Experiences of conducting effective Patient and Public Involvement and Engagement (PPIE) by the WILL Trial (When to Induce Labour to Limit risk in pregnancy hypertension) management team.

BACKGROUND: Patient and Public Involvement and Engagement (PPIE) is an essential element of any clinical trial, to ensure that the research design and resources are acceptable and the trial produces findings that are relevant to research participants and service-users. The WILL (When to Induce Labour to Limit risk in pregnancy hypertension) Trial Management Team involved a group of patient and public members to develop an infographic, for service-users and -providers, to communicate the risks of chronic and gestational hypertension at term gestational age. METHODS: Based on national resources

Serum uric acid as a diagnostic biomarker for preeclampsia: comparison with renal indices and cerebroplacental ratio.

OBJECTIVE: To investigate the diagnostic performance of renal function-related indices (blood urea nitrogen (BUN), serum creatinine (sCr), serum uric acid (sUA), serum cystatin C (sCys C)) and the cerebroplacental ratio (CPR) for preeclampsia (PE). METHODS: In a retrospective case-control design, we reviewed the medical records of 172 pregnant women (84 with PE and 88 normotensive controls) who delivered between 2021 and 2024. Independent risk factors were identified using logistic regression, and the Receiver Operating Characteristic (ROC) curve analysis was used to assess the discriminative

Gut microbiota is different before the development of preeclampsia.

OBJECTIVES: The gut microbiota contributes to the regulation of blood pressure during and outside pregnancy. Preeclampsia (PE) is characterised by the development of hypertension along with renal, liver or other systemic complications. In women with PE, alterations in the gut microbiota composition have been reported. STUDY DESIGN: We investigated whether changes in the gut microbiota composition were present before the onset of symptoms in a group of 10 women who developed late-onset PE and 24 women who remained normotensive throughout pregnancy. Faecal samples were obtained at 28 weeks' ges

Modulatory effect of magnesium sulfate on NLRP3 inflammasome expression by human monocytes stimulated in vitro with monosodium urate.

The NLRP3 inflammasome contributes to inflammation in preeclampsia (PE), a hypertensive pregnancy disorder associated with immune imbalance. Magnesium sulfate (MgSO4), the standard therapy for eclampsia, also exhibits anti-inflammatory properties linked to intracellular calcium regulation. This study investigated the modulatory effect of MgSO4 on NLRP3 inflammasome expression in monocytes from 16 healthy non-pregnant women stimulated with monosodium urate (MSU). Monocytes were cultured in vitro with or without MSU and MgSO4. Gene and protein expression of NLRP3, caspase-1, and gasdermin D were

Association of Statin Discontinuation in Pregnancy With Maternal Cardiovascular Health and Birth Outcomes: A Nationwide Cohort Study.

BACKGROUND: Discontinuing statin therapy before pregnancy remains challenging, especially in high-risk women. We evaluated the risks of maternal cardiovascular, gestational, and fetal outcomes associated with continuing versus discontinuing statin therapy before the last menstrual period (LMP). METHODS: We conducted a nationwide cohort study using data from the National Health Insurance Database of South Korea collected between 2009 and 2023. Women who used statins for 12 to 24 weeks before their LMP between 2010 and 2022 were stratified by whether they discontinued statins before their LMP.

LIMCH1-enriched extracellular vesicles promote vascular permeability in early-onset preeclampsia.

Preeclampsia (PE) is a major pregnancy complication characterized by hypertension and multiple end-organ dysfunctions; however, its detailed pathogenesis remains unclear. Extracellular vesicles (EVs) play diverse and critical roles in intercellular communication, and we have demonstrated interaction between EVs and vascular endothelial cells. Through serum proteomic analysis, we identified LIM and calponin homology domain-containing protein 1 (LIMCH1) as a PE-associated EV protein that is highly expressed in PE placentas, particularly in syncytiotrophoblasts, which release EVs into the materna

Abnormal Pro-inflammatory Immune Cell Responses Precede Clinical Onset of Hypertensive Disorders of Pregnancy.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are significant contributors to maternal morbidity and mortality, as well as to long-term cardiovascular health, with an inequitable burden among Black individuals. While there is growing interest in the possibility that inflammatory responses are involved with HDP, work to date has primarily been cross-sectional, and immune cell profiling has focused on cell phenotyping that may not capture relevant functional properties of the immune cells. METHODS: We examined whether an abnormal inflammatory cellular profile is found in the blood of Bl

Remote electrophysiological cardiotocography (eCTG), evaluation of feasibility in complicated pregnancies from 32 until 37 weeks gestational age in a home@hospital setting (HASTA): A prospective cohort study protocol.

INTRODUCTION: High-risk pregnancies, including those complicated by pre-eclampsia, fetal growth restriction or preterm pre-labor rupture of membranes, often require hospitalization for fetal and maternal monitoring. Home monitoring may reduce the psychological and family burden, improve patient satisfaction, and lower health care costs. The primary diagnostic tool for fetal home monitoring is cardiotocography (CTG). However, conventional CTG can suffer from poor signal quality due to maternal obesity or movements. Since electrophysiological cardiotocography (eCTG) relies on electrical activity

Changes in ambulatory blood pressure monitoring during the first half of pregnancy to predict preeclampsia.

This historical cohort study assessed 253 high-risk pregnant women to determine whether early pregnancy blood pressure (BP) changes, measured via office BP (OBP) and ambulatory BP monitoring (ABPM), predict preeclampsia (PE) or adverse maternal-fetal outcomes. BP was measured twice during early pregnancy, and 8.3% developed PE, while 30.4% experienced adverse outcomes. Although BP declined significantly between evaluations, these changes were not associated with PE or complications. Adjusted logistic regression models confirmed the lack of predictive value. Findings suggest that early BP chang

Angiogenic and anti-angiogenic factors are the shared mechanistic pathways between preeclampsia and Alzheimer's disease: Perspective and take-away.

Preeclampsia (PE), a major hypertensive disorder of pregnancy, is increasingly recognized as a significant risk factor for cognitive decline and Alzheimer's disease (AD). Placental ischemia in PE leads to an anti-angiogenic state, characterized by elevated soluble FMS-like tyrosine kinase-1 (sFlt-1) and reduced placental growth factor (PlGF) and vascular endothelial growth factor (VEGF), causing systemic endothelial dysfunction. These alterations may persist during the postpartum period, promoting cerebrovascular impairment, blood-brain barrier (BBB) disruption, and neuroinflammation. Furtherm

Low molecular weight heparin (LMWH) therapy guided by low placental growth factor (PlGF) in a diabetes-prevalent population: preliminary retrospective cohort.

OBJECTIVES: Low placental growth factor (PlGF < 5th centile) is predictive of preeclampsia, fetal growth restriction (FGR) and preterm birth. In our region, diabetes during pregnancyincreasesthe risk of placental dysfunction. Preclinical and clinical studies suggest that low-molecular-weight heparin (LMWH) can increase PlGF levels. We assessed whether prophylactic LMWH was associated with improved outcomes in pregnancies with low PlGF within a diabetes-prevalent population. STUDY DESIGN: Retrospective cohort of 471 singleton pregnancies tested for PlGF between Dec 2021-Jan 2025. PlGF was clas
