INTRODUCTION: Endometrial cancer is the most common gynecological malignancy in high-income countries. Although postmenopausal bleeding is its cardinal presenting symptom, only 5-10% of women with postmenopausal bleeding have malignancy. Postmenopausal bleeding (PMB) raises significant concern in women, as it may signal early uterine malignancy. Consequently, all episodes of PMB require thorough investigation. Approximately 60-80% of cases are linked to atrophic endometrium, with about 10% each attributable to endometrial hyperplasia and carcinoma. However, one case involved a woman with PMB, where no definitive cause was identified beyond a forgotten intrauterine contraceptive device. Further evaluation revealed a rare uterine infection as the underlying cause of the PMB.
