertility: diagnostic and therapeutic perspectives.

Infertility is a public health issue affecting millions of women of reproductive age worldwide. Bacterial infections caused by Chlamydia trachomatis, Neisseria gonorrhoeae, Mycoplasma hominis, and Ureaplasma urealyticum are among the most prevalent. They are associated with pelvic inflammatory disease, chronic endometritis, fallopian tube obstruction, impaired endometrial receptivity and implantation, and increased risk of ectopic pregnancy and pregnancy loss. Timely diagnosis using nucleic acid amplification testing represents the current standard, while advanced techniques, such as sequencin

Neutrophil extracellular traps in gynecological disease: pathogenic mechanisms and therapeutic opportunities.

Gynecologic disorders, including infections, sterile inflammatory diseases, endocrine abnormalities, and malignancies, share a common signature of dysregulated immunity within a uniquely hormone-responsive reproductive tract. Neutrophil extracellular traps (NETs) are increasingly recognized as central effectors at this interface of innate immunity, endocrine signaling, tissue remodeling, and thrombosis. In this review, we first outline the mechanistic basis of NET formation and emphasize how the cyclical anatomy, fluctuating sex hormones, and regional microbiota of the female reproductive trac

IgG Gal-ratio, a promising serum biomarker for detecting ovarian cancer in as early as stage I disease.

BACKGROUND: Early detection of epithelial ovarian cancer (EOC) is hampered by the absence of an ideal serum biomarker. This study aimed to further validate the utility of IgG Gal-ratio in pre-operative diagnosis of ovarian cancer. METHODS: Serum samples were collected from patients presenting with pelvic mass or elevated serum CA125 levels before surgery at the Obstetrics & Gynecology Hospital of Fudan University between June 2017 and December 2019. IgG was purified from these samples and incubated with PNGase F to release IgG N-glycans. Oligosaccharides from each sample were analyzed and quan

Cervical tuberculosis mimicking carcinoma of the cervix in a patient with long-standing secondary amenorrhea.

BACKGROUND: Tuberculosis remains a significant cause of extrapulmonary disease in endemic regions. Genital tuberculosis most commonly affects the fallopian tubes and endometrium, while cervical involvement is rare. Presentation as a cervical mass is particularly uncommon and may closely mimic cervical cancer, leading to diagnostic challenges. CASE: We report a case of a woman in her 40s with a history of long-standing secondary amenorrhea for 12 years who presented with postcoital bleeding. Clinical examination revealed a cervical growth. Histopathological evaluation of the cervical biopsy de

Isolated ovarian tuberculosis in a hemodialysis patient: An incidental pre-transplant discovery with diagnostic and management implications.

Female genital tuberculosis remains a rare extrapulmonary manifestation, particularly when presenting as isolated ovarian involvement mimicking malignancy. We report a case discovered incidentally during pre-transplant screening in an immunocompromised patient on hemodialysis. A 37-year-old woman with end-stage renal disease on hemodialysis for 5 years underwent routine pre-kidney transplant evaluation. Computed tomography revealed a multiloculated left adnexal mass (38 × 33 mm). She was asymptomatic without tuberculosis exposure history. Transvaginal ultrasound demonstrated a thin-septated av

A Case Report of Extrapulmonary Tuberculosis: A Potpourri of Imaging Manifestations.

Extrapulmonary tuberculosis (EPTB) can mimic pyogenic infection, inflammatory disease, and malignancy. We report the case of a middle-aged female patient who presented with low-grade fever, left hip pain, and left loin pain. Pelvic imaging demonstrated osteomyelitis with an abscess centered in the left femoral neck. An abscess was also present in the left obturator internus muscle. Further abdominal imaging demonstrated left hydronephrosis with a mid-ureteric stricture and retroperitoneal lymphadenopathy. Pus was aspirated and tested positive for Mycobacterium tuberculosis, establishing the di

Prevalence and risk factors for laparoscopic managed ectopic pregnancies at a tertiary referral hospital - a 10-year synopsis: a cross sectional study.

BACKGROUND: Ectopic pregnancies (EP) are a common pregnancy associated complication. They are associated with significant morbidity, and rarely mortality, if not managed properly. EP have a myriad of predisposing factors that help identify patients at risk and facilitate early diagnosis and intervention. We set out to conduct an audit of EP managed surgically at our hospital for a 10-year period and discern the incidence, prevalence and risk factors of their occurrence. METHODS: This study was designed as a Retrospective Clinical Audit based at the Aga Khan University Hospital (AKUH). Study p

Pelvic Inflammatory Disease With Presumptive Tubo-Ovarian Abscess Presenting With Rectal Spasm.

BACKGROUND: Pelvic inflammatory disease (PID) is an infectious process of the upper female genital tract, commonly caused by Chlamydia trachomatis, Neisseria gonorrhoeae, or Mycoplasma genitalium. Complications may include tubo-ovarian abscess (TOA) due to localized infection. Typical symptoms include pelvic pain, vaginal discharge, dyspareunia, and abnormal bleeding, with diagnosis often made clinically and supported by imaging and laboratory testing. Empiric antibiotic therapy is recommended promptly to prevent long-term sequelae. Rectal spasms, or proctalgia fugax, are characterized by flee

Examining the impact of the Chlamydia muridarum -induced synthesis of IFN-β during genital tract infection 1.

UNLABELLED: Chlamydia trachomatis infection of the female genital tract can result in severe reproductive sequelae, including pelvic inflammatory disease, tubal scarring, and infertility. Type I interferons have been implicated in both host defense and immunopathogenesis during chlamydial infection, with conflicting conclusions across experimental systems. However, the specific contributions of individual interferon subtypes remain poorly defined. Here, we examined the role of interferon beta (IFN-β) in regulating epithelial immune responses and intracellular bacterial development during Chlam

Pelvic Inflammatory Disease: A Narrative Review of Disparities Between Black and White Women in the United States and Strategies for Improvement.

Pelvic Inflammatory Disease (PID) is an inflammatory syndrome of the upper female reproductive organs caused by infection, most commonly via Neisseria gonorrhoeae or Chlamydia trachomatis, spreading to one or more reproductive organs. With PID responsible for many of the major morbidities of sexually transmitted infections (STIs), such as ectopic pregnancy and chronic pelvic pain, the urgency is high to mitigate the impacts of untreated PID, especially in higher-risk populations. This paper discusses the current disparities in PID diagnosis, treatment, and management between Black and white wo

Can we predict reproductive complications after Chlamydia trachomatis infection? A cohort study integrating demographic, behavioural, host genetic and infection-related factors.

OBJECTIVES: Chlamydia trachomatis (chlamydia) infection has a variable course; most infections resolve but some lead to reproductive complications. Behavioural, immunological and host genetic factors likely influence risk. We aimed to identify predictors of complications in persons with and without chlamydia and develop prediction models for risk stratification. METHODS: We analysed data from a long-term cohort of Dutch individuals registered as female in municipal registries and previously screened for chlamydia, including demographic, behavioural, chlamydia-infection and serology variables,
