Development of a CRISPR/RspCas13d-based on-site rapid detection system for GII Norovirus.

Noroviruses (NoVs) are major cause of acute viral gastroenteritis and a serious public health concern. Current detection methods are limited in rapidity, equipment requirements, or sensitivity. In this study, we developed a rapid, sensitive, and specific detection assay for GII NoV by combining RT-RAA, T7 transcription, and the RspCas13d system. The RspCas13d protein was expressed and purified. RT-RAA primers and crRNA were designed against the conserved region of GII Nov. The assay was optimized and evaluated for specificity, sensitivity, and clinical performance. Results showed that the RT-R

Impact of the BioFire® FilmArray® Gastrointestinal Panel on clinical management of adult acute gastroenteritis: a retrospective analysis from a Saudi tertiary-care setting.

BACKGROUND: Acute gastroenteritis (AGE), a leading cause of worldwide morbidity, can be triggered by a wide array of pathogens. Syndromic molecular platforms, such as the BioFire® FilmArray® Gastrointestinal Panel (GIP), allow rapid and comprehensive detection of enteric pathogens; however, evidence on their utility from Saudi Arabia is lacking. This study therefore aimed to assess the prevalence of enteric pathogens, determine the detection yield of the FilmArray GIP, and explore its potential impact on clinical management. METHODS: We conducted a retrospective cohort study at Dr. Soliman Fa

High levels of co-detection of norovirus and other enteric pathogens in patients with and without acute gastroenteritis in a hospital-based surveillance study in Bangladesh, 2018-2021.

OBJECTIVES: The contribution of various enteropathogens to the occurrence of acute gastroenteritis (AGE) remains uncertain in highly endemic settings. We describe the frequency of norovirus-only detections and norovirus co-detections in hospitalized patients in Bangladesh and compare their clinical severity and viral load. METHODS: From March 2018 to October 2021, 1250 AGE cases and 1250 non-AGE controls of all ages were enrolled at 10 tertiary care hospitals in Bangladesh. All norovirus-positive AGE cases (n = 111) and non-AGE controls (n = 182), and a randomly selected subset of 126 norovir

Emergence of norovirus GII.7 and predominance of recombinant strains in pediatric acute gastroenteritis in Japan, 2021-2024.

Noroviruses (NoVs) are a leading cause of acute gastroenteritis (AGE) worldwide, with genetic diversity driven by recombination and genotype replacement. This study investigated the prevalence and molecular characteristics of NoVs among Japanese children with AGE from July 2021 to June 2024. A total of 1,095 stool samples were collected from three prefectures (Kyoto, Osaka, and Shizuoka), and NoV RNA was detected in 456 cases (41.6%). Genogroup GII predominated (96.9%), while GI infections were rare. Genotyping identified eight genotypes, including two GI (GI.1, GI.6) and six GII (GII.2, GII.3

Large-scale gastroenteritis outbreaks occurring in a university town of Guangzhou city during the global wave of the norovirus in China.

Noroviruses are the primary viral pathogens responsible for epidemic acute gastroenteritis. The novel GII.4 Sydney 2012 variant, which emerged during the 2012/13 winter season, led to significant global outbreaks. This study reports large-scale outbreaks occurring in the university town of Guangzhou, China, from January 8 to April 3, 2013. These outbreaks affected 652 individuals across five universities and one middle school. Of the 386 samples tested, 114 were norovirus-positive, and 19 stool or swab samples were genotyped as the GII.4 Sydney 2012 strain. The genome sequence of the GII.4 Syd

Epidemiology and Clinical Impact of Pediatric Viral Gastroenteritis Before and After Rotavirus Vaccination in Sicily.

Acute gastroenteritis (AGE) remains a leading cause of pediatric mortality and morbidity, with rotavirus as the leading cause of severe disease. Post-vaccine surveillance is essential to monitor circulating pathogens and assess vaccination impact. Sicily was the first Italian region to implement universal rotavirus vaccination in 2012. We retrospectively studied 693 children hospitalized for suspected viral AGE at the Children's Hospital of Palermo (March 2017-February 2020), testing stool samples for viral and bacterial enteric pathogens. Rotavirus remained the most common agent (13.3%), foll

Norovirus wastewater monitoring during a community outbreak investigation in Pennsylvania.

Norovirus is a leading cause of acute gastrointestinal illness (AGI). Norovirus is not notifiable in the United States; therefore, health departments typically only receive reports of norovirus outbreaks. While wastewater surveillance has been demonstrated to be useful for tracking community norovirus case reports, its value during outbreak investigations is less understood. This study evaluates the impact of a single norovirus outbreak on wastewater measurements in a community. Norovirus genogroup 2 (GII) concentrations were measured at two wastewater treatment facilities (WWTFs) near a resta

Integrating wastewater surveillance of norovirus epidemics with recreational water risk assessment for enhanced urban swimming safety.

Noroviruses are the leading cause of hand-borne viral gastroenteritis worldwide, but recreational water exposure is an important transmission route to consider. This study integrates wastewater-based epidemiology with quantitative microbial risk assessment to enhance urban contamination knowledge and swimming safety in the Greater Paris region. Raw wastewater from 8 wastewater treatment plants and up to 16 Paris sewerage network sites were sampled from November 2021 to March 2025, totaling over 3000 composite samples. Surface waters from the Seine and Marne rivers, crossing the region, were co

Waterborne norovirus outbreaks in China, 2000-2022: a systematic review.

In China, waterborne transmission is a relatively rare route for norovirus outbreaks; however, once it occurs, the outbreaks tend to be large in scale, prolonged in duration, and difficult to control. This systematic review characterizes the epidemiological features of such outbreaks in China. We searched the WANFANG, CNKI, PubMed, and Web of Science databases for literature on waterborne norovirus outbreaks published up to November 2023. From 2000 to 2022, a total of 112 outbreaks were reported in China among the 97 articles included. These outbreaks involved approximately 19,796 cases and 56

A 3-year wastewater-based epidemiology study of noroviruses in Senegal.

UNLABELLED: Norovirus is a leading cause of acute gastroenteritis worldwide due to its high contagiousness, widespread transmission, and the lack of treatment or vaccine. In Senegal, no routine community-level monitoring is performed, and most available data concern other enteric viruses, leaving a gap in understanding norovirus circulation. To assess the potential of wastewater surveillance as an early warning system, we analyzed the spatiotemporal circulation in Senegal of noroviruses using clinical and environmental data collected between 2020 and 2023, combining statistical and machine lea

Health and economic impact of introducing norovirus vaccination in England accounting for acute kidney injury: Model-based cost-effectiveness analysis.

BACKGROUND: Norovirus vaccines are currently undergoing advanced clinical trials. Acute kidney injury (AKI) is a serious sequelae of norovirus infection. This study evaluated the health and economic impact of implementing a norovirus vaccination programme in England, and assessed the contribution of AKI. METHODS: We constructed a deterministic age-stratified dynamic-transmission compartmental model. Three single-dose norovirus vaccination strategies were compared to a no-vaccination strategy: targeting children under 5 years of age, targeting adults aged 65 years or older, and targeting both

Drivers of the paradigm shift in norovirus diagnostics: technological innovation, contextual demands, and collaborative synergy.

This paper systematically reviews paradigm shifts in diagnostic technologies for norovirus. Given the substantial burden of acute gastroenteritis caused by this virus and the current lack of licensed antiviral therapies, early and accurate diagnosis is of paramount importance. Diagnostic technologies have evolved from electron microscopy and immunological assays to molecular detection methods, with reverse transcription quantitative polymerase chain reaction (RT-qPCR) currently recognized as the gold standard for molecular diagnostics. However, RT-qPCR is constrained by complex operational pro
