Chagas disease across pregnancy and early infancy: Leukoencephalomalacia and achondroplasia in two unusual cases.

BACKGROUND: Congenital Chagas disease remains an underdiagnosed cause of neonatal morbidity and mortality in endemic regions. Although most infected neonates are asymptomatic or present with nonspecific findings, severe and atypical manifestations involving the central nervous system or coexistence with genetic disorders are rarely reported. The full spectrum of neurologic injury and potential interactions between Trypanosoma cruzi infection and fetal developmental conditions remain poorly characterized. CASES: We report two unusual clinical presentations of T. cruzi infection during early in

Management of Heart transplant recipients for the non Heart Transplant health care provider. A Clinical Consensus Statement of the Heart Failure Association (HFA) and the Association of Cardiovascular Nursing & Allied Professions (ACNAP) of the ESC.

Heart transplantation (HTx) remains the gold standard treatment for selected patients with advanced heart failure and survival after transplantation has substantially improved over recent decades. As a consequence, the number of long-term HTx recipients continues to increase, and these patients are now frequently encountered in routine practice outside specialized transplant centers. Their management remains complex due to the use of chronic immunosuppression, transplant-specific complications, and a growing burden of comorbidities in the perspective of increasing longevity. Non-transplant car

Prolonged infusion of meropenem for suspected intracystic infection in polycystic kidney disease during pregnancy: a case report.

BACKGROUND: Intrarenal cyst infection in polycystic kidney disease (PKD) is hard to confirm and harder to treat during pregnancy, when lipid-soluble antibiotics are generally avoided. CASE: A pregnant woman at approximately 10 weeks' gestation presented with recurrent fever and flank pain for over one month, with acute exacerbation one day prior to admission. Laboratory tests revealed progressively elevated C-reactive protein (CRP) levels despite repeatedly negative cultures. Serial ultrasound showed a right upper-pole renal cyst that evolved to exhibit internal septations and debris. TREATM

Comprehensive Imaging Review of the Ureter.

The ureter is an important conduit between the kidney and the bladder and is frequently imaged to rule out cancer in the upper genitourinary tract. However, while malignancy can occur, a spectrum of other pathologic ureteral findings can exist, and these can be divided into congenital, intrinsic, and extrinsic abnormalities that range from postoperative and neoplastic conditions to inflammatory disease. This article provides a comprehensive overview of the ureter, including the ureteral anatomy, ureteral imaging strategies, and various ureteral conditions and their associated imaging features.

Diagnosis, diagnostic approach and challenges in pregnancy-associated AKI-the ADQI 32 consensus meeting.

Despite advancements in obstetric care, pregnancy-associated acute kidney injury (PrAKI) continues to be a significant public health concern, contributing to substantial maternal and fetal morbidity and mortality. The most common causes of PrAKI include preeclampsia/eclampsia, haemorrhage and infection. Diagnosis is influenced by gestational age and individual risk factors. Alongside traditional investigations such as imaging and kidney biopsy, novel diagnostics and biomarkers may provide valuable insights for differential diagnosis. This review aims to offer an update on the diagnostic approa

Extracorporeal Membrane Oxygenation (ECMO) for Acute Respiratory Distress Syndrome (ARDS) During Pregnancy: A Systematic Review.

Acute respiratory distress syndrome (ARDS) in pregnancy is rare but life-threatening. Extracorporeal membrane oxygenation (ECMO) has been used as rescue therapy in refractory cases, though evidence remains limited. This systematic review aimed to synthesize maternal and neonatal outcomes, describe complications, and evaluate study quality to inform clinical practice and research. A comprehensive search of MEDLINE, Embase, and Cochrane databases, through May 28, 2025, identified observational studies and case series reporting ECMO use for ARDS during pregnancy or the postpartum period. Two revi

The Oral Microbiome and Systemic Health: Current Insights into the Mouth-Body Connection.

The oral cavity contains a complex and dynamic microbial ecosystem that plays a central role in maintaining both local and systemic homeostasis. Emerging evidence indicates that disturbances in oral microbial communities-including genetic and functional diversity within species-are associated not only with oral diseases but may also contribute to the development and progression of systemic diseases. This narrative review summarises the current state of knowledge on bidirectional interactions between oral microbial communities and major organ systems. A comprehensive search of PubMed/MEDLINE, W

Endomyocardial calcification with pulmonary and systemic hypertension in a term neonate with multisystem inflammatory syndrome of newborn (MIS-N).

Multisystem inflammatory syndrome in neonates (MIS-N) is a rare condition linked to perinatal exposure to SARS-CoV-2. We report a case of a term male neonate presenting at birth with severe respiratory failure and persistent pulmonary hypertension of the newborn. Initial evaluations, including chest X-ray and ECG, were unremarkable, but echocardiography showed elevated pulmonary pressures without structural abnormalities. Over the next few days, the infant developed diffuse skin peeling, acute kidney injury and hypertension, creating a perplexing clinical picture. Only after retrospective mate

A newborn with type 1 Bartter syndrome: challenges in the treatment and development during 30 months follow-up-a case report.

Type I Bartter syndrome is a rare autosomal recessive tubulopathy resulting from mutations in the SLC12A1 gene, leading to defective sodium-chloride reabsorption in the thick ascending limb of the loop of Henle. Affected neonates typically present with profound fluid and electrolyte disturbances, polyuria, and metabolic derangements. Early diagnosis and individualized management are crucial to prevent life-threatening complications and support appropriate growth and development. We report the case of a male preterm infant born at 33 weeks' gestation following a pregnancy complicated by severe
