Twin pregnancies in women with heart disease are complicated by high risk of heart failure: data from the EORP ROPAC Registry.

BACKGROUND: Cardiovascular adaptation to pregnancy is more marked in twin than singleton pregnancies and may result in a higher rate of adverse cardiac events in women with heart disease. The objective was to test the hypothesis that women with heart disease and a twin pregnancy have a higher rate of adverse cardiac events. METHODS AND RESULTS: Registry Of Pregnancy And Cardiac disease (ROPAC) is a prospective (2007-2018), global registry of pregnant women with heart disease. Pregnancy outcomes in twin (n=96) were compared with 5643 women with singleton pregnancies. At baseline, twin mothers

Preeclampsia Prediction by Machine Learning in Twin Pregnancies.

INTRODUCTION: While machine learning models successfully predict preeclampsia in singleton pregnancies and aspirin prophylaxis prevents preterm preeclampsia, no parallel models exist for twin pregnancies. This study developed machine learning algorithms to predict preeclampsia in twins using maternal factors and biomarkers from all three trimesters. METHODS: We prospectively enrolled 596 pregnant women with twin pregnancies at 11+0 to 13+6 weeks' gestation. Machine learning models assessed the efficacy of maternal factors and biomarkers for preeclampsia prediction across all trimesters. Scree

The influence of discordant growth during pregnancy in dichorionic twins on adverse birth outcomes.

OBJECTIVE: To investigate the influence of growth discordance in different trimesters of dichorionic twins on frequent adverse birth outcomes (preterm birth (PTB), low birth weight (LBW), and small for gestational age (SGA)). METHODS: Based on the Hunan Maternal and Child Health Hospital (HMCH) twin pregnancy cohort. Participants with dichorionic twin pregnancies who received prenatal care and delivered at HMCH between January 2019 and December 2024 were divided into four groups: whole-pregnancy concordant group (n = 428), isolated third-trimester discordant group (n = 30), isolated first-tri

Diagnosis of Amniotic Sac Herniation Into Uterine Dehiscence Using Combined Ultrasound and Magnetic Resonance Imaging-A Rare Complication Following Fetoscopic Laser Photocoagulation.

A 27-year-old primigravida with monochorionic diamniotic (MCDA) twin pregnancy underwent fetoscopic laser photocoagulation at 17 + 1 weeks' gestation for Quintero stage II twin-twin transfusion syndrome (TTTS). On postoperative day 20, transabdominal ultrasound identified a cystic anechoic structure adjacent to the anterior uterine wall. Subsequent magnetic resonance imaging (MRI) demonstrated a 2-mm fistulous tract extending from the recipient twin's amniotic sac to the extrauterine cavity, consistent with amniotic sac herniation. Parental concerns prompted induced labor at 23 weeks, with fol

Clinical Outcomes of Twin Reversed Arterial Perfusion (TRAP) Sequence: A Retrospective Analysis From a Tertiary Center.

Twin Reversed Arterial Perfusion (TRAP) sequence is a rare complication of monochorionic twin pregnancies characterized by reversed blood flow from the pump twin to a nonviable acardiac twin. This study aims to share the clinical outcomes and procedural details of TRAP sequence cases managed at our tertiary fetal therapy center, with emphasis on various intrauterine treatment modalities. We conducted a retrospective review of 20 pregnancies diagnosed with TRAP sequence and managed at a tertiary fetal therapy unit between January 2018 and July 2025. Diagnosis was confirmed via prenatal ultrason

Twin pregnancies are risk factors for both early- and late-onset hypertensive disorders of pregnancy: the Japan Environment and Children's study.

This study investigated the associations of twin pregnancies with early-onset (EO)- and late-onset (LO)-hypertensive disorders of pregnancy (HDP). Totally, 86,717 pregnant women were included in a prospective birth cohort study. The associations of dichorionic diamniotic (DD)- and monochorionic diamniotic (MD)-twin pregnancies with EO-HDP (Diagnosed from 20 to <34 weeks of gestation) and LO-HDP (Diagnosed at ≥34 weeks of gestation) were analyzed using a multinomial logistic regression model. Compared with singleton pregnancies, both DD- and MD-twin pregnancies had significantly higher odds for

Associations of exposure to PM2.5 and its constituents during pregnancy with the risk of premature rupture of membranes in twin pregnancies.

PM2.5 and its constituents are linked to various comorbidities and complications during pregnancy. However, evidence remains limited regarding the association of total PM2.5 mass and PM2.5 constituents with premature rupture of membranes (PROM) in twin pregnancies. Accordingly, contributing new insights to this field is of significant value. In this study, we employed logistic regression, restricted cubic spline regression, weighted quantile sum, quantile g-computation, Bayesian kernel machine regression, and subgroup analyses to quantify the individual and joint associations of PM2.5 and its

Risk Factors and Pregnancy Outcomes of Twin Pregnancies With Gestational Diabetes Mellitus: A Comparison Based on Chorionicity.

BACKGROUND: There is insufficient evidence to determine whether the risk factors and pregnancy outcomes associated with gestational diabetes mellitus (GDM) in twin pregnancies vary by chorionicity. MATERIALS: A retrospective cohort study was conducted among twin pregnancies. GDM was diagnosed using the IADPSG diagnostic criteria. Logistic regression and generalized estimation equation (GEE) models were used to identify the risk factors of GDM and its impact on pregnancy outcomes, stratified by monochorionic (MC) and dichorionic (DC) pregnancies. RESULTS: Advanced maternal age (MC: aOR 2.18,

Low PAPP-A levels and their association with adverse perinatal outcomes in twin pregnancies.

PURPOSE: While in singleton pregnancies the maternal serum biomarkers used in the first-trimester screening for aneuploidies, particularly PAPP-A, may be, also, used as predictors of adverse perinatal outcomes, there is a scarcity of data regarding the association of first-trimester biomarkers with unfavorable pregnancy outcomes in twin pregnancies. The main purpose of our study was to evaluate the association of low PAPP-A levels in twin pregnancies with the subsequent development of perinatal complications. METHODS: 454 twin pregnancies were recruited over a period of 11 years, and their da

[Reference Values for Sonographic Estimated Fetal Weight and Biometry in Twin Gestations - A Nationwide Cohort].

OBJECTIVES: Twin gestations are subjected to higher rates of growth abnormalities. Accurate, locally customized reference values may contribute to an accurate diagnosis of abnormal growth. We aimed to determine reference values for sonographically estimated fetal weight (sEFW) and biometry in twin gestations. METHODS: A multicenter retrospective longitudinal analysis of sEFW and biometric measurements evaluations of twin gestations was performed between 2010 and 2022 in 11 medical centers in Israel. The sEFW at 14-40 gestational weeks was calculated using the Hadlock 1985 formula. Only data f

Prenatal Diagnosis of Peters-Plus Syndrome: A Case Report.

Peters-Plus syndrome is a rare autosomal recessive disorder caused by biallelic pathogenic variants in the B3GLCT gene and characterized by multisystem involvement. Fewer than 100 cases have been reported to date, and only a limited number have been diagnosed prenatally. Prenatal identification is challenging due to the variable and non-specific nature of fetal findings and the frequent absence of detectable ocular anomalies during routine ultrasound. We report a prenatal diagnosis of Peters-Plus syndrome in a monochorionic diamniotic twin pregnancy, based on the progressive identification of

Low PAPP-A Levels and Growth in Twin Pregnancies.

BACKGROUND/OBJECTIVES: It is well established in the modern literature that newborns delivered from multiple gestations are more predisposed to low birthweight in comparison to their singleton equivalents. In this study, we sought to explore the potential of first-trimester biochemical (PAPP-A and free β-hCG) and biophysical indices (uterine artery Doppler) to predict low birthweight in one or both twins. METHODS: This is a retrospective cohort analysis of 400 twin viable pregnancies presenting for routine first-trimester assessment in four fetal medicine centers between 2014 and 2025. The ex
