Diagnostic Accuracy of Endometrial Aspiration Cytology Compared With Histopathology in Abnormal Uterine Bleeding: A Cross-Sectional Study at a Tertiary Care Hospital.

Introduction Abnormal uterine bleeding (AUB) is a frequent indication for endometrial evaluation. Histopathological examination remains the diagnostic reference standard, but tissue sampling by curettage or biopsy may be invasive and resource-intensive. Endometrial aspiration cytology (EAC) offers a rapid, minimally invasive alternative that can provide an early impression of endometrial pathology. This study assessed the diagnostic performance of EAC against histopathology in women presenting with AUB. Materials and methods This prospective cross-sectional study was conducted from January 202

Successful treatment of recurrent epithelioid trophoblastic tumor (ETT) with pembrolizumab after pembrolizumab: A case report and review of literature in treatment of ETT.

BACKGROUND: Epithelioid Trophoblastic Tumors (ETT) are rare, which makes an optimal treatment regimen difficult to determine. Since PD-L1 is expressed in placental tissue and in gestational trophoblastic neoplasia (GTN), immunotherapies such as pembrolizumab have been proposed as possible treatments. CASE PRESENTATION: The case presented is a 50-year-old postmenopausal female who presented with postmenopausal bleeding and a large, fungating cervical mass. Cervical biopsies confirmed ETT. The patient underwent primary debulking surgery with radical abdominal hysterectomy, bilateral salpingo-oo

Barriers to the recognition and evaluation of abnormal uterine bleeding among individuals at increased risk for endometrial Cancer.

OBJECTIVE: To evaluate knowledge of abnormal uterine bleeding (AUB) and postmenopausal bleeding (PMB) as symptoms of endometrial cancer (EC), assess understanding of diagnostic evaluation, and identify barriers and facilitators to care. METHODS: We conducted a web-based survey of participants (N = 603) with a history of AUB/PMB and increased risk for EC. The survey, informed by semi-structured interviews, assessed symptom recognition, knowledge of EC, familiarity with diagnostic evaluation, and healthcare-seeking behaviors. Descriptive statistics, chi-square tests, and multivariable logistic

Risk factors and predictive modeling for occult endometrial cancer in women with atypical hyperplasia: a retrospective study.

PURPOSE: Atypical endometrial hyperplasia (AEH) is a known precursor to endometrioid endometrial carcinoma. However, occult carcinoma may already be present at diagnosis, complicating surgical planning. Accurate preoperative risk stratification is crucial, especially for guiding the selective use of sentinel lymph node biopsy. This study aimed to identify predictors of occult carcinoma and develop a model to estimate the risk of malignancy. METHODS: We conducted a retrospective case-control study of 101 women diagnosed with AEH who underwent hysterectomy between 2010 and 2024 at Galilee Medic

Menstrual disturbances in women with advanced heart failure and heart transplant recipients.

BACKGROUND: Menstrual disturbances (MDs) are common among women with advanced heart failure. These disorders can arise from hormonal, hemodynamic, pharmacological, and psychological factors. Heart transplantation (HT) involves changes in the cardiovascular system and requires adjustments in medical treatment. However, the prevalence and impact of MDs before and after HT have not been thoroughly evaluated. METHODS: We conducted a single-center retrospective observational study that included all female heart transplant recipients followed from 1999 to 2020. A questionnaire was used to assess th

Association Between Menstrual Pain and Procedural Pain During Office Hysteroscopy: A 10-Year Cohort Study.

BACKGROUND: Office hysteroscopy (OH) is the standard diagnostic method for evaluating intrauterine and endocervical pathology, yet the fear of procedural pain remains a major barrier to patient acceptance. This study examined whether menstrual pain can be used as a clinical reference to anticipate OH procedural pain and explored other predictors of procedural discomfort. METHODS: A historical cohort study was conducted among women undergoing OH for abnormal uterine bleeding or postmenopausal bleeding at a tertiary referral hospital between 2014 and 2024. All procedures were performed by a sin

"Identification of Malignancy Risk Factors in Endometrial Pathologies: The Role of Clinical, Laboratory Parameters, and Peripheral Blood Inflammatory Indices".

OBJECTIVE: Postmenopausal bleeding (PMB) and increased endometrial thickness are key clinical indicators that may suggest underlying malignancies. While endometrial biopsy remains the diagnostic gold standard, its invasiveness underscores the need for alternative, noninvasive biomarkers. This study evaluates the potential of clinical, laboratory, and peripheral blood inflammatory indices (PBII) in distinguishing malignant from benign endometrial pathologies. METHODS: This retrospective study included 162 patients who underwent endometrial biopsy due to PMB and/or increased endometrial thickne

Endometrial Polyps.

Endometrial and endocervical polyps are common intrauterine lesions characterized by focal overgrowth of glands and stroma. Prevalence increases with age, obesity, ovulatory disorders, and tamoxifen use. The lesions may be asymptomatic or linked to infertility, abnormal uterine bleeding (AUB), or cancer, a rare diagnosis in the early reproductive years. Polyps frequently cause postmenopausal bleeding; before menopause, the typical presenting AUB symptom (AUB-polyp) is intermenstrual bleeding. The role that polyps play in infertility or in reducing the success of assisted reproductive technique

A 60-Year-Old Woman With Endometrial Intraepithelial Neoplasia: A Case of Persistent Postmenopausal Bleeding.

Endometrial intraepithelial neoplasia (EIN) is a premalignant lesion of the endometrium that may progress to endometrioid adenocarcinoma. This case is unique because EIN occurred in a 60-year-old woman who was 10 years postmenopausal, lacked risk factors, and had symptoms despite prior treatment for a presumed benign endometrial polyp. The patient presented with several months of pelvic pain and postmenopausal spotting. Laboratory studies were largely normal. Transvaginal ultrasound revealed a slightly thickened 7-mm endometrium and multiple intramural fibroids, suggesting benign pathology.  D

Histological signatures of hormone replacement therapy in the endometrium.

BACKGROUND: Hormone replacement therapy (HRT) is widely prescribed for managing menopausal symptoms, yet its effects on the endometrium remain clinically important. While large-scale studies have established that estrogen-only HRT increases endometrial cancer risk and continuous combined regimens reduce this risk, real-world data on endometrial histology remain valuable for understanding contemporary practice patterns. METHODS: We conducted a retrospective analysis of women who underwent hysteroscopy with endometrial sampling at Princess Royal University Hospital. Of the 62 patients, we inclu

Coincidental occurrence of endometrial hyperplasia with atypia and vaginal leech infestation in a postmenopausal woman: a rare case report.

BACKGROUND: Postmenopausal bleeding is a common gynecological complaint with multiple etiologies, ranging from benign to malignant conditions. Leech infestation of the female genital tract is a rare cause, particularly in rural regions with exposure to natural pooled water sources. CLINICAL PRESENTATION: We report a 50-year-old postmenopausal woman who presented with a 1-week history of vaginal bleeding. Ultrasound evaluation revealed a thickened endometrium (20 mm), and histopathology confirmed endometrial hyperplasia with atypia. She underwent total hysterectomy with bilateral salpingo-ooph

A Rare Case of Uterine Embryonal Rhabdomyosarcoma in a Postmenopausal Woman.

This report describes a rare presentation of embryonal rhabdomyosarcoma (ERMS) in a 76-year-old postmenopausal woman who presented with several months of unreported vaginal bleeding followed by obstructive urinary symptoms. Imaging revealed a large uterine mass, and biopsy with immunohistochemical staining confirmed ERMS characterized by high-grade histopathologic features. Given her advanced age, comorbidities, poor functional status, and extensive tumor burden, the patient opted for palliative care focused on comfort and quality of life. This case highlights the diagnostic challenges, aggres

Female-specific risk factors for lung cancer: insights from the All of Us Research Program.

INTRODUCTION: Lung cancer is the leading cause of cancer deaths in females, with slower declines in incidence compared to males. While smoking is the leading risk factor, it doesn’t fully explain the increasing lung cancer burden in females. Reproductive factors, metabolic disorders, family history, and their interactions may also influence risk; however, these remain poorly understood, particularly across diverse groups. To explore this, we conducted a case-control study using All of Us Research Program data to examine these factors and their association with lung cancer risk in adult females

Incidence of uterine cancer after supracervical hysterectomy.

BACKGROUND: Supracervical hysterectomy (SCH), commonly performed during sacrocolpopexy, involves removal of the uterine corpus, leaving the cervix in-situ. Significant anatomic variation exists in remnant tissue, which may develop into a uterine malignancy. No prior studies have sought to describe the future risk of developing de novo uterine cancer after SCH with benign pathology. This study aims to describe the incidence rate of uterine cancer following SCH performed for benign indication. METHODS: Retrospective cohort study of patients who underwent SCH between June 2000 and December 2022

A rare incidental finding of an occult ovarian Sertoli-Leydig cell tumor presenting with hyperestrogenism in a postmenopausal woman: Diagnostic challenges.

OBJECTIVE: Ovarian Sertoli-Leydig cell tumor (SLCT) is an exceptionally rare subtype of ovarian sex cord-stromal tumor, comprising less than 0.5% of ovarian neoplasms. While SLCT typically manifests in reproductive-age women, occurring in fewer than 10% of cases either prior to menarche or postmenopausally, it often presents with androgen secretion, resulting in virilization. However, occult lesions associated with hyperestrogenism in postmenopausal women are infrequent and may be easily overlooked. CASE REPORT: We present a rare case of SLCT in a 60-year-old postmenopausal woman who experien
