ERS/ESRS clinical practice guideline for continuous positive airway pressure for the treatment of adult obstructive sleep apnoea.

BACKGROUND: Continuous positive airway pressure (CPAP) is the primary treatment for obstructive sleep apnoea (OSA), although its effectiveness may vary across OSA subtypes. This joint European Respiratory Society (ERS)/European Sleep Research Society (ESRS) clinical practice guideline evaluates CPAP's impact on patient-centred and prognostically relevant outcomes, comparing it to no or sham treatment in different OSA subgroups. METHODS: The joint ERS/ESRS Task Force consisted of experts in pulmonology, cardiology and respiratory sleep medicine, patient representatives and methodologists. Ques

Cerebral cavernous malformations: epidemiology, molecular pathogenesis, diagnosis and therapy.

Cerebral cavernous malformations (CCMs) are cerebrovascular diseases characterized by vascular lesions of the central nervous system (CNS) that can result in epileptic seizures, headaches, focal neurologic deficits (FNDs), and hemorrhagic stroke. Germline and somatic loss-of-function (LOF) mutations in one of the three established CCM-associated genes, namely CCM1 (or krev interaction trapped protein 1, KRIT1), CCM2, and CCM3 (or programmed cell death 10, PDCD10), which have been validated as core drivers of CCM pathogenesis and progression. Over the past few decades, a substantially improved

Baseline circulating immunophenotype as a predictor of immune-related adverse events in NSCLC patients treated with immune checkpoint inhibitors.

BACKGROUND: Immune-related adverse events (irAEs) represent a challenge in non-small cell lung cancer (NSCLC) patients receiving immune checkpoint inhibitors (IO). Baseline circulating immunophenotype may identify patients at increased irAEs risk. METHODS: This observational study included patients with advanced NSCLC receiving IO alone or with chemotherapy. Baseline clinicopathological data and blood samples were prospectively analyzed to investigate the association of clinicopathological characteristics, circulating immune cells and cytokines, with irAEs onset, severity, timing, and type.

Development of the eye in grass carp (Ctenopharyngodon idella): Ocular histomorphology, corneal surface ultrastructure, and retinal calretinin and tyrosine hydroxylase immunoreactivity.

Eye posthatching development was examined in grass carp (Ctenopharyngodon idella) from 1 day posthatching (1 dph) to 365 dph. Eye histology and morphometry, retinal expression of calretinin (CR) and tyrosine hydroxylase (TH), and corneal ultrastructure were studied using light and scanning electron microscopy. Basic eye structure was detectable at 1 dph, though important features, including retina and iris pigmentation as well as photoreceptor differentiation, were delayed till 4 dph. Differentiated photoreceptors seemed to emerge at 4 dph with evident increases in their count and shape divers

Unequal survival after ischaemic heart disease in Eastern Europe: a sex-specific GBD 2023 analysis.

AIMS: Sex disparities in ischaemic heart disease (IHD) outcomes are well documented in Western Europe, but remain poorly characterized across Eastern Europe, a region marked by substantial mortality inequalities. We aimed to determine whether population-level case fatality among individuals living with IHD differed by sex in Eastern Europe and to identify modifiable risk factors associated with disparities. METHODS AND RESULTS: We analysed sex-specific estimates from the Global Burden of Disease 2023 study for seven Eastern European countries. Age-standardized IHD mortality and prevalence rat

Oxidative Stress in Neonatal Disease: Established Organ Injury and Emerging Evidence for Skeletal Involvement.

Reactive oxygen and nitrogen species participate in physiological signaling and adaptation during fetal and neonatal development, whereas excessive, prolonged, or spatially dysregulated production may contribute to oxidative and nitrosative tissue injury. Redox dysregulation has been extensively investigated in neonatal conditions, including bronchopulmonary dysplasia (BPD), hypoxic-ischemic encephalopathy, retinopathy of prematurity (ROP), necrotizing enterocolitis (NEC), congenital heart disease (CHD), and hyperbilirubinemia. Its relationship with bone metabolism is well established in adult

The impact of semaglutide initiated six months after laparoscopic sleeve gastrectomy on long-term weight loss and metabolic outcomes: single-center preliminary study.

BACKGROUND: Although laparoscopic sleeve gastrectomy (LSG) is an effective treatment for severe obesity, some patients experience suboptimal weight loss or an early weight-loss plateau during postoperative follow-up. Semaglutide has emerged as an effective pharmacological option for obesity management; however, evidence regarding its use after bariatric surgery remains limited. This study evaluated the impact of adjunctive semaglutide therapy initiated six months after LSG on long-term weight loss and metabolic outcomes. MATERIALS AND METHODS: This retrospective single-center study included 8

A spatiotemporal atlas of radiation-induced brain injury reveals dysfunction of the blood-CSF barrier.

Radiotherapy for brain tumors frequently induces irreversible cognitive decline, yet the underlying mechanisms remain poorly understood. Here, we present an integrated single-cell and spatial transcriptomic atlas of radiation-induced brain injury (RIBI) in mice, spanning the acute to chronic phases. We reveal that the early-delayed phase following irradiation represents a critical window during which the blood-cerebrospinal fluid barrier (BCSFB) integrity is compromised. This disruption results from the selective depletion of a radiosensitive Tmsb4x+ epithelial subpopulation in the choroid ple

Affinity and costimulatory architecture-associated toxicity in mesothelin-targeted TCR-mimetic CAR-T cells.

INTRODUCTION: TCR-mimetic CAR-T combines antibody-based targeting with physiological TCR signaling, yet suboptimal co-stimulation and safety risks remain key barriers for solid tumor therapy. The prevailing assumption that stronger co-stimulation yields superior efficacy has not been systematically tested in TCR-mimetic CAR-T architectures. METHODS: We developed a Mesothelin-targeted scFv-CD3ϵ TCR-mimetic CAR-T platform and systematically compared co-stimulatory modules-4-1BB, BRM-truncated 4-1BBΔBRM, and the 80BB chimera-alongside CD3ζ overexpression. To define the requirements for toxicity,

Endovascular magnetic filters for removing excess chemotherapeutic drugs from blood flow using dimensionless scaling laws.

Intra-arterial chemotherapy (IAC) for hepatocellular carcinoma (HCC) is limited by the escape of chemotherapeutic agents into systemic circulation, causing severe side effects such as heart failure. This study aims to develop and optimize endovascular magnetic filtration devices to enhance IAC efficacy, by targeted removal of excess drug-coated ultrasmall superparamagnetic iron oxide (USPIO) particles from circulation. Device performance was evaluated through multi-physics simulations that coupled the transport of drug-coated magnetic particles in blood flow with magnetic interactions and magn

Monitoring fatty acid trafficking during Drosophila melanogaster oogenesis reveals a role for the triglyceride synthase DGAT1 in protecting mitochondrial integrity.

UNLABELLED: Successful oogenesis requires the precise coordination of nutrient uptake, storage, and utilization to meet the high metabolic demands of egg production. In mammals, fatty acid (FA) metabolism has emerged as a key driver of oocyte maturation; however, the mechanisms by which follicles regulate FA trafficking and utilization remain poorly understood across all systems. To address these issues, we leverage the genetic tractability of Drosophila melanogaster oogenesis. We found that nurse cell mitochondria are metabolically active and catabolize FA in a stage-dependent manner, with fa

Treg dysregulation in the oxidative lung milieu: an emerging mechanistic link in interstitial lung disease associated with systemic autoimmune rheumatic diseases.

Systemic autoimmune rheumatic disease-associated interstitial lung disease (SARD-ILD) is a fatal complication, in which the progression of pulmonary fibrosis has been closely linked to immune microenvironment imbalance and oxidative stress. Regulatory T cells (Tregs), as a central component of immune tolerance, play a crucial role in suppressing excessive inflammation. Current evidence suggests that oxidative stress may contribute to Treg dysregulation through several interrelated mechanisms, including redox-sensitive regulation of Foxp3 expression and stability, disruption of IL-2 signaling,

H2A.J is an epigenetic driver of IL-17-associated inflammation in TH17 cells and keratinocytes.

Psoriasis is a common autoinflammatory disease driven by excessive IL-17 expression. While the pathomechanism of the disease has been revealed, epigenetic factors contributing to psoriasis are less well understood. Here, we investigated the role of the poorly studied histone variant H2A.J in psoriasis. We found that H2A.J gets overexpressed in the skin of psoriasis patients, with even non-lesional skin showing increased expression compared to the skin of healthy donors. To gain mechanistic insights, we studied mice with a global H2A.J knockout (KO) and found them to be partially protected agai

Aldosterone, the brain, and the blood pressure ceiling: a neurological case for aldosterone synthase inhibitors in resistant hypertension.

Hypertension is the leading modifiable risk factor for stroke and a major contributor to cognitive impairment and dementia, with its global burden increasing alongside population aging. Hypertension promotes cerebrovascular damage through endothelial dysfunction, blood-brain barrier disruption, small vessel disease, and accelerated atherosclerosis, all potentially contributing to both ischemic and hemorrhagic stroke risk. These vascular alterations also contribute to neurodegenerative processes, linking hypertension-particularly when present in midlife-to vascular dementia and Alzheimer's dise

From low renin to the primary aldosteronism spectrum: aldosterone synthase inhibitors and the endocrine management of hypertension.

BACKGROUND: Hypertension remains a major contributor to global cardiovascular morbidity despite decades of available therapy. AIM: This review advances an endocrinological framework in which chronic hypertension, including resistant hypertension, is understood as a consequence of dysregulated hormonal control of renal sodium handling, mediated by excess renin-angiotensin II activity, inappropriate aldosterone excess, or both. RESULTS: Within this framework, the traditional binary distinction between "essential" and "secondary" hypertension gives way to a mechanistic continuum characterized b

Aldosterone synthase inhibitors: biological rationale, mechanism of action, and results of clinical trials.

Resistant and uncontrolled hypertension remain major unmet needs, with aldosterone dysregulation or frank excess increasingly recognized as a central driver. To this end, aldosterone synthase inhibitors (ASIs) represent a novel therapeutic approach targeting CYP11B2, the key enzyme responsible for aldosterone biosynthesis. By reducing aldosterone production at its source, ASIs may overcome limitations associated with conventional renin-angiotensin-aldosterone system (RAAS) blockade and attenuate both genomic and non-genomic effects of aldosterone. Advances in drug development have led to secon
