Mendelian randomization study: investigating the causal impact of Covid-19 on adverse pregnancy outcomes.

BACKGROUND: Due to limitations in observational studies, the link between COVID-19 and adverse pregnancy outcomes (APOs) remains inconclusive. This study uses two-sample Mendelian randomization (MR) analyses to assess COVID-19's causal effects on APO traits. METHODS: We applied inverse variance weighting (IVW), MR-Egger, weighted median, weighted mode, and simple mode to thoroughly evaluate the effects of COVID-19 infection, hospitalization, and critical status on eight APO traits. RESULTS: Our findings indicate that COVID-19 infection is associated with a decreased risk of eclampsia (OR: 0.

Clinical profile and outcome of primary membranous nephropathy: A tertiary care center experience from North India.

BACKGROUND: Primary membranous nephropathy (MN) is a major cause of nephrotic syndrome in adults, characterized by immune complex formation on the outer side of the glomerular basement membrane. Primary MN accounts for 80% of the cases, and 20% cases are associated with a secondary etiology. While immunosuppressive therapy has improved patient outcomes in MN, the clinical profile and treatment outcomes of MN in our patient population have not been previously studied. AIM: To investigate the clinicopathological profile and outcomes of primary MN patients. We included consenting patients aged ≥

Immune profiling-informed immunomodulation associated with gestational extension in early-onset preeclampsia with monochorionic twins: a case report.

BACKGROUND: Early-onset preeclampsia (EOPE) is associated with placental malperfusion and systemic endothelial dysfunction. Maternal immune dysregulation has been implicated in EOPE, but immune findings obtained during severe disease are descriptive and do not define a validated EOPE subtype. CASE PRESENTATION: A 22-year-old woman (G2P0) with a monochorionic diamniotic twin pregnancy developed gestational hypertension at 25+5 weeks and met criteria for EOPE at 30-31 weeks. Because of refractory progression under standard management, immune testing obtained at 31+1 weeks showed elevated TNF-α,

Limited Predictive Value of Inflammatory and Renal Markers in the Progression of Isolated Gestational Proteinuria to Preeclampsia: A Retrospective Cohort Study.

Objective: Isolated gestational proteinuria (IGP) has traditionally been considered a benign condition; however, emerging evidence suggests that it may represent an early stage in the spectrum of preeclampsia. This study aimed to evaluate clinical and laboratory predictors of progression from IGP to preeclampsia. Methods: This retrospective cohort study included pregnant women diagnosed with proteinuria ≥ 300 mg/day after 20 weeks of gestation between January 2023 and December 2024. After applying predefined exclusion criteria, 319 women with isolated gestational proteinuria (IGP) were include

Endothelin Receptor Antagonists in Resistant Hypertension and Proteinuric Kidney Disease: Receptor Strategy and Volume Management.

This narrative review examines how endothelin receptor antagonists (ERAs) can be used at the intersection of difficult-to-control hypertension, chronic kidney disease (CKD), and albuminuria or proteinuria. For clinicians, the central question is not receptor selectivity alone, but whether a specific agent, indication, dose, background therapy, and monitoring plan can deliver meaningful blood-pressure or kidney benefit without unacceptable fluid retention. Pulmonary arterial hypertension established the pharmacology and major safety liabilities of the class; the current implementation questions

[Once upon a time in nephrology - Episode 3: A brief history of preeclampsia].

Preeclampsia is a maternal syndrome of placental origin that complicates 2–5% of pregnancies. It remains one of the leading causes of maternal mortality worldwide and is a major contributor to fetal and neonatal morbidity and mortality. It is classically defined, after 20 weeks of gestation, by the combination of arterial hypertension and proteinuria ≥ 0.3 g/24 h. In its severe form, it may induce endothelial injury consistent with thrombotic microangiopathy in the liver and kidneys and can lead to fatal posterior cerebral edema. For many years, preeclampsia was referred to as the “disease of

Examination of the Utility of Ultrasound in Prenatal Diagnosis and Perinatal Outcomes for 13 Instances of Mirror Syndrome.

This study investigated the diagnostic utility of prenatal ultrasound in identifying mirror syndrome, drawing on the existing literature. A comprehensive series of assessments was undertaken on fetal growth and development, organogenesis, and ancillary structures, including amniotic fluid and placenta, using systematic prenatal ultrasonography in conjunction with hemodynamic evaluations. In cases where fetal edema was identified, additional chromosomal karyotyping and chromosomal microarray analyses were recommended. An analysis of 13 patients diagnosed with mirror syndrome and admitted to the

Obstructive hydrocephalus masquerading as impending eclampsia in a 37-week pregnant woman: a case report.

BACKGROUND: In preeclampsia, the neurologic symptoms, especially a lingering headache, can sometimes mimic more severe intracranial conditions, pushing doctors to quickly consider early delivery. Obstructive hydrocephalus during pregnancy is an uncommon but tricky diagnosis, one that could easily be missed when hypertension and proteinuria are already present, potentially delaying the identification of a neurosurgical emergency. CASE PRESENTATION: A 25-year-old first-time mother, 37 weeks and 1 day pregnant, presented with a month-long history of progressively worsening bilateral lower extrem

Hemorrhagic posterior reversible encephalopathy syndrome with pontine infarction in early-onset eclampsia: An atypical MRI spectrum.

Posterior reversible encephalopathy syndrome is a clinicoradiological condition characterized by reversible vasogenic edema predominantly involving the posterior cerebral regions. It is commonly associated with hypertensive emergencies and pregnancy-related hypertensive disorders such as eclampsia. Although the typical imaging pattern involves the parieto-occipital white matter, atypical manifestations including brainstem involvement, infarction, and hemorrhage have been increasingly recognized. A 26-year-old primigravida at 25 + 2 weeks of gestation presented with multiple generalized tonic-c

Acute Bilateral Vision Loss in Pregnancy: A Case of Posterior Reversible Encephalopathy Syndrome.

Posterior reversible encephalopathy syndrome (PRES) is a clinicoradiological entity characterized by headache, seizures, altered consciousness, and visual disturbances, with preeclampsia and eclampsia among its most common triggers. Although visual symptoms occur in roughly one-third of cases, acute cortical blindness as the predominant presenting feature in the absence of significant hypertension is uncommon and diagnostically challenging. We report a 27-year-old woman (gravida 2, abortion 1) at 36 weeks of gestation who presented with a one-day history of headache, a generalized tonic-clonic

Pregnancy-associated dense deposit disease with neurological complications caused by Salmonella Typhi: A case report.

BACKGROUND: Membranoproliferative glomerulonephritis (MPGN) is an uncommon cause of renal dysfunction and is increasingly being reclassified into immune complex-mediated and complement-mediated forms. Infections, particularly in endemic regions, may act as disease triggers. Pregnancy further complicates renal pathology through physiological and immunological changes. Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Practical Considerations Concerning Preeclampsia Subgroups.

Preeclampsia is one of the most serious clinical syndromes which can occur during pregnancy. According to our current knowledge, preeclampsia cannot be cured. However, a significant step forward is the recognizing preeclampsia is not a homogenous syndrome, i.e., different pathological events can lead to the hypertension + symptoms of organ damage, occurring in the second half of pregnancy. Clinically, two kinds of preeclampsia can be distinguished. The "classic" placental preeclampsia of immunological origin is characterized by contracted blood volume, fetal growth restriction, and marked alte

NFĸB and its inhibitors in preeclampsia: mechanisms and potential interventions.

Preeclampsia (PE), which affects between 2 and 15% of pregnancies, is one of the most often reported prenatal problems. It is defined as gestational hypertension beyond 20 weeks of pregnancy, along with widespread edema or proteinuria and specific types of organ damage. PE is characterized by increased levels and activation of nuclear factor kappa B (NF-κB) in the mother's blood and placental cells. This factor controls over 400 genes linked to inflammatory, apoptotic, angiogenesis, and cellular responses to hypoxia and oxidative stress. In the final stages of physiological pregnancy, NF-κB le
