Systemic Lupus Erythematosus: What Every Clinician Needs to Know.

Systemic lupus erythematosus (SLE) is a chronic, multisystem autoimmune disease that disproportionately affects women of reproductive age and is associated with substantial morbidity, premature mortality, and racial and ethnic disparities. Advances in diagnosis and immunosuppressive therapy have improved survival, yet patients remain at increased risk for kidney failure, cardiovascular disease, infection, pregnancy complications, and treatment related damage. Primary care and hospital-based clinicians are often the first point of contact and are central to early recognition, evaluation, and co

Atypical hemolytic uremic syndrome triggered by acute pancreatitis.

The clinical manifestations of atypical hemolytic uremic syndrome (aHUS) are hemolytic anemia, thrombocytopenia, and acute kidney injury. It is one of the Thrombotic microangiopathy (TMA), with triggers for activation of the complement bypass pathway, such as infection and pregnancy, present in 70-80% of patients. Treatment for this disease requires plasma exchange or eculizumab, we encountered a patient with aHUS induced by gallstone-pancreatitis, after the initial plasma exchange, the patient's condition was improved, but the subsequent incomplete treatment eventually led to severe sequelae.

Optimizing ADAMTS13 prophylaxis to reduce relapse and organ failure in congenital thrombotic thrombocytopenic purpura.

Congenital thrombotic thrombocytopenic purpura (cTTP) is caused by a severe inherited ADAMTS13 (a disintegrin and metalloprotease with thrombospondin type 1 motif, member 13) deficiency. Although acute episodes are life-threatening, long-term burden of ischemic complications, and effectiveness of prophylactic strategies remain underexplored. We conducted a 25-year national, multicenter study of 88 patients with cTTP enrolled in the French Thrombotic Microangiopathy registry. Patients were stratified by age and clinical context at disease onset: pediatric (n = 42), pregnancy (n = 33), and adult

Diagnosis and treatment of complement-mediated thrombotic microangiopathies: consensus of the Genetic Diseases Committee of the Chilean Society of Nephrology.

Thrombotic microangiopathy (TMA) is a clinical-pathological syndrome defined by microangiopathic hemolytic anemia, thrombocytopenia, and organ dysfunction, commonly affecting the kidneys. The etiologies are diverse and include genetic disorders (affecting complement proteins or other pathways, such as cobalamin metabolism), infections, autoimmune diseases, malignancies, transplantation, pregnancy, and drugs. Differentiating these causes is essential, as treatment strategies and prognoses vary widely. This consensus document, developed by a multidisciplinary group of clinicians and geneticists,

Optimizing the management of congenital thrombotic thrombocytopenic purpura.

BACKGROUND: Congenital thrombotic thrombocytopenic purpura (cTTP) is a rare, life-threatening thrombotic microangiopathy caused by genetic mutations in the ADAMTS-13 gene, leading to severe enzyme deficiency. This results in the accumulation of ultralarge von Willebrand factor multimers, which trigger platelet aggregation, microangiopathic hemolytic anemia, thrombocytopenia, and organ damage. cTTP episodes are often triggered by physiological stressors and have a bimodal age of presentation. OBJECTIVES: To summarize the clinical course, complications, and advances in treatment of cTTP, highli

Unmet Needs in the Management of Adult Patients with Lupus Nephritis.

Lupus nephritis is a major cause of morbidity and long-term kidney damage in patients with systemic lupus erythematosus. Despite therapeutic advances, complete remission rates remain modest, and progress to kidney failure still occurs in a substantial proportion of patients. Contemporary management has shifted toward early initiation of 'triple therapy', yet patients continue to suffer from the adverse effects of prolonged glucocorticoid exposure. Challenges persist, including treatment duration, safe tapering, and the high rates of flare following immunosuppression withdrawal, underscoring th

Co-trimoxazole-induced reproductive toxicity and placental-barrier disruption: impact on cell-cell junctions and ERK signaling pathway.

Co-trimoxazole (CTX), a combination of sulfamethoxazole (SMZ) and trimethoprim (TMP), is used during pregnancies complicated by infections like Urinary Tract Infections (UTI) and Human Immunodeficiency Virus (HIV), despite potential fetal safety concerns. This study examines CTX's effects on the placenta using both in vivo and in vitro models. Pregnant mice received CTX for 4 days during both early and late gestation. On gestational day 19, tissues were collected for analysis. Furthermore, in vitro, human BeWo placental cells were exposed to non-cytotoxic concentrations of CTX for 24 h. Gene e

Roles of the HLA-G gene and its polymorphisms in renal transplantation.

Human leukocyte antigen-G (HLA-G) is a nonclassical major histocompatibility complex (MHC) class Ib molecule that plays a critical role in immune tolerance, primarily by promoting immune escape and inducing T-cell anergy. Early studies of HLA-G focused mainly on its function during pregnancy, where it is essential for maintaining maternal-fetal tolerance. In recent years, accumulating evidence has revealed the broader immunoregulatory functions and mechanisms of HLA-G in a variety of pathological conditions. As a key immunosuppressive molecule, HLA-G has been extensively investigated in the co

Prenatal and postnatal manifestations of WBP11-related disorder in Chinese patients: expanding the phenotypic and mutational spectrum.

BACKGROUND: Heterozygous pathogenic variants in WBP11, a spliceosome-associated gene, have recently been linked to VACTERL syndrome, yet prenatal manifestations and genotype–phenotype correlations remain poorly characterized. METHODS: Genomic DNA was extracted from fetal tissues, amniotic fluid cells, or peripheral blood samples for trio-based whole-exome sequencing (WES) to identify potential genetic etiologies. The structural stability of the WBP11 protein associated with missense variants was characterized using 3D protein structural modeling. The impact of splicing variant was further eval

Obstetric outcomes in pregnancies complicated by polycystic kidney disease.

OBJECTIVE: Polycystic kidney disease (PKD) is a genetic condition characterized by the growth of multiple renal cysts that affect kidney function. Although the disease is present from conception, individuals with the more common form of the disease, autosomal dominant PKD, often remain asymptomatic until after reproductive age. There is a paucity of large studies on PKD and obstetric outcomes; hence, the purpose of the present study is to evaluate maternal and neonatal/fetal outcomes in patients with PKD. METHODS: A retrospective cohort study was executed using the 2016-2021 Healthcare Cost U

A novel PKHD1 missense variant disrupting splicing in a fetus with Caroli disease.

BACKGROUND: Caroli disease (CD) is a rare inherited disorder characterized by dilatation of intrahepatic bile ducts, and prenatal diagnosis of this disease is extremely rare. PKHD1 is the only known causative gene, yet the pathogenicity of most missense variants remains unclear. METHODS: Exome sequencing (ES) was performed on a fetus with clinical features of CD. Candidate variants were validated by Sanger sequencing in the family. The impact of the novel missense variant on pre-mRNA splicing was assessed using minigene assays, and structural modeling of the PKHD1 protein was conducted with A

Recurrent UTIs in Children With CAKUT.

Congenital abnormalities of the kidney and urinary tract (CAKUT) encompass diverse diagnoses that commonly present as urinary tract dilatation and are the leading cause of kidney failure in children. Antenatal hydronephrosis occurs in ~1% of pregnancies; most resolve spontaneously, but conditions such as posterior urethral valves (PUV), pelviureteric junction obstruction (PUJO), vesicoureteric junction obstruction (VUJO), duplex systems, ureteroceles, and vesicoureteric reflux (VUR) require specific approaches to diagnosis and management. Postnatal surveillance relies predominantly on renal tr

Catastrophic Outcome of Pregnancy Complicated With Tricuspid Valve Nonbacterial Thrombotic Endocarditis.

Nonbacterial thrombotic endocarditis (NBTE) is a rare condition characterized by sterile fibrin-platelet vegetations on cardiac valves, typically associated with hypercoagulable states. Isolated tricuspid valve (TV) involvement during pregnancy is exceedingly rare, with no previously reported cases of fatal maternal and fetal outcome in the published literature. We report a 31-year-old woman (gravida 3) with a complex medical history including human immunodeficiency virus infection, chronic kidney disease, recurrent venous thromboembolism, and thrombocytopenia, who presented at 9 weeks of gest

Analysis of energy expenditure and behavioral characteristics in different mouse strains under normal and disease conditions.

BACKGROUND: Metabolic and behavioral traits in mice exhibit substantial variability across strains, ages, and disease states, which can confound preclinical study outcomes. However, systematic characterization of these parameters (e.g., energy metabolism, activity, and feeding behavior) across diverse experimental conditions remains limited. METHODS: Core metabolic and behavioral parameters were assessed via metabolic cages in five 8-week-old mouse strains (ICR, C57BL/6J, BALB/c, BALB/c-nude, NOD-SCID), six C57BL/6J age groups (3 weeks to 18 months), and four mouse disease models (LPS-induced
