Female- and age-specific reference values for blood pressure and its relationship with cardiac aberrations after pre-eclampsia.

AIMS: Cardiovascular risk increases progressively increases from lower blood pressure (BP) values in females than males, questioning current universal thresholds for hypertension. In this study, we established female- and age-specific thresholds for hypertension and assessed its association with subclinical cardiac aberrations in a high-risk female group of women after pre-eclampsia. METHODS: A standardized cross-sectional cardiovascular assessment was performed among former pregnant women, including 30-minutes BP measurement and transthoracic echocardiography to assess cardiac function and r

Eclampsia and early readmission for cardiovascular disease.

BACKGROUND AND AIMS: Pre-eclampsia confers increased risks of long-term cardiovascular disease (CVD). However, little is known about CVD risk among patients diagnosed with eclampsia, especially in the post-partum period. The aim of this study was to determine whether patients with eclampsia are at increased risk for readmission for CVD within the first year after delivery. METHODS: Using the Nationwide Readmissions Database from 2010 to 2018, readmissions for CVD events were identified during the calendar year after delivery in patients with eclampsia. Prevalence rates of CVD readmission (per

Subclinical atherosclerosis in women after pre-eclampsia: a systematic review and meta-analysis.

BACKGROUND: Women with pre-eclampsia are at increased risk of later-life cardiovascular disease (CVD). Despite suggestions that women affected by pre-eclampsia should undergo routine CVD screening, uniform recommendations are lacking. Subclinical atherosclerosis provides a window of opportunity to identify and treat those at risk of CVD. Coronary artery calcium scoring (CAC), carotid intima-media thickness (CIMT) and ankle brachial index (ABI) are effective methods of detecting subclinical atherosclerosis. This systematic review sought to explore associations between timing of measurement and

Clinical Utility of miR-590-5p as a Biomarker for Stratifying Disease Severity and Anticipating Adverse Perinatal Outcomes in Preeclamptic Patients.

BACKGROUND: Chronic hypertension, gestational hypertension (GH), and preeclampsia (PE) together comprise hypertensive disorders complicating pregnancy. They are one of the most common complications of pregnancy. Aberrant microRNA expression may signal PE and adverse outcomes. AIMS: Investigating the clinical significance of miR-590-5p in assessing the severity of PE and predicting adverse pregnancy outcomes. METHODS: A total of 240 subjects were enrolled in this study, including 60 patients with GH, 120 patients with PE, and 60 healthy controls. The PE group consisted of 60 mild PE (MP) and

Clinical Deployment of Interpretable AI: Bridging Routine Clinical Tests and Proteomic Signatures for Preeclampsia Risk Stratification.

INTRODUCTION: Preeclampsia (PE) is the second-leading global cause of maternal mortality, affecting 5% of primigravidas. Owing to the substantial heterogeneity of clinical manifestations in PE, an urgent need arises to quantitatively evaluate the efficacy of existing diagnostic methods based on positive proteinuria (PRO) and to develop novel biomarkers to enhance diagnostic accuracy. METHODS: We based 1,215 pregnant women obtained from who delivery at the hospital in January 2018 and April 2022 and involved predictors of 66 routine clinical laboratory tests (RCLTs). In addition, from 362 peri

Utility of placental growth factor for preeclampsia prediction in pregnancies complicated by sickle cell disease.

Outside of pregnancy, placental growth factor (PlGF), is produced by erythroid cells in typically undetectable levels. In pregnancy, PlGF is strongly expressed by the trophoblast layer covering the placental villi. PlGF levels rise progressively due to placental growth, peak at 28 to 30 weeks gestation, and then slowly decline toward term. Low PlGF has emerged as a powerful diagnostic test for preterm preeclampsia. However, its interpretation in context of sickle cell disease (SCD) is potentially confounded by upregulation of cellular PlGF expression in nonpregnant individuals with SCD, and hi

Placental Steroid Hormones in Preeclampsia: Multilayered Regulation of Endocrine Pathogenesis.

The human placenta serves as the predominant endocrine organ throughout pregnancy, assuming a central role in preserving endocrine homeostasis, facilitating maternal physiological adaptation, and safeguarding fetal well-being. Preeclampsia (PE), a multifaceted and systemic gestational complication, stands as a primary contributor to maternal and perinatal morbidity and mortality. Defective placental development has been extensively acknowledged as the fundamental pathological foundation underlying this condition. Accumulating evidence has unveiled a disruption in the balance of steroid hormone

Hypertensive disorders of pregnancy, maternal cardiovascular disease mortality and the role of familial predisposition: a Norwegian population-based sibling-comparison, sibling-spillover and negative-control cohort study.

Hypertensive disorders of pregnancy (HDP) are associated with increased maternal cardiovascular disease (CVD) mortality, with risks varying by HDP subtypes and subsequent pregnancy outcomes. The contribution of shared familial factors given this heterogeneity is unclear. We conducted a population-based study using Norwegian registries (1967-2020) including 1 106 658 women with complete pregnancy histories, of whom 628 345 had at least one full sibling. Women with HDP were classified into low-risk (gestational hypertension or term pre-eclampsia followed by no HDPs) and high-risk (all other patt

Is there an increased risk of preeclampsia using donor sperm, depending on sexual condition?

OBJECTIVE: An increased risk of preeclampsia has been described when performing donor intrauterine insemination. One of the possible causes is immunological tolerance to semen. Both lesbian and single women, who have theoretically lower exposure to seminal fluid than heterosexual women requesting artificial insemination with donor, sperm should have a higher incidence of preeclampsia, although this fact has not been analyzed previously. METHODS: The population under study consisted of 439 gestations <24 weeks, achieved with artificial insemination with donor sperm, performed in two different

Pregnancy identification method as a source of bias in studies of prenatal exposures using real-world data.

Researchers typically identify pregnancies in healthcare data based on observed outcomes. This approach misses pregnancies that received prenatal care but whose outcomes were not recorded, potentially inducing selection bias in prenatal effect estimates. Alternatively, prenatal encounters can be used to identify pregnancies with unobserved outcomes, but this requires addressing loss to follow-up (LTFU). We simulated 10 000 000 pregnancies and estimated the total effect of treatment on preeclampsia. Across 36 scenarios, we varied the treatment effect on miscarriage and/or preeclampsia; percent

Maternal Apocynin During Experimental Preeclampsia Prevents BBB Permeability and Increased Vascular Tone of Cerebral Arteries in Male but Not Female Rat Adult Offspring.

OBJECTIVE: We examined if exposure to experimental preeclampsia (ePE) impacts vascular permeability and reactivity of posterior cerebral arteries (PCAs) in adult offspring, and if maternal antioxidant treatment prevents these effects in adult male and female offspring. METHODS: Offspring (F1) from rats with Normal pregnancy (NormPreg_F1), ePE_F1 and ePE treated with apocynin (ePE + apo_F1) were weighed at p24, p31, p38, and p45. Maternal apocynin was administered weekly in drinking water at a dose of ∼24 mg/kg (3 mM) on gestational days 11-20. Blood-brain barrier (BBB) permeability, structure

Associations of pregnancy complications with paternal cardiovascular risk: a retrospective cohort study.

BACKGROUND: Early cardiovascular disease risk detection opportunities are limited in men, whereas gestational diabetes, gestational hypertension and preeclampsia are risk indicators in women. We hypothesised adverse pregnancy outcomes also signal risk in fathers, due to shared environments and behaviours. METHODS: Our retrospective cohort study included fathers whose female partners had at least two singleton deliveries between April 1990 and December 2012. We examined population-based data up to April 2019 from Quebec province, Canada (health administrative databases, birth, stillbirth and d

Trends and Disparities in the Utilization of Low-Dose Aspirin for Pregnancies with Moderate or High-Risk of Preeclampsia: A National Study.

BACKGROUND AND OBJECTIVE: Preeclampsia (PE), a hypertensive disorder of pregnancy, is a leading cause of maternal morbidity and mortality globally. Low-dose aspirin (LDA) is recommended for PE prevention in high- and moderate-risk women. Despite clinical trial evidence, adherence to LDA guidelines and disparities in usage remain underexplored in real-world settings. This study assesses LDA initiation rates among high- and moderate-risk pregnant women, examining disparities by race, geography, and hospital characteristics. METHODS: We conducted a retrospective cross-sectional analysis of the U

Association between Serum Ferritin and Preeclampsia.

Preeclampsia (PE) is a common obstetrical disorder. It is a major cause of morbidity and mortality of mother as well as fetus. Its complex pathophysiology still remains unclear. Serum ferritin is an important marker of status of iron. Iron plays role as a catalyzer of oxidative stress as well as lipid peroxidation in the pathophysiology of preeclampsia. This study was conducted with an aim to explore the relation between serum ferritin and preeclampsia. This cross sectional comparative research was carried out in the Department of Obstetrics and Gynecology of Mymensingh Medical College Hospita

The Role of Epigenetics in Preeclampsia.

Preeclampsia is a multifaceted pregnancy syndrome that significantly contributes to maternal and neonatal morbidity and mortality. Characterized by hypertension, proteinuria, and multi-organ involvement, it is influenced by genetic, immunological, and environmental factors. Recent research has highlighted the role of epigenetic modifications, such as DNA methylation, histone modifications, and microRNA regulation, in the pathogenesis of preeclampsia. This study examines how these epigenetic mechanisms impact gene expression in the placenta, contributing to abnormal trophoblastic invasion, immu
