Maternal and neonatal outcomes after prophylactic and ultrasound-indicated emergency cervical cerclage.

OBJECTIVES: To evaluate maternal and neonatal outcomes after prophylactic and ultrasound-indicated emergency cervical cerclage. METHODS: This retrospective cohort study included all the women who underwent McDonald cervical cerclage at a tertiary medical center during 2010-2024 and delivered at ≥23 weeks. Women were categorized according to the clinical timing and urgency of cerclage placement: prophylactic cerclage, placed electively based on obstetric history before cervical change was detected; and ultrasound-indicated emergency cerclage, performed for ultrasound-detected cervical shorteni

Hypertensive Disorders of Pregnancy Subtypes and Long-Term Cardiovascular Risk.

IMPORTANCE: Hypertensive disorders of pregnancy (HDPs) are associated with an increased long-term risk of cardiovascular disease, but the risks across different HDP subtypes, particularly those other than preeclampsia, remain unclear. OBJECTIVE: To examine whether the risk and distribution of specific cardiovascular outcomes differ across HDP subtypes. DESIGN, SETTING, AND PARTICIPANTS: This nationwide cohort study retrospectively analyzed women with deliveries in South Korea from 2010 to 2018 using the National Health Insurance Service database. HDPs were classified into 5 subtypes: chronic

Complete glucocorticoid resistance: a lethal disorder in the neonatal period.

Generalized glucocorticoid (GC) resistance (Chrousos syndrome) results from impaired GC receptor signaling due to variants in the NR3C1 gene. Severe neonatal presentations are extremely rare. We report an 8-week-old male infant born at 31-week gestation, with progressive respiratory failure, refractory hypotension, hypokalemic alkalosis, anasarca, jaundice, and multiorgan dysfunction. Laboratory evaluation demonstrated persistently elevated serum cortisol [>60 µg/dL (SI: >1655 nmol/L) (reference range 2.5-9.1 µg/dL, SI: 69.0-251.0 nmol/L)], adrenocorticotropic hormone [>2400 pg/mL (SI: >528 pm

Multimodal Monitoring in Eclamptic Intracerebral Hemorrhage Complicated by Reversible Cerebral Vasoconstriction Syndrome (RCVS): A Case Report.

BACKGROUND: Eclampsia is a rare but severe obstetric condition associated with maternal morbidity and mortality. Neurological complications are rare and often compounded by systemic involvement such as HELLP syndrome and coagulopathy. In this complex setting, multimodal neuromonitoring may support individualized management. CASE: A 32-year-old woman at 37 weeks' gestation presented with new-onset tonic-clonic seizures, severe hypertension, and laboratory findings consistent with HELLP syndrome and disseminated intravascular coagulation. Following emergency cesarean section, brain CT revealed

Dysregulation of the Heparanase/TFPI-2/Heparan Sulfate Axis in Recurrent Pregnancy Loss.

Recurrent pregnancy loss (RPL) is a clinically heterogeneous reproductive disorder, and in many women the underlying mechanism remains unexplained by standard thrombophilia evaluation. Pregnancy is characterized by a tightly regulated hemostatic balance at the maternal-placental interface. Heparanase, tissue factor pathway inhibitor 2 (TFPI-2), and heparan sulfate (HS) are expressed in placental and hemostatic pathways and may contribute to coagulation-related pregnancy complications. We investigated whether the heparanase/TFPI-2/HS axis is altered in women with RPL. Blood samples were obtaine

Fatal air embolism associated with HELLP syndrome following first-trimester pregnancy termination: An autopsy-based case report.

In forensic practice, sudden maternal deaths following obstetric procedures require meticulous postmortem investigation to establish the cause of death. Although pregnancy termination is generally considered a safe procedure, rare but potentially fatal complications may occur. Hemolysis, Elevated Liver enzymes, and Low Platelet count (HELLP) syndrome and venous air embolism are individually uncommon but life-threatening conditions. A 23-year-old woman (gravida 2, para 1) at 9 weeks and 6 days of gestation underwent an uncomplicated elective pregnancy termination following normal preoperative l

Idiopathic Intracranial Hypertension in a Child with Marfan Syndrome: Clinical, Neuroimaging, and Biomarker Findings from a Case Report.

Marfan syndrome (MFS) is a connective tissue disorder classically associated with cardiovascular, musculoskeletal, and ocular manifestations. Neurological involvement is increasingly recognized and is most commonly related to spontaneous intracranial hypotension secondary to dural ectasia and cerebrospinal fluid (CSF) leakage. By contrast, idiopathic intracranial hypertension (IIH) is exceptionally rare in pediatric patients with MFS. We report the case of an 8-year-old girl with Marfan syndrome presenting with neck pain, diplopia, bilateral papilledema, and bilateral sixth cranial nerve palsy

Recurrent pregnancy loss and thyroid autoimmunity: A review of mechanisms and clinical management.

As a major challenge currently plaguing reproductive medicine, recurrent pregnancy loss(RPL) has a variety of causes. Thyroid autoimmunity(TAI) is a prevalent thyroid disorder among women of childbearing age, potentially impacting maternal physiological changes and the balance of the maternal-fetal interface during pregnancy through endocrine and autoimmune pathways, which can lead to abortion and RPL. The selection of therapeutic agents and the effectiveness of treatment for patients experiencing RPL with thyroid autoimmunity remain contentious. Current research have explored the efficacy of

Chronic liver disease questionnaire analysis in pregnant women with hepatitis B: a prospective study.

BACKGROUND: Although domestic and international guidelines increasingly emphasize preventing mother-to-child transmission (MTCT) of hepatitis B virus (HBV), the multidimensional impacts of HBV infection on pregnant women's health-related quality of life (HRQoL) have received insufficient attention. Our previous 1:1 matched cohort study [1] identified HBV infection as an independent risk factor for impaired HRQoL in pregnant women, after adjusting for age, gestational age, and parity. Building upon this foundation, this study is a longitudinal descriptive investigation within the population of

Monospecific antibodies reveal reduction of granulins in frontotemporal dementia caused by GRN mutations.

PURPOSE: Primary open-angle glaucoma (POAG) is a common ocular disease, associated with abnormalities in aqueous humor circulation and an increase in intraocular pressure (IOP), leading to progressive optical neuropathy and loss of vision. POAG pathogenesis includes alterations of the structural properties of the sclera, especially in the optic nerve head area, contributing to the degeneration of the retinal ganglion cells. Abnormal sclera biomechanics hinder adequate compensation of IOP fluctuations, thus aggravating POAG progression. The proteomic basis of biomechanical disorders in glaucoma

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Whole-Exome Sequencing in Recurrent Pregnancy Loss: Current Evidence and Future Directions in Saudi Arabia.

BACKGROUND: Recurrent pregnancy loss or RPL is a controversial subject in reproductive medicine. Although we perform clinical tests and cytogenetic analyses, we are often left puzzled. Whole-exome sequencing (WES) is changing this trend, whose usage has helped discover inherited and new genetic changes related to miscarriage. OBJECTIVE: This review summarizes international experience of using WES in RPL diagnostics and genetics, particularly from Saudi Arabia and other consanguineous populations. METHODS: Based on PRISMA protocols, database searches were performed in PubMed, Scopus, Embase,

Unveiling the molecular mechanisms of recurrent miscarriage through endoplasmic reticulum stress related gene expression.

Recurrent miscarriage (RM) is a reproductive disorder affecting couples worldwide. The underlying molecular mechanisms remain elusive, even though emerging evidence has implicated endoplasmic reticulum stress (ERS). We investigated RM- and ERS-related genes to develop a diagnostic model that can enhance predictive ability. We utilized the R package GEO query to extract and process Gene Expression Omnibus data, applying batch correction, normalization, and differential gene expression analysis with limma. ERS-related differentially expressed genes (ERSRGs) were identified through Gene Ontology

Associations of Th1, Th2, Th17, and Treg cell levels and imbalance with recurrent spontaneous abortion: A meta-analysis.

OBJECTIVE: Abnormal levels and imbalances of T cell subsets are common in recurrent spontaneous abortion (RSA) patients, but most studies have small sample sizes, and comprehensive evaluations are lacking. Therefore, this meta-analysis aimed to comprehensively investigate T cell subsets and their ratios in RSA patients. METHODS: Four databases (PubMed, EMBASE, Web of Science, and Cochrane Library databases) were searched until 10 January 2024. T helper 1 (Th1), Th2, Th1/Th2 ratio, Th17, regulatory T (Treg), Treg/Th17 ratio, and the correlation between Th17 and Treg were considered endpoint in

Knights in shining armour and (M)others in life jackets: Women's experiences of advocating for care alone when suffering recurrent early pregnancy loss during the SARS-CoV-2 pandemic.

BACKGROUND: Recurrent early pregnancy loss [rEPL] is a traumatic experience, marked by feelings such as grief and depression, and often anxiety. Despite this, the psychological consequences of rEPL are often overlooked, particularly when considering future reproductive health or approaching subsequent pregnancies. The SARS-CoV-2 pandemic led to significant reconfiguration of maternity care and a negative impact on the perinatal experience, but the specific impact on women's experience of rEPL has yet to be explored. This study aimed to examine the impact of changes to early pregnancy loss care

Silencing TGF-β3 Alleviates Recurrent Spontaneous Abortion Inflammation in Mice: the Importance of Treg/Th17 Cell Balance.

BACKGROUND: Recurrent spontaneous abortion (RSA), also known as repeated miscarriage, refers to the consecutive loss of pregnancy three times or more before the fetus reaches viability. In this study, we aimed to investigate the impact of transforming growth factor β3 (TGF-β3), which plays a crucial role in immune dysregulation, on the imbalance of regulatory T cell (Treg) and T helper 17 (Th17) cells. METHODS: First, we isolated T cells from an RSA mouse model we established in-house. Tregs and Th17 cells were labeled by targeting forkhead box protein P3 (Foxp3) and retinoic-acid-receptor-γt
