BACKGROUND: Pre-eclampsia is a multisystem disorder affecting 2.8% of pregnancies in the UK. It usually presents after 20-week gestation with new-onset high blood pressure and proteinuria. Complications include eclampsia, stroke, and HELLP syndrome for the mother and preterm birth, fetal growth restriction, and stillbirth for the baby. Delivery is the only definitive cure, with antenatal care focusing on early detection and management of complications and optimising timing of delivery. Previous studies have shown calcium supplementation may reduce the risk of pre-eclampsia, but findings are dr

BACKGROUND: Severe pre-eclampsia is a major contributor to maternal and perinatal morbidity and mortality worldwide. Antihypertensive therapy is essential, yet the impact of the timing of intravenous labetalol initiation on maternal cerebral hemodynamics and clinical outcomes remains uncertain, particularly in Chinese populations.
METHODS: In this retrospective cohort study, we evaluated women with severe pre-eclampsia treated at the Department of Obstetrics and Gynecology, The First Affiliated Hospital of Soochow University, Jiangsu Province, China, between March 2023 and July 2025. Women di

HELLP syndrome, characterized by hemolysis, elevated liver enzymes, and low platelets, is a severe form of preeclampsia that can result in rare life-threatening rupture of a hepatic hematoma, causing intra-abdominal hemorrhage. This complication has been reported in up to 1.6% of HELLP syndrome cases. Here, we report a patient who developed intra-abdominal hemorrhage due to a ruptured subcapsular hepatic hematoma at 21 weeks of gestation, representing an unusually early presentation of this complication. Despite surgical treatment, the pregnancy was lost, and 5 days after discharge, the patien

Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory syndrome characterized by uncontrolled immune activation and excessive cytokine release. Pregnancy-associated HLH is particularly uncommon and presents a significant diagnostic challenge because its clinical manifestations frequently overlap with obstetric conditions such as sepsis, hemolysis, elevated liver enzymes, and low platelet count (HELLP) syndrome, and acute fatty liver of pregnancy. We report the case of an 18-year-old primigravid woman at 34 weeks of gestation who presented with persistent fever,

INTRODUCTION AND IMPORTANCE: We report two cases of parturients with HELLP syndrome who were diagnosed with subcapsular liver hemorrhage (SLH) during an emergency cesarean section.
CASE PRESENTATION: In case 1, a 30-year-old woman who was pregnant at 34+ weeks was admitted because of fetal heart rate deceleration. On the third day of hospitalization, the patient experienced upper abdominal pain. Considering the diagnosis of HELLP syndrome, an emergency cesarean section was performed under general anesthesia. Multiple SLH sites were discovered during laparoscopic exploration, but no interventi

Hypertensive disorders of pregnancy (HDPs) are a spectrum of disease contributing to immediate and lasting morbidity and mortality in pregnant patients and neonates globally. With decreasing access to obstetric care in the United States, an expected increase in patients will seek care in emergency departments. Consequently, emergency physicians will likely assume a greater share of initial care and resuscitation of patients with HDPs. HDPs represent a spectrum of disease including gestational hypertension; chronic hypertension in pregnancy; pre-eclampsia; superimposed pre-eclampsia; Hemolysis,

Hypertensive disorders of pregnancy (HDP), including chronic hypertension, gestational hypertension, pre-eclampsia, eclampsia, and HELLP syndrome, are major contributors to adverse maternal and neonatal outcomes. However, epidemiological data from the United Arab Emirates (UAE) remain limited. This study aimed to investigate the prevalence and clinical characteristics of HDP in a large tertiary center, providing evidence from an underrepresented region and a basis for future studies. This six-year retrospective cohort study (2018-2023) was conducted at a tertiary referral center in Dubai, UAE.

INTRODUCTION: Pre-eclampsia (PE) involves systemic endothelial dysfunction and microvascular injury, yet routine obstetric care lacks noninvasive readouts of maternal microvascular health. We evaluated whether hypertensive retinopathy (HR) detected during pregnancy is associated with maternal disease severity and adverse neonatal outcomes.
METHODS: We performed a retrospective cohort study of singleton pregnancies with PE and at least one ophthalmic assessment during pregnancy. Retinal findings were graded as none, mild, moderate, or severe. Primary outcomes were maternal composite adverse ou

OBJECTIVE: To assess the recurrence risk of severe hypertensive disorders of pregnancy (HDP) in a U.S. cohort because prior studies report inconsistent recurrence rates for severe HDP.
METHODS: This is a retrospective cohort study using the Premier Inpatient database. Individuals with two or more observed pregnancies between October 2015 and December 2020 were identified. Clinical characteristics were identified with International Classification of Diseases, Tenth Revision codes. The risk and relative risk (RR) of developing a severe HDP, defined as preeclampsia with severe features, superimp

Placental abruption (PA) without vaginal bleeding is known to be associated with severe outcomes when compared to symptomatic cases; the presence of hypertensive disorders of pregnancy (HDP) is an additional negative prognostic factor. According to guidelines, severe HDP are indications for prompt delivery after maternal-fetal stabilization. Considering gestational age, parity and clinical obstetric examination, the induction of labor should be prioritized to avoid additional risks associated with cesarean section. However, since only a minority of cases of PA may be detected by ultrasonograph

OBJECTIVE: Evaluate the association of APS and SLE, focusing on LN, with risk of pre-eclampsia (PE) and PE with severity criteria (PESC).
METHODS: Analysis of pregnant women admitted between 2016-2022, using data from the Spanish Hospital Discharge Database. Factors associated with PE and PESC, including early-onset PE, severe PE, eclampsia and HELLP syndrome were evaluated.
RESULTS: Among 1 973 249 admissions, 1787 patients had SLE (9.1‰), with prior LN in 13.2%. SLE patients experienced higher PE (5.5% vs 2.1%) and PESC (2.1% vs 0.6%) rates; these rose further in SLE women with LN alone (P

Background: Thrombotic thrombocytopenic purpura (TTP) is a rare but life-threatening thrombotic microangiopathy (TMA) caused by severe deficiency of the von Willebrand factor-cleaving protease ADAMTS13. Pregnancy is a recognized trigger for both immune-mediated and congenital TTP and is associated with increased maternal and fetal morbidity. Clinical overlap with other pregnancy-associated TMAs, including preeclampsia and Hemolysis, Elevated Liver enzymes, and Low Platelet count (HELLP) syndrome, often delays diagnosis. This review synthesizes current evidence on pathophysiology, diagnostic un

INTRODUCTION: Complement homeostasis is essential for healthy pregnancy and excessive complement activation can lead to devastating maternal and fetal outcomes by precipitating or worsening complement-mediated disorders. This review summarizes changes in complement regulation during pregnancy, management considerations in paroxysmal nocturnal hemoglobinuria (PNH), thrombotic microangiopathies (TMA), and antiphospholipid syndrome (APS), and emerging evidence regarding the role of complement in the severe hypertensive disorders of pregnancy (preeclampsia with severe features, HELLP syndrome).

Thrombotic thrombocytopenic purpura (TTP) is a rare yet potentially fatal thrombotic microangiopathy that poses significant risks during pregnancy. It is characterized by microangiopathic hemolytic anemia, profound thrombocytopenia, and widespread microvascular thrombosis. We present a case of pregnancy-associated TTP presenting with isolated severe thrombocytopenia, highlighting the diagnostic challenge. A 27-year-old, G3P1 woman at 31+6 weeks of gestation presented with hypogastric pain and a resolved episode of vomiting. Laboratory evaluation revealed severe thrombocytopenia (platelets: 9 ×

Thrombocytopenia affects up to 10% of pregnant women and represents the second most common blood disorder during pregnancy. Its causes include immune-mediated thrombocytopenia, hemolysis, elevated liver enzymes and low platelet count (HELLP) syndrome, preeclampsia (PE), and benign pregnant thrombocytopenia. Diagnosis is crucial because the cause dictates the effect on maternal health, pregnancy management, and neonatal outcomes. This narrative review examines the range of thrombocytopenia during pregnancy, primarily focusing on diagnostic evaluation, underlying pathophysiological causes, and d

In these two cases, we highlight the concept of pregnancy-associated thrombotic microangiopathy (TMA) overlap syndrome, where two different TMAs occur together, likely one driving another. Thrombotic microangiopathies occurring in pregnancy threaten the lives of both mother and fetus, and yet precise diagnosis may be difficult because of overlapping clinical and laboratory features. The three main syndromes to be considered are thrombotic thrombocytopenic purpura (TTP) - an acute disorder caused by severe ADAMTS13 deficiency; preeclampsia/HELLP syndrome - characterised by haemolysis, elevated

Thrombotic microangiopathies (TMAs) that occur during pregnancy or the postpartum period-including preeclampsia/HELLP syndrome (hemolysis, elevated liver enzymes, low platelets), thrombotic thrombocytopenic purpura, and atypical hemolytic uremic syndrome (aHUS)-present a diagnostic challenge owing to their overlapping clinical features. We report a case of a 21-year-old primigravida of African origin who developed HELLP syndrome followed by postpartum aHUS. The clinical course was marked by thrombocytopenia, hemolytic anemia, acute kidney injury, and massive proteinuria. Kidney biopsy revealed
