Impact of early diagnosis of overt diabetes on pregnancy outcomes: a retrospective cohort study among Brazilian women.

OBJECTIVE: Early diagnosis of overt diabetes has not been extensively explored. We compared pregnancy outcomes of women diagnosed with overt diabetes in the first trimester (up to the 13 completed weeks) to those with a later diagnosis. SUBJECTS AND METHODS: We evaluated women with overt diabetes and a type 2 diabetes phenotype. Pregnancy evolution (gestational weight gain, HbA1c, inpatient glycemic control) and outcomes (preeclampsia, neonatal hypoglycemia, and admission to the neonatal intensive care unit) were compared using multivariable analyses (linear regression or Poisson regression w

Maternal and cord-blood inflammatory markers and BDNF in diabetic vs. non-diabetic pregnancies.

Maternal diabetes is associated with increased systemic inflammation and has been linked to adverse neonatal outcomes, including developmental delays that persist into early childhood. In this study we sought to characterize and compare the maternal levels and fetal cord-blood levels of the inflammatory markers C-reactive protein (CRP) and IL-6, as well as the neurotrophin brain-derived neurotrophic factor (BDNF) between mothers with pre-gestational Type-1 diabetes (T1DM) or Type-2 diabetes (T2DM), and non-diabetic controls (nonDM).A prospective cohort design was employed, analyzing biomarker

Predictive value of early pregnancy laboratory markers for recurrence of hypertensive disorders of pregnancy: A retrospective study.

OBJECTIVE: Recurrence of hypertensive disorders of pregnancy (HDP) is linked to increased complications, but progress in reducing recurrence has been limited. Lasting vascular or metabolic changes after prior HDP may raise risk and could appear as subtle abnormalities in early pregnancy lab values. This study examined associations between early pregnancy labs and HDP recurrence or adverse perinatal outcomes in individuals with a history of HDP. STUDY DESIGN: We conducted a retrospective study of individuals with an index pregnancy complicated by HDP and a subsequent pregnancy with delivery >2

Does a Biochemical Approach Facilitate the Diagnosis of Prenatal Alcohol Exposure and Fetal Alcohol Spectrum Disorder in Neonatal Period?

Prenatal alcohol exposure (PAE) can cause fetal alcohol spectrum disorder (FASD). The FASD continuum encompasses facial dysmorphism, growth failure, and central nervous system (CNS) abnormalities/dysfunctions. Because some of these features may not be apparent in newborns, detecting PAE in the neonatal period is challenging, while early diagnosis may improve neurodevelopmental outcomes. Maternal self-reported alcohol consumption is limited by recall bias and denial, leading to misdiagnosis. Currently, there is a lack of universally implemented and standardized tools for identifying PAE/FASD in

Laparoscopic cervical cerclage at 10 weeks gestation in a patient with an adverse obstetric history following fertility-sparing surgery for cervical cancer.

BACKGROUND: For women who undergo fertility-sparing treatment for early cervical cancer, transabdominal cerclage (TAC) may be considered to prevent adverse obstetric outcomes due to cervical insufficiency. Laparoscopic-TAC (LPS-TAC) is now preferred over conventional transabdominal approaches because of decreased pain and bleeding, shorter hospitalisation and quicker recovery. However, a systematic, precise approach to performing LPS-TAC during pregnancy is necessary to overcome the lack of uterine manipulation and minimise complications such as bleeding and pregnancy loss. OBJECTIVES: To dem

Model-Based Cost-Effectiveness Analysis of Routine Omega-3 Testing and Targeted Supplementation to Reduce Early Preterm Birth in Australia Compared with Current Practice.

BACKGROUND: Preterm birth remains a leading cause of newborn death, disability, and long-term health challenges globally, with a substantial impact on families, the health system, and society. Omega-3 supplementation in women with low omega-3 status in early pregnancy has been shown to reduce the risk of preterm birth, particularly early preterm birth (<34 weeks of gestation). The cost implications of incorporating omega-3 testing and targeted supplementation into routine antenatal care to reduce early preterm birth in Australia are unclear. METHODS: A decision analytic model was developed us

Protein networks are influenced by maternal BMI and differentiate preterm birth types.

BACKGROUND: Preterm birth remains a leading cause of neonatal morbidity and mortality. It is classified as spontaneous, characterized by the unexpected onset of labor, or medically indicated, resulting from obstetric intervention due to pregnancy complications. The mechanisms underlying each subtype are incompletely understood, and obesity further modulates preterm birth risk through unclear biological pathways. This study aims to identify second trimester maternal plasma proteomic signatures distinguishing spontaneous and medically-indicated preterm birth and to determine how body mass index

Re-defining Care: Australia's First Outpatient Manual Vacuum Aspiration Service for the Surgical Management of Miscarriage.

BACKGROUND: Outpatient manual vacuum aspiration (MVA) has been recently implemented in Australia as an option for the management of first-trimester miscarriage. In other international settings, it has been shown to be a safe and effective alternative to suction dilatation and curettage (D&C). Our study presents the outcomes of outpatient MVA under local anaesthetic compared to suction D&C under general anaesthetic. This service is the first to be established in Australia. AIMS: To evaluate the safety and efficiency of manual vacuum aspiration curettage (MVA) as a management option for early p

Assessing the Effect of Hypothyroidism on Maternal Morbidity and Pregnancy Outcome Among Pregnant Women Attending a Tertiary Care Hospital in Central India: A Prospective Study.

Hypothyroidism is among the most prevalent endocrine disorders complicating pregnancy, leading to a broad range of adverse maternal and neonatal outcomes, especially when thyroid function is inadequately managed. Despite the significant disease burden in India, regional evidence on the impact of uncontrolled hypothyroidism on fetomaternal outcomes remains scarce. This prospective observational study evaluated the socio-demographic profile, maternal morbidities, neonatal outcomes, and influence of thyroid control status on pregnancy outcomes among hypothyroid pregnant women at a tertiary care c

Massive Puerperal Vulvovaginal Hematoma Following Vaginal Delivery: A Case Report.

Puerperal hematomas are rare but potentially life-threatening complications of vaginal delivery. They may expand rapidly, causing significant blood loss and hemodynamic instability. We report a case of a massive vulvovaginal hematoma occurring shortly after an apparently uncomplicated vaginal delivery in a 30-year-old multipara, presenting with severe perineal pain and signs of hypovolemia. Prompt surgical exploration with incision, drainage, and ligation of bleeding vessels resulted in complete recovery. This case highlights the importance of early clinical recognition and timely surgical man

Development of a pregnancy-specific physiologically based pharmacokinetic model for cadmium and estimation of fetal internal dose using serial maternal biomarkers and clinical chemistry parameters.

Cadmium (Cd) poses developmental risks during pregnancy, yet single time-point biomarkers inadequately capture cumulative fetal internal dose. We developed a pregnancy-specific semi-mechanistic physiologically based pharmacokinetic (PBPK) model to estimate fetal Cd exposure trajectories in 2778 mother-infant pairs from the Korea Children's Environmental Health Study (Ko-CHENS). The ten-compartment maternal-fetal model incorporated serial blood Cd at early (T1; gestational weeks 12-16) and late pregnancy (T3; weeks 28-34) with paired cord blood Cd, and was individually parameterized using four

Labour epidural analgesia and early childhood behavioural outcomes: the moderating role of maternal mental health and pro-inflammatory cytokines.

INTRODUCTION: Labour epidural analgesia is the most effective method for intrapartum pain relief and is associated with improved maternal outcomes. However, concerns have been raised regarding potential associations between labour epidural analgesia and adverse psycho-emotional outcomes in children. Evidence from large epidemiological studies is inconsistent and potential biological mechanisms remain unclear. Maternal immune activation during pregnancy may play a role. We aimed to investigate behavioural and psycho-emotional outcomes in children of mothers who received epidural analgesia durin

Non-immune hydrops fetalis (NIHF): diagnostic evaluation and lessons learned from a case series.

BACKGROUND: Fetal hydrops, characterized by abnormal fluid accumulation in two or more fetal compartments, is a critical condition with a poor prognosis, often resulting in significant fetal mortality. The condition itself is not a diagnosis but a clinical sign with many underlying causes, ranging from infections and chromosomal abnormalities to rare maternal conditions such as mirror syndrome. CASE PRESENTATION: This review examines three cases of fetal hydrops, highlighting common etiologies, such as infections and chromosomal disorders, and more unusual causes like maternal complications.

From guesstimates to game changers: Introducing objective measurement of blood loss for timely postpartum hemorrhage detection in Nigeria, Zambia, and Kenya.

Accurate, timely diagnosis of postpartum hemorrhage (PPH) is critical for effective management. However, in low-resource settings, reliance on visual estimation leads to missed diagnoses in up to 50% of cases, underscoring the need for more objective and reliable methods. Evidence suggests that the use of tools for objective measurement of blood loss, such as calibrated drapes, can improve early detection and prompt treatment of PPH. This article shares learning from the Accelerating Measurable Progress and Leveraging Investments for Postpartum Hemorrhage Impact (AMPLI-PPHI) project's experien

Validation of a Pregnancy-Associated Glycoprotein-Based Lateral Flow Assay for Early Pregnancy Detection in Goats.

BACKGROUND: The significant cross-reactivity between bovine, ovine, and caprine pregnancy-associated glycoproteins (PAGs) enables the adaptation of bovine-specific diagnostics for use in other ruminants; consequently, the Alertys OnFarm Pregnancy Test (AOFPT)-a blood-based lateral flow assay-provides a rapid and practical solution for pregnancy detection directly at the animal's head under field conditions. OBJECTIVES: This study evaluated to assess and validate the performance of AOFPT in goats at days 21 and 28 post-mating, by comparing the results with serum progesterone (P4) analysis and

Serum fatty acid distributions and prevalence of low omega-3 status in early pregnancy: A cross-sectional analysis of 26,389 samples from an omega-3 test-and-treat program.

BACKGROUND: Despite evidence that omega-3 supplementation reduces early preterm birth, population-level data on the distribution of fatty acids in pregnancy are limited. Statewide early-pregnancy screening data were used to describe serum fatty acid profiles. METHODS: We analysed serum fatty acid classes (saturated, monounsaturated, omega-6 polyunsaturated fatty acids [PUFA] and omega-3 PUFA) from women in South Australia's Omega-3 Test-and-Treat Program (Apr 2021-Apr 2025). Samples collected at <20 weeks' gestation were measured by gas chromatography (% total fatty acids). Total omega-3 PUFA

The amniotic fluid lactate level as a possible diagnostic tool for uterine fatigue, leading to prolonged labour.

OBJECTIVES: Arrested labour is a common and dangerous complication of delivery leading to adverse outcomes for both the mother and child. Early identification of slow labour is essential for managing the condition. Currently, clinical assessment consists of vaginal examination, abdominal palpation, and the use of a partogram, but there are no well-described, objective and quantifiable markers. One possible cause of labour arrest is uterine fatigue. The aim of the study was to assess the feasibility of using lactic acid concentration in amniotic fluid as a marker of uterine fatigue contributing

Impact of prenatal manganese and selenium exposure on childhood thyroid hormone parameters up to 4 years of age: Japan environment and children's study.

Thyroid hormones are essential for fetal and early childhood development. Manganese (Mn) and selenium (Se) are involved in thyroid hormone homeostasis; however, the effects of prenatal Mn and Se exposure on thyroid function in offspring remain unclear. We analyzed 1,773 mother-child pairs from the Japan Environment and Children's Study, measuring Mn and Se in maternal whole blood during the second or third trimester and in cord blood at delivery. Neonatal thyroid-stimulating hormone (TSH) was measured on postnatal days 4-6, whereas plasma TSH and free thyroxine (FT4) were measured at ages 2 an
