Gestational diabetes mellitus (GDM) is an increasing public health challenge in Africa, contributing substantially to maternal and neonatal morbidity. Evidence from our systematic review, meta-analysis, and bibliometric analysis shows that inconsistent screening practices, reliance on selective rather than universal testing, and limited health system capacity contribute to underdiagnosis and poor pregnancy outcomes. GDM is associated with adverse maternal and neonatal complications and increases the long-term risk of type-2 diabetes for mothers and offspring. Our bibliometric findings reveal u

Gestational diabetes (GDM) in pregnant women leads to the development of macrosomia in newborns and also influences the characteristics of neonatal length development. It is worth noting that findings regarding the association between GDM and gestational age at birth vary, and there is no evidence to support treating GDM as an independent risk factor for determining gestational age at birth. The aim of this study was to determine the impact of GDM on neonatal physical parameters, including birth weight and length, as well as gestational age at birth. Data were collected using a standardized qu

INTRODUCTION: Pregnancy imposes a sustained hemodynamic load that can unmask latent cardiovascular vulnerability, yet distinguishing physiological adaptation from early maladaptive remodeling remains difficult with conventional echocardiography.
AREAS COVERED: This special report reviews trimester-specific cardiac adaptation, myocardial deformation imaging, right ventricular-pulmonary arterial coupling, postpartum reverse remodeling, and the maladaptive phenotypes of preeclampsia, gestational diabetes, and peripartum cardiomyopathy. It also covers circulating biomarkers and the risk framework

Coronary artery calcium (CAC) scoring is selectively used to refine cardiovascular risk assessment in primary prevention, but interpretation still relies largely on absolute thresholds applied similarly to women and men. This approach may overlook relevant sex-related differences in coronary atherosclerosis. Women tend to develop detectable CAC later in life and have lower absolute CAC values for a given age, while sex-related differences in coronary plaque composition may not be fully captured by calcium scoring alone. Consequently, a low absolute CAC score may still represent a high age- and

OBJECTIVE: An increasing number of women of childbearing age worldwide are using glucagon-like peptide-1 receptor agonists (GLP-1 RAs) and are advised to use effective contraception during treatment. Nevertheless, maternity services are encountering increasing numbers of women exposed to GLP-1 RAs shortly before conception or during pregnancy. The true extent of exposure is likely underestimated because routine enquiry is not universal. The effects of GLP-1 RA exposure on pregnancy remain poorly understood, and there is currently limited guidance on counselling women with inadvertent periconce

BACKGROUND: Preeclampsia is a leading cause of maternal and neonatal morbidity and mortality worldwide, yet effective pharmacological preventive strategies remain uncertain. Statins, metformin, and vitamin D, alone or combined with omega-3, have been investigated for potential protective effects, but comparative evidence is limited.
OBJECTIVE: To compare the effectiveness of statins, metformin, and vitamin D in preventing preeclampsia and related adverse outcomes among high-risk pregnant women.
METHODS: A systematic review and network meta-analysis of randomized controlled trials was conduct

BACKGROUND: Gestational diabetes mellitus (GDM) complicates roughly one in seven pregnancies and predicts later maternal type 2 diabetes mellitus (T2DM). Yet its pharmacological management still rests on a handful of agents (insulin, metformin and glibenclamide), none of them selected on the basis of the disease's molecular genetics. Each emerged from biological and clinical observation long before the genetics of diabetes was known.
AIMS: This review advances a single contention: that GDM is largely T2DM expressed under the metabolic stress of pregnancy. It then examines what this implies fo

INTRODUCTION: Women with pregestational diabetes mellitus (PGDM) are at increased risk of preeclampsia (PE) and are commonly prescribed aspirin prophylaxis based on clinical risk factors alone. First-trimester screening algorithms may allow a more individualized allocation of aspirin. We aimed to compare the association between three first-trimester aspirin allocation strategies and the incidence of preterm PE in women with PGDM in routine clinical practice.
MATERIAL AND METHODS: This retrospective multicenter observational cohort study included 519 singleton pregnancies with PGDM managed at

Neonatal macrosomia is a major perinatal complication associated with maternal metabolic disorders. This study aimed to assess whether hemoglobin concentration and gestational diabetes mediate the association between pre-pregnancy body mass index and neonatal macrosomia. A retrospective analysis was conducted among 700 singleton full-term pregnancies at El Idrissi Provincial Hospital, Kenitra, Morocco. Macrosomia was defined as birth weight ≥4000 g, and elevated hemoglobin as ≥12.5 g/dL. Gestational diabetes was diagnosed using fasting glucose or a 75-g oral glucose tolerance test. Mediation a

Hypertensive disorders during pregnancy and postpartum are major causes of maternal morbidity and mortality, yet associations with social determinants of health, including neighborhood deprivation, remain understudied. We examined the association of periconceptional neighborhood deprivation with the risk of gestational and postpartum hypertensive disorders in a population-based cohort of 281,621 pregnant individuals aged 15-44 years without chronic hypertension who delivered at Kaiser Permanente Northern California from 2011 to 2020. Neighborhood deprivation index (NDI) quartiles were assigned

This study investigated the relationships between specific biological factors and the risk of gestational diabetes mellitus (GDM). A retrospective case-control study was conducted in pregnant women with GDM and in healthy pregnant women who served as a control group. ABO and Rh blood groups, maternal age, pre-pregnancy and third-trimester body mass index (BMI), and first-trimester thyroid-stimulating hormone (TSH) and free thyroxine (FT4) levels were analyzed in both groups. Associations were assessed using the chi-square (χ²) test, together with odds ratios (OR) and 95% confidence intervals (

BACKGROUND: Gestational diabetes mellitus (GDM) is a common pregnancy complication. Although lipid parameters, the triglyceride-glucose (TyG) index, and TyG-body mass index (TyG-BMI) have been associated with GDM, their relative value within an integrated lipid-glycaemic prediction framework remains unclear. This study developed and internally validated an interpretable model for GDM risk estimation.
METHODS: This single-centre retrospective study included 287 pregnant women who underwent 75 g oral glucose tolerance testing and fasting lipid assessment at 24-28 gestational weeks, including 72

Gestational diabetes mellitus (GDM) is commonly diagnosed at 24-28 weeks of gestation; however, metabolic susceptibility that precedes dysglycemia may emerge earlier. We examined whether the rate of early pregnancy body mass index (BMI)-derived estimated body fat percentage (eBFP) change was associated with subsequent GDM, and whether this association differed according to an early hepatic-metabolic phenotype defined by the natural log-transformed fatty liver index (FLI_Ln). This secondary analysis used de-identified data from a prospective Korean pregnancy cohort and included 583 singleton pr

BACKGROUND: Gestational diabetes (GDM) is frequently associated with adverse neonatal outcomes, but the underlying mechanisms linking maternal hyperglycemia to fetal harm are not fully understood. Oxidative stress has been suggested as a key pathway; however, studies simultaneously evaluating multiple oxidative and antioxidant biomarkers in umbilical cord blood are limited. This study aimed to investigate the impact of GDM on the fetus by comprehensively evaluating oxidative stress and antioxidant defense mechanisms in umbilical cord blood and to examine their potential associations with neona

Polyendocrine metabolic ovarian syndrome (PMOS), formerly known as polycystic ovary syndrome (PCOS), is a multisystem endocrine-metabolic disorder that significantly increases the risk of adverse pregnancy outcomes, partly independently of obesity. PMOS patients frequently exhibit a characteristic metabolic-endocrine milieu of insulin resistance, hyperandrogenemia, dyslipidemia, and chronic low-grade inflammation. The physiological insulin resistance of pregnancy, driven by placental hormones, exacerbates these pre-existing disturbances and disrupts endometrial receptivity and placental functi
