Variant phenotypic expression of mosaic Turner syndrome with type II MRKH (MURCS association).

We present a rare case of an unmarried woman in her mid-twenties with primary amenorrhoea who was found to have the unusual coexistence of mosaic Turner syndrome (45, XO/46, XX) and Mayer-Rokitansky-Küster-Hauser syndrome with MURCS association (Müllerian agenesis, Renal anomalies, Cervicothoracic Somite dysplasia). The patient exhibited normal secondary sexual characteristics and was phenotypically female. The patient underwent successful laparoscopic Davydov vaginoplasty in anticipation of marriage. This case highlights the diagnostic complexity and management challenges associated with dual

Delayed diagnosis of juvenile hemochromatosis due to missed ferritin testing in a case of hypogonadotropic hypogonadism.

Juvenile hemochromatosis (JH) is a rare disorder caused by iron metabolism errors that lead to severe iron loading and organ failure in young adults before 30 years of age. We report a challenging case of a 26-year-old female whose diagnosis was delayed by nearly a decade due to deceptively nonspecific symptoms. She initially presented with oligomenorrhea at age 18 and was misdiagnosed with polycystic ovary syndrome (PCOS). Subsequent work-up for primary infertility revealed idiopathic hypogonadotropic hypogonadism (IHH). She progressively developed insulin-dependent diabetes mellitus and life

Induction of puberty in hypogonadal forms of primary amenorrhoea: current evidence and future perspectives.

Primary amenorrhoea is defined as the absence of spontaneous menarche by the age of 15 years with normal pubertal development or of 13 years in the absence of pubertal signs. The aim of this review was to provide a comprehensive overview of current evidence on puberty induction in girls with primary amenorrhoea, to highlight clinical practices, and to identify directions for future research. Puberty induction aims not only to achieve secondary sexual characteristics but also to ensure an appropriate growth spurt and attainment of final adult height, optimal peak bone mass achievement, uterine

The Prevalence of Abnormal Uterine Bleeding and Its Subtypes Among Adolescent Girls in India: A Systematic Review and Meta-Analysis.

This study aimed to determine the national pooled prevalence of abnormal uterine bleeding (AUB), its subtypes, and dysmenorrhea among adolescent girls in India. We conducted a systematic review and meta-analysis following the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, searching PubMed, Embase, and Web of Science from inception to December 4, 2024. Twenty nine observational studies involving adolescent girls aged 10-19 years, with sample sizes ranging from 127 to 2,000, conducted across India, were included. Due to high heterogeneity, pooled prevalen

MSH4 and MSH5 variants in premature ovarian insufficiency: A literature review and case study.

Premature ovarian insufficiency (POI) affects up to 4% of women under 40 years, with genetic factors implicated in a substantial proportion of cases. MSH4 and MSH5 encode components of the MutSγ complex, which stabilises Holliday junctions during meiotic recombination. This review examines the evidence for MSH4 and MSH5 involvement in POI. Biallelic MSH4 variants are well-established causes of POI and non-obstructive azoospermia (NOA), with MSH4 recognised as a high-evidence gene on clinical panels. MSH5 variants have similarly been reported in POI and NOA, though many lack functional validati

Performance of exome sequencing in premature ovarian insufficiency: a systematic review and meta-analysis.

BACKGROUND: Premature ovarian insufficiency (POI), one of the major and intractable causes of female infertility, affects 1.9-3.5% of the population. An increasing number of genes has found to be involved in the pathogenesis of POI. METHODS: Ovid MEDLINE, Embase, Cochrane Central Register of Controlled Trials, Web of Science, and CINAHL were searched from inception until May 2024. Studies were examined to assess the positive rate of monogenic pathogenic (P) and likely pathogenic (LP) variants, classified according to the American College of Medical Genetics and Genomics (ACMG) criteria, of ex

Long-term outcomes in ovotesticular DSD: insights from a single-centre Indian cohort.

OBJECTIVES: Ovotesticular disorders of sex development (OT-DSD) are rare disorders that constitute ∼5 % of all DSDs. Data on long-term outcomes with respect to gender identity, gonadal malignancy, pubertal/adulthood gonadal and sexual functions are scarce. METHODS: This retrospective study, reports the long-term outcomes of patients with OT-DSD from a single centre in western India. RESULTS: Fifteen patients (14: 46XX, 1: 46XX/46XY) with atypical genitalia and diagnosed as OT-DSD (unilateral-OT with contralateral ovary: 8, bilateral-OT: 3, lateral gonads: 4) were followed up for a median dur

Successful pregnancy after reconstructive surgery for a woman with complete cervical and vaginal agenesis - a case report and literature review.

Congenital cervical agenesis is a rare but important obstructive cause of primary amenorrhoea. Initial diagnosis is often delayed. With advances in reconstructive surgery, fertility restoration is now a realistic option for some women. We present a case of complete cervical and vaginal agenesis diagnosed in a 16-year-old girl who presented with cyclical pelvic pain and primary amenorrhoea. Following successful vaginoplasty and direct uterovaginal anastomosis, regular, pain-free menses, and later, uncomplicated sexual intercourse was achieved. Following a 10-year period of primary infertility a

All-in-one exome sequencing approach for genetic testing of unexplained premature ovarian insufficiency.

STUDY QUESTION: Is exome sequencing (ES) an efficient approach for simultaneous analysis of causative single gene defects and copy number variants (CNV) in unexplained premature ovarian insufficiency (POI)? SUMMARY ANSWER: Among 51 idiopathic POI cases, a conservative molecular diagnostic yield of 12% was achieved with equal contribution of pathogenic or likely pathogenic (P/LP) monogenic causes and pathogenic microdeletions. WHAT IS KNOWN ALREADY: POI is a clinically and genetically heterogeneous condition, yet most cases remain idiopathic. Although numerous monogenic causes and CNVs have b

Metastatic rosette-forming glioneuronal tumour presenting as isolated primary amenorrhoea.

Primary amenorrhoea is defined as the absence of menarche by 15 years of age in individuals with normal secondary sexual characteristics and is most commonly due to constitutional delay, gonadal dysgenesis or outflow tract abnormalities. We describe an adolescent female who presented with isolated primary amenorrhoea despite normal pubertal onset and progression and no neurological symptoms. Biochemical evaluation demonstrated a central hypothalamic-pituitary pattern of hypogonadism, prompting neuroimaging. MRI revealed a large intraventricular mass causing obstructive hydrocephalus with compr

Age-stratified hormonal profiles in adolescent and young adult PCOS/PMOS: higher mid-adolescent AMH largely unexplained by BMI.

Polycystic ovary syndrome (PCOS) - for which a global multistakeholder consensus has recently proposed the name polyendocrine metabolic ovarian syndrome (PMOS) - shows phenotypic variation across adolescence that is poorly characterised in Turkish populations, and because puberty raises both body mass index (BMI) and reproductive hormones in parallel, age and BMI effects are potentially confounded. Within a single-centre retrospective cohort of clinician-assigned PCOS/PMOS (January 2024-February 2026), we described how hormonal, anthropometric and clinical profiles differ across age and whethe

Primary functional hypothalamic amenorrhoea in the context of feeding and eating disorders or excessive exercise: Insights from non-athletic and elite athletic populations.

Primary amenorrhoea is defined as the absence of menarche by the expected age, with or without delayed pubertal development in adolescents. It may result from anatomical, genetic, endocrine, and functional causes. Among the latter, we propose the term primary functional hypothalamic amenorrhoea (P-FHA) to describe primary amenorrhoea characterised by hypothalamic-pituitary-ovarian axis suppression in the absence of organic disease. P-FHA is frequently associated with low energy availability (LEA), particularly in individuals with feeding and eating disorders (FEDs) or excessive exercise, often

Iron overload disorders in adults: a comprehensive review of gonadal function, reproductive, and sexual health.

BACKGROUND: Iron overload (IO) disorders, including thalassaemias, hereditary haemochromatosis, and transfusion-dependent anaemias, represent a growing clinical challenge with widespread systemic implications. Reproductive dysfunction remains severely underappreciated despite its high prevalence. Hormonal changes due to iron toxicity are frequently reported, yet are seldom the focus of reproductive medicine, causing fragmented knowledge, inconsistent clinical approaches, and a lack of consensus guidelines. OBJECTIVE AND RATIONALE: This review synthesizes evidence on the impact of IO on male a

Hyperprolactinaemia masked by the hook effect in a patient with a giant prolactinoma with growth hormone co-secretion.

We report a woman in her 50s with a giant co-secreting pituitary macroadenoma producing both prolactin and growth hormone. She presented with headache, proptosis, chemosis and secondary amenorrhoea since her 20s. Workup showed hyperprolactinaemia (251 μg/L; normal ≤23 μg/L). A brain MRI showed a giant mass at the base of the skull. Repeat prolactin assay after serum dilution showed marked hyperprolactinaemia (386 020 μg/L), consistent with a giant prolactin-secreting tumour. Additional tests established the presence of growth hormone excess. She was treated with cabergoline, leading to substan
