Risk of Venous Thromboembolism in Patients with Presumed Primary Raynaud's Syndrome: A Propensity-Matched Study Using the TriNetX Database.

BACKGROUND: Raynaud's syndrome (RS) is traditionally considered a benign vasospastic disorder; however, emerging evidence suggests that it may reflect systemic vascular dysfunction and a prothrombotic state. Emerging epidemiological evidence suggests an association between RP and VTE, but further validation across different populations and analytical approaches is needed. METHODS: We conducted a retrospective cohort study with the TriNetX global electronic health record database. Adult patients diagnosed with RS between 2015 and 2024 were identified and matched 1:1 with individuals without RS

Postoperative venous thromboembolism following cesarean delivery: prevalence, pathophysiology, diagnosis, treatment, and prevention.

Venous thromboembolism contributes to approximately 10% of pregnancy-associated deaths in the United States, with the highest incidence rate in the immediate postpartum period. Cesarean deliveries are associated with 4-fold relative risk of venous thromboembolism vs vaginal delivery. While most patients have some evidence of thrombosis in the pelvic veins after cesarean delivery, only a small proportion of patients (2.6-4.3 per 1000 deliveries) develop a clinically relevant venous thromboembolism. We outline the normal physiologic process of postcesarean hemostasis and the pathways by which ab

Elucidating the protective mechanisms of umbilical cord mesenchymal stem cells against stenosis-induced deep venous thrombosis during pregnancy: a transcriptomic and metabolomic study.

OBJECTIVE: This study aims to integrate metabolomics and transcriptomics data to investigate the protective effects of umbilical cord mesenchymal stem cells (UC-MSCs) on obstetric deep vein thrombosis (DVT) and to elucidate the underlying molecular mechanisms. METHODS: A pregnant rat model of DVT was established using the inferior vena cava (IVC) stenosis method. The protective effects of UC-MSCs on DVT and endothelial cell injury were evaluated both in vivo and in vitro. Transcriptomic and metabolomic analyses were performed to identify differentially expressed genes (DEGs) and differentiall

A post-marketing surveillance study on the safety and effectiveness of recombinant antithrombin gamma in patients with congenital antithrombin deficiency in Japan.

BACKGROUND: Congenital antithrombin deficiency (CAD) is a serious inherited thrombophilia, classified as a rare disease. According to a study measuring antithrombin (AT) activity in the general Japanese population, approximately one in 650 people (0.15%) have AT deficiency. Deep vein thrombosis of the lower limbs is the most common clinical symptom. Pregnancy further increases the risk of venous thromboembolism in thrombophilia carriers. Recombinant human AT gamma (rhAT-gamma) is marketed for the treatment of thrombophilia due to CAD; however, no clinical trials for Japanese CAD patients have

The efficacy and safety of enhanced recovery after surgery (ERAS) protocols in cesarean delivery: a meta-analysis of randomized controlled trials.

BACKGROUND: Given the concerns regarding maternal and neonatal safety, the application of enhanced recovery after surgery (ERAS) in cesarean delivery remains controversial and has not been adopted in some regions. This study aims to conduct a meta-analysis to compare the ERAS pathway with conventional postoperative care, in order to provide evidence on the safety and efficacy of the ERAS protocols. METHODS: PubMed, Embase, Cochrane Library, Web of Science, CNKI, WanFang Data, and CBM were searched from their inception until February 2025. Randomized controlled trials (RCTs) published in Engli

Incidental deep vein thrombosis with concurrent subchorionic haematoma in early pregnancy: challenges and considerations.

Despite advances in perinatal medicine, deep venous thrombosis (DVT), and the associated complications, remain a leading cause of maternal morbidity and mortality, with an incidence of approximately 1:1000 pregnancies. Pregnancy adds further challenge to recognising DVT, as clinical presentation can be camouflaged by normal physiological changes. The management of DVT in pregnancy poses additional risks including bleeding, preterm labour and spontaneous abortion. In this report, the authors discuss a case of a woman in her 30s presenting at 6+3 weeks' gestation with vaginal bleeding and left i

Executive summary of the consensus document for recommendations on the diagnosis of venous thromboembolism in pregnancy. Recommendations from the Spanish Society of Internal Medicine, the Spanish Society of Medical Radiology, the Spanish Society of Vascular Interventional Radiology, and the Spanish Society of Gynecology and Obstetrics 2025.

INTRODUCTION: Pregnancy and the postpartum period increase the risk of venous thromboembolism (VTE). In pregnant women, diagnosing VTE is challenging due to the similarity between normal pregnancy symptoms and those of VTE, the physiological elevation of D-dimer levels, and the altered validity of diagnostic scoring systems. In 2019, Pregnancy-Adapted YEARS Algorithm for diagnosis of suspected pulmonary embolism was published, providing evidence on the safety and effectiveness of this diagnostic algorithm. METHODS AND RESULTS: This consensus document, developed by the Spanish Societies of Int

Recognition and Management of Pregnancy-Associated Venous Thromboembolism.

Pregnancy-associated venous thromboembolism is a leading cause of maternal mortality, particularly in the postpartum period. Thrombotic risk is heightened by physiological adaptations of pregnancy, including hypercoagulability, venous stasis, and vascular compression. Clinical signs and symptoms of pregnancy-associated venous thromboembolism such as edema and shortness of breath are frequently normal findings in pregnancy. Given the overlap of pathology and physiology, there is a low threshold for evaluation in the emergency department. Computed tomography is the gold standard for diagnosing p

Venous Thromboembolism in Pregnancy: Pathophysiology, Diagnostic Algorithms, and Therapeutic Approaches.

Venous thromboembolism (VTE), comprising deep venous thrombosis (DVT) and pulmonary embolism (PE), are frequently observed during pregnancy and may cause maternal death. During pregnancy, several physiological and anatomical changes occur that enhance thrombotic risk. In addition, this risk might be significantly increased by several patient-specific risk factors. Therefore, VTE risk assessment in all pregnant women is essential for early identification of those at increased risk so as to obtain a timely diagnosis and plan a correct therapeutic approach. Unfortunately, clinical examinations ma

Recurrent Pregnancy Loss Despite Therapeutic Anticoagulation in a Patient With May-Thurner Syndrome: A Case Report.

Pregnancy is a hypercoagulable state associated with an increased risk of venous thromboembolism. May-Thurner syndrome (MTS), an anatomic compression of the left common iliac vein by the right common iliac artery, predisposes affected individuals to left-sided deep vein thrombosis (DVT) and is often underrecognized in pregnant patients. While maternal thrombotic complications related to MTS have been described, its potential impact on fetal outcomes remains poorly characterized. We report the case of a 29-year-old gravida 7, para 2 woman with confirmed MTS and recurrent pregnancy-associated le

Thrombophilia testing in venous thromboembolism: When, who and why it matters. A practical review.

Thrombophilia encompasses inherited and acquired biological abnormalities that predispose to venous thromboembolism (VTE). Although thrombophilia testing has been available for decades, routine screening of all patients presenting with deep vein thrombosis or pulmonary embolism is neither recommended nor cost-effective. Current international guidelines support testing for clinical scenarios where results may meaningfully inform recurrence risk and thereby influence therapeutic decisions regarding anticoagulation duration, anticoagulant choice, contraceptive counselling or pregnancy management.

Risk of recurrence in patients with provoked venous thromboembolism: a prospective cohort study.

Venous thromboembolism (VTE) provoked by transient risk factors is generally considered low risk, supporting short-term anticoagulation. However, this paradigm is based on heterogeneous definitions of provoking factors. In a prospective cohort study, we evaluated recurrence risk using the International Society on Thrombosis and Haemostasis (ISTH) classification of major and minor transient risk factors. Patients with symptomatic deep vein thrombosis and/or pulmonary embolism who completed ≥3 months of anticoagulation were followed after treatment discontinuation. Patients with cancer, major th

Oral Contraceptive Use Is Associated With an Increased Risk of Venous Thromboembolism After Lower-Extremity but Not Upper-Extremity Arthroscopic Sports Procedures.

BACKGROUND: Oral contraceptive pills (OCPs) are widely used among premenopausal women and have been associated with an increased risk of venous thromboembolism (VTE) due to the prothrombotic effects of estrogen. Although arthroscopic procedures generally carry low postoperative VTE risk, it remains unclear whether OCP use differentially influences thromboembolic complications following upper- and lower-extremity arthroscopic procedures. METHODS: This study used the TriNetX Research Network to query women aged 14-40 who underwent arthroscopic shoulder/elbow or hip/knee/ankle procedures between

Executive summary of the consensus document for recommendations on the treatment of venous thromboembolism in pregnancy: consensus document from the Spanish Society of Internal Medicine, the Spanish Society of Medical Radiology, the Spanish Society of Vascular Interventional Radiology and the Spanish Society of Gynecology and Obstetrics 2026.

INTRODUCTION: Low-molecular-weight heparin is the anticoagulant treatment of choice for managing venous thromboembolism (VTE) during pregnancy and lactation. Vitamin K antagonists are contraindicated between weeks 6-12 of gestation due to their association with a specific embryopathy, and during the third trimester due to the risk of bleeding. Direct oral anticoagulants are contraindicated during pregnancy and breastfeeding because of the risk of prenatal malformations. Reperfusion techniques, such as systemic thrombolysis or mechanical thrombectomy, are reserved for cases of pulmonary embolis

Evaluation of Systemic Inflammatory Indices in Pregnant Women With Deep Vein Thrombosis.

OBJECTIVE: Venous thromboembolism (VTE) remains an important cause of maternal morbidity and mortality, yet the diagnosis of deep vein thrombosis (DVT) during pregnancy is challenging because clinical symptoms overlap with physiological gestational changes and D-dimer specificity is reduced. This study evaluated the association between complete blood count-derived systemic inflammatory indices and pregnancy-associated DVT. METHODS: This retrospective case-control study included 36 pregnant women with Doppler ultrasonography-confirmed DVT and 36 healthy pregnant controls matched for maternal a
