Faecalibacterium prausnitzii: A microbial ally against preeclampsia via extracellular vesicle-mediated ferroptosis inhibition.

Mounting evidence highlights preeclampsia (PE) pathogenesis involves gut microbiota dysregulation and trophoblast ferroptosis. However, the mechanistic nexus bridging intestinal homeostasis and placental ferroptosis remains elusive. Here, we found that Faecalibacterium prausnitzii (F. prausnitzii) reduced blood pressure (SBP/DBP), proteinuria, and levels of anti-angiogenic (sFlt-1) and pro-inflammatory (IL-6, TNF-α) mediators in PE rats established using reduced uterine perfusion pressure (RUPP) surgery. Critically, F. prausnitzii elevated fetal survival rate, increased fetal length and weight

Brain Volumes After Hypertensive Pregnancy and Postpartum Blood Pressure Management: A POP-HT Randomized Clinical Trial Imaging Substudy.

IMPORTANCE: Hypertensive pregnancy increases risk of cognitive decline, stroke, and dementia, especially after preeclampsia. Women with prior hypertensive pregnancy show lower brain volumes, but it was unknown whether early postpartum blood pressure optimization could alter these outcomes. OBJECTIVE: To evaluate whether an intervention designed to achieve better postpartum blood pressure control after a hypertensive pregnancy is associated with differences in brain volumes around 9 months post partum compared with usual care. DESIGN, SETTING, AND PARTICIPANTS: This randomized clinical trial,

Reduced syncytiotrophoblast-glycocalyx expression is associated with placental malperfusion and the severity of late-onset preeclampsia.

INTRODUCTION: The involvement of syncytiotrophoblast-glycocalyx (STB-GCX) in placental perfusion remains unclear. In this study, we investigated whether the STB-GCX is altered in preeclampsia (PE) and whether its expression correlates with clinical indicators of hypertension and reduced placental blood flow. METHODS: In a cohort of 61 pregnant women {late-onset PE(LOPE), n = 25; uncomplicated controls, n = 36}, placental perfusion was assessed by measuring the Flow Index (FI) using three-dimensional vocal power Doppler sonography to estimate placental blood flow. Maternal circulating levels o

ZFP36 inhibits trophoblast ferroptosis and attenuates preeclampsia development by promoting NCOA4 mRNA degradation and suppressing ferritinophagy.

Preeclampsia (PE), a pregnancy-specific disorder characterized by placental dysfunction and severe maternal-fetal complications, involves ferroptosis-an iron-dependent cell death driven by oxidative stress and lipid peroxidation. This study investigated the role of zinc-finger protein 36 (ZFP36) in regulating ferroptosis in trophoblast cells and its underlying mechanisms. In vitro, hypoxia/reoxygenation was applied to human trophoblast HTR-8/SVneo cells to model ischemic injury. ZFP36 was found to inhibit oxidative stress and ferroptosis by suppressing nuclear receptor coactivator 4 (NCOA4)-med

The clinical value of nomogram combined with machine learning models in predicting the progression of hypertensive disorders in pregnancy to severe preeclampsia.

BACKGROUND: Hypertensive disorders in pregnancy (HDP) include gestational hypertension, preeclampsia, and eclampsia. Not all cases of gestational hypertension or mild preeclampsia progress to severe conditions. However, once they develop into severe preeclampsia (SPE), the risks to both the mother and the fetus increase significantly. We aimed to establish a nomogram and train a machine learning (ML) model that could identify SPE, early in the course of HDP. METHODS: In this retrospective study, 593 patients with HDP were enrolled in the training cohort. For predicting SPE early, six supervis

Metformin: An emerging therapeutic approach for prevention and treatment of preeclampsia.

Preeclampsia (PE) is a pregnancy-specific disorder that impacts 2-10% of pregnant mothers and causes significant maternal and fetal death. Clinical symptoms usually arise after 20 weeks of gestation, but evidence suggests that the pathophysiological mechanisms start much earlier. PE is caused by a combination of factors such as abnormal placentation, systemic inflammation, oxidative stress, and, most importantly, an imbalance between angiogenic mediators (VEGF and PlGF) and anti-angiogenic factors (sFlt-1 and sEng). This imbalance leads to endothelial dysfunction, decreased uteroplacental perf

Subtypes and onset of hypertensive disorders of pregnancy and cardiovascular disease within 5 years after delivery.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are established predictors of long-term cardiovascular disease (CVD), but the short-term postpartum CVD risk by HDP subtype and onset remains unclear. MATERIALS AND METHODS: We linked electronic health records with vital statistics for 755,606 singleton deliveries in Florida (2012-2017) to examine how different subtypes and onset of HDP are associated with CVD within 5 years postpartum. We classified HDP into six subtypes-chronic hypertension, gestational hypertension, mild preeclampsia, severe preeclampsia, eclampsia, and superimposed pre

Hypertensive disorders of pregnancy and related critical illnesses remain leading global causes of maternal and perinatal morbidity, and magnesium sulfate is now a cornerstone therapy across obstetric, perinatal, and critical-care settings. However, its use in practice remains heterogeneous. This narrative, practice-oriented review synthesizes mechanistic, pharmacologic, clinical, and implementation evidence on magnesium sulfate use in pregnancy and critical illness. We draw on mechanistic and pharmacokinetic studies, randomized and observational clinical research, guidelines, and quality-impr

An early-onset preeclampsia study of C19MC: DNA methylation in the spermatozoa and, DNA methylation and expression in the placental villi.

Abnormal paternal methylation of imprinted genes is associated with preeclampsia. C19MC is an imprinted miRNA cluster that plays an important role in placental development. Thus, the present study investigated methylation of C19MC in the spermatozoa and placentae and, its expression in the placentae of early-onset preeclampsia (EOPE). The study was carried out with n = 25 (controls) and 14 (EOPE cases). Methylation study of C19MC differentially methylated region (DMR) was performed by pyrosequencing of spermatozoa and placental villi samples. Expression levels of C19MC miRNAs in the placental

Preparation of placenta-tropic mRNA lipid nanoparticles for pregnancy disorders.

Lipid nanoparticles (LNPs) have garnered tremendous enthusiasm in preclinical and clinical settings for the delivery of nucleic acids such as mRNA. With applications in protein replacement therapies, vaccines and gene editing, mRNA LNPs have only recently been explored in the context of pregnancy disorders. There is a significant need for the design of novel therapeutic technologies such as mRNA LNPs to treat obstetric disorders like pre-eclampsia that are associated with placental pathology and detrimental effects on maternal and fetal health. Here, we present a step-by-step procedure for the

HIF-1α mediated placental ischemic signaling in the development of early-onset preeclampsia.

Early-onset preeclampsia is a severe placental disorder driven primarily by impaired uteroplacental perfusion and early placental ischemia. Increasing evidence identifies HIF-1α-mediated hypoxic signaling as a central regulator linking placental ischemia to downstream molecular and clinical manifestations. During early gestation, tightly controlled hypoxia guides normal trophoblast differentiation and vascular remodeling. Persistent or excessive hypoxia stabilizes HIF-1α, leading to defective trophoblast invasion, impaired spiral artery remodeling, and progressive placental ischemia. Sustained
