Ondansetron Use for Women With Hyperemesis Gravidarum and Risks of Spontaneous Abortion and Stillbirth: A Retrospective Cohort Study.

BACKGROUND: Ondansetron, a selective 5-hydroxytryptamine type 3 serotonin receptor antagonist, is the most prescribed medication to treat hyperemesis gravidarum in the United States. However, certain characteristics of ondansetron, such as QT interval prolongation and its ability to cross the placenta, may suggest possible fetal harm following maternal exposure. OBJECTIVE: To assess the association between ondansetron use and risk for spontaneous abortion and stillbirth in pregnant women affected by hyperemesis gravidarum. DESIGN: Retrospective cohort study. SETTING: Electronic healthcare d

Prevalence and sociodemographic factors associated with hyperemesis gravidarum in pregnant women in European Arctic Russia, 2006-2018.

BACKGROUND: Hyperemesis gravidarum is defined as intractable nausea and vomiting which occurs in 0.3–3% of all pregnancies. Data on hyperemesis in Russia are scarce. This study estimates the prevalence of hyperemesis gravidarum in pregnant women in two Arctic regions of European Russia. METHODS: A retrospective registry-based study was used. Data were collected from two population-based birth registries between 2006 and 2018. Bivariate associations between the prevalence of hyperemesis and potential determinants were studied via chi-squared tests. To assess factors contributing to the changes

YouTube as a source of (mis)information for morning sickness self-help - A content analysis and literature review of recommendations for nausea and vomiting in pregnancy.

BACKGROUND: One of the most common pregnancy complications is nausea and vomiting (NVP). Most women use the internet to search for information about pregnancy complications, which raises concerns about encountering misinformation online. OBJECTIVE: This study assessed which recommendations regarding NVP women encounter in YouTube videos, and the extent to which these recommendations are evidence-based. DESIGN AND METHOD: This was a two-phase study. First, we conducted an inductive content analysis of the 45 most frequently watched full-length English YouTube videos on the topic of NVP target

Fetal and Neonatal Complications Associated With Acute Fatty Liver of Pregnancy.

Background Acute fatty liver of pregnancy (AFLP) is a rare but life-threatening obstetric emergency that can quickly endanger both the mother and fetus, typically presenting in the third trimester. Although greater awareness and supportive management have improved maternal survival, perinatal outcomes remain poor, particularly in resource-limited settings. This study aimed to describe fetal and neonatal complications associated with AFLP and identify maternal factors contributing to poor perinatal outcomes. Methods This retrospective study analysed medical records of 73 women diagnosed with AF

Impact of CYP2D6 Metabolizer Status on Ondansetron Efficacy in Early Pregnancy Induced Nausea and Vomiting: A Case Control Study.

Real-world data investigating CYP2D6 on the efficacy of ondansetron for nausea and vomiting in pregnancy (NVP) is lacking. Evidence shows CYP2D6 ultrarapid metabolizers are at risk of ondansetron nonresponse due to increased metabolism. We conducted a retrospective cohort study of early pregnant patients on ondansetron for NVP. Genotype data for 11 CYP2D6 variants and copy number variations were translated into activity score (AS) and metabolizer status: poor (PM), intermediate (IM), normal (NM), and ultrarapid (UM) metabolizers. A total of 264 women met inclusion/exclusion criteria (99 cases

Another prospective study on the safety of ondansetron for nausea and vomiting of pregnancy: addressing ongoing concerns.

BACKGROUND: The safety of ondansetron for nausea and vomiting in pregnancy (NVP) remains a subject of ongoing debate, driven by conflicting study results and a disagreement between the European Medicines Agency (EMA) and the European Network of Teratology Information Services (ENTIS) regarding its potential fetal risks. OBJECTIVE: To assess the safety of using ondansetron for NVP and its possible association with major birth defects and neonatal outcomes. METHODS: A prospective cohort study was conducted based on structured telephone follow-ups of women with NVP who contacted the NVP Helplin

Multi-ancestry genome-wide association study of severe pregnancy nausea and vomiting.

Most pregnancies are affected by nausea and vomiting, but the most severe form-hyperemesis gravidarum-can be life threatening. Here we performed a multi-ancestry genome-wide association study of hyperemesis gravidarum in 10,974 cases and 461,461 controls across European, Asian, African and Latino ancestries. We identified ten associations: four identified previously (GDF15, IGFBP7, PGR and GFRAL) and six additional loci (SLITRK1, SYN3, IGSF11, FSHB, TCF7L2 and CDH9). Downstream analyses revealed GDF15 and TCF7L2 expression primarily in extravillous trophoblasts, with opposing effects for GDF15

Parturients Vulnerability to Hyperbaric Bupivacaine-Induced Neurotoxicity After Intrathecal Administration: A Series of Four Cases.

Spinal anaesthesia using intrathecal hyperbaric bupivacaine is widely employed in caesarean sections due to its rapid onset, effective analgesia, and minimal foetal transfer. While generally safe, neurotoxicity following intrathecal bupivacaine is rare. This case series highlights an unusual cluster of neurotoxic events observed exclusively in pregnant patients. Four parturients undergoing lower segment caesarean section (LSCS) under spinal anaesthesia with 10 mg (2 ml) of 0.5% hyperbaric bupivacaine developed neurological complications. Neurological symptoms ranging from headache, nausea, vom

Successful use of intravenous droperidol in hyperemesis following failure of sequential antiemetic therapy.

Nausea and vomiting in pregnancy is a common affliction, especially in the early trimesters. This is typically managed at home with doxylamine-pralidoxime or other home remedies. Many women, however, require additional adjuncts to control their symptoms. In this case we present a G2P0 female at 7 weeks gestation who arrived in the emergency department with persistent vomiting for several days. This patient already had a home medication regimen of multiple antiemetics. Additional treatment with ondansetron, prochlorperazine, and metoclopramide via IV administration as well as fluid administrati

The Australian MotherSafe enhanced service for nausea and vomiting in pregnancy and hyperemesis gravidarum: a mixed methods study.

BACKGROUND: Nausea and vomiting in pregnancy (NVP) and hyperemesis gravidarum (HG) can have significant physical, psychological and economic impacts on affected women and their families. With the onset of severe symptoms usually occurring in the first trimester and women in Australia typically not engaging in regular antenatal care until beyond this period, there is a gap in access to suitable models of care. In 2023, the MotherSafe teratogen information service introduced an enhanced NVG/HG service that provides medication advice for both over the counter and prescribed medications and psycho

Nausea and Vomiting During Pregnancy.

Nausea and vomiting are some of the most common symptoms during pregnancy. Severity should be assessed with a tool such as the Pregnancy-Unique Quantification of Emesis and Nausea (PUQE) score. In severe cases, additional medical history should be obtained to rule out possible secondary causes. Treatment depends on the severity of symptoms, beginning with behavioral modification such as trigger avoidance and dietary changes for mild cases. Modification includes meals that are small, frequent, bland, dry, and high in protein. First-line pharmacologic management for mild or moderate cases is vit

Pregnancy-Associated Cerebral Venous Thrombosis Presenting in the First Trimester: A Case Report.

Cerebral venous thrombosis in pregnancy in the first trimester is a very rare condition but should not be confused with the clinical presentation of other common conditions in pregnancy such as pre-eclampsia. Although it is rare and difficult to diagnose, it is potentially serious and can increase maternal mortality. Diagnosis by imaging tests such as MRI and early treatment with heparin reduces maternal and perinatal morbidity and mortality. We present the case of a 39-year-old pregnant woman at 8 weeks' gestation who presented with early onset of severe headache and vomiting; MRI with angiog

A core outcome measurement set for hyperemesis gravidarum research: a consensus study.

BACKGROUND: Outcome reporting in hyperemesis gravidarum (HG) research lacks standardization, which hampers the aggregation of data. A core outcome set (COS) for HG research has been developed to increase standardization. While a COS represents which key outcomes should be measured, it does not describe how and when they should be measured. Our aim was to develop a consensus-based core outcome measurement set for HG complementing the existing COS for HG. METHOD: This study included multiple phases: (1) systematic review to identify all existing outcome measurement instrument (OMI)s in HG resea

A prospective study of periconceptional perceived stress and rate of miscarriage.

STUDY QUESTION: To what extent is perceived stress during preconception and pregnancy associated with miscarriage incidence? SUMMARY ANSWER: Perceived stress during early pregnancy, but not preconception, was associated with higher miscarriage incidence. WHAT IS KNOWN ALREADY: Some studies have found that higher stress levels are associated with miscarriage risk. However, many of these studies were retrospective, focused on occupational stress only, and/or suffered from under-ascertainment of miscarriage. STUDY DESIGN, SIZE, DURATION: Pregnancy Study Online (PRESTO) is an ongoing prospectiv

Hyperemesis Gravidarum and the Genetics of Growth Differentiation Factor 15 (GDF-15): A Case Report.

Hyperemesis gravidarum (HG), the extreme form of nausea and vomiting during pregnancy, significantly impacts the quality of life of a subset of pregnant individuals, leading to hospitalization and severe health complications. Despite the prevalence and intensity of HG, therapeutic approaches remain largely palliative, focusing on symptom management without addressing the underlying pathophysiology. This report delineates a case of severe HG in a 31-year-old woman, highlighting the complexity of diagnosis and management, and discusses the potential of growth differentiation factor 15 (GDF-15) a
